单独应用斜外侧椎间融合术治疗L4-5腰椎疾病

本研究描述了斜外侧椎间融合术在腰椎脊柱手术中的应用。

摘要

腰椎疾病常引起腰痛、下肢疼痛、麻木和感觉异常。在严重情况下,可能出现间歇性跛行,影响患者的生活质量。当保守治疗无效或患者症状难以忍受时,通常需要手术治疗。手术方式包括椎板切除术、椎间盘切除术以及椎体间融合术。椎板切除术和椎间盘切除术的主要目的是缓解神经压迫;然而,由于脊柱不稳,术后复发较为常见。椎体间融合术在缓解神经压迫的同时改善了脊柱稳定性,与非融合手术相比显著降低了复发风险。然而,传统的后路椎间融合术需要剥离肌肉以暴露手术节段,给患者带来较大创伤。相比之下,斜外侧椎体间融合(OLIF)技术能够以最小的创伤实现脊柱融合,并缩短患者的恢复时间。本文介绍了腰椎独立OLIF手术的操作步骤,为其他脊柱外科医生提供参考。

引言

腰椎疾病是一个全球性的经济问题,患者的生活质量显著下降1。腰椎疾病的治疗可分为两类:保守治疗和手术治疗。保守治疗包括休息、口服非甾体类药物、按摩和物理治疗。例如,研究表明雌激素可用于延缓椎间盘退变,从而为腰椎疾病的治疗提供依据2,3。对于保守治疗无效的患者,通常需要手术治疗腰椎疾病。在针对此类患者的手术方法中,椎体间融合术通常是首选。

简而言之,所报道的技术包括前路腰椎椎间融合术(ALIF)、直接侧方腰椎椎间融合术(DLIF)、极外侧腰椎椎间融合术(XLIF)、斜外侧腰椎椎间融合术(OLIF)、后路腰椎椎间融合术(PLIF)以及经椎间孔腰椎椎间融合术(TLIF)4,5。在这些手术入路中,OLIF 具有其独特的优点。与 DLIF 和 XLIF 相比,OLIF 已被证明可降低术后髋关节屈曲无力和大腿麻木的发生率6,7。此外,与 ALIF 相比,OLIF 手术在术后逆行射精及椎体前方血管损伤方面的风险更低8。然而需要指出的是,PLIF 和 TLIF 技术具有良好的手术视野,有助于减少对重要结构的损伤;这些方法所使用的椎弓根螺钉固定可降低椎间融合器不融合的概率。但与 OLIF 相比,PLIF 和 TLIF 需要在术中进行肌肉分离并长时间牵拉肌肉,可能导致术中出血量增加、伤口愈合缓慢以及患者恢复时间延长9,10,11

通过采用OLIF技术,可经由腰大肌前缘与腹部主要血管之间的间隙到达椎间隙。手术通过切除病变椎间盘并植入椎间融合器来完成。由于对肌肉的损伤较小,OLIF可最大限度地减少术中出血量和手术时间,缩短患者的术后恢复周期。此外,既往研究显示,植入椎间融合器有助于恢复椎间隙高度以及患者脊柱的生理曲度6,12,13,14,15,16。本研究详细介绍了单独应用OLIF治疗L4-5节段腰椎疾病的手术操作步骤。

方案

本研究已获得河北医科大学第三医院伦理委员会的批准。患者签署了知情同意书,同意被拍摄,并允许研究人员使用其手术数据。

1. 患者选择

  1. 根据以下纳入标准选择患者。
    1. 选择保守治疗时间超过3个月且治疗效果不佳的患者。
    2. 选择症状和体征与影像学检查结果相符的患者。
    3. 选择腰椎磁共振成像(MRI)显示腰大肌与椎体前缘之间距离大于1 cm的患者。
    4. 选择腹部血管检查无明显变异的患者。
  2. 应用以下排除标准。
    1. 排除患有椎管感染及肿瘤并发症的患者。
    2. 排除因严重小关节突增生或髓核突出导致椎管狭窄的患者。
    3. 排除患有III度以上重度腰椎滑脱的患者。
    4. 排除有腹部手术史的患者。

2. 术前准备

  1. 指导患者进行腰椎计算机断层扫描(CT)、腰椎磁共振成像(MRI)及腰椎X线检查。进行心脑血管及腹腔血管检查,以识别并更好地控制危险因素,确保手术安全17,18
  2. 术前指导患者进行手术区域皮肤准备,术前需禁食12小时。
  3. 手术当日,在实施气管插管全身麻醉后,将患者置于手术台上,采取右侧卧位。
  4. 在患者右侧腹部下方放置软垫,根据目标椎间盘位置略向后倾斜,以充分暴露腰大肌与下腔静脉前方之间的生理性OLIF入路通道。
  5. 选择患者左侧腹部作为手术区域。
  6. 确保患者在手术过程中保持正确体位,使用胶带固定患者,并确认手术阶段。

3. 开始操作

  1. 将方形定位器置于患者左腹部。确保定位器的上下边缘与病变椎间盘的上下椎体边缘对齐,前后边缘与病变椎间盘的前后边缘对齐。
  2. 通过X线透视确认目标节段在患者皮肤上的投影位置,并标记手术部位(图1)。
  3. 确定手术部位后,对手术切口区域进行消毒,铺无菌巾,准备手术器械。
  4. 沿腹外斜肌纤维方向做4–5 cm的皮肤切口。用手指钝性分离腹外斜肌腱膜、腹横肌及腹横筋膜(图2)。
  5. 使用长柄镊子夹持花生米状纱布团,显露腹膜血管鞘与腰大肌之间的手术椎间隙。
  6. 完成目标椎间隙显露后,插入克氏针,并进行术中X线检查,确认显露节段为手术节段(图3A)。
  7. 完成椎间隙定位后,依次插入扩张管进行逐级扩张。通过术中X线确认扩张通道的位置(图3B)。
  8. 利用通用臂和手术床夹将牵开器固定于手术床上,确保术中通道不发生移动,然后安置工作光源。
  9. 使用长柄锐利刀片切除纤维环,用长柄髓核钳清除病变椎间盘组织(图3C)。
  10. 使用刮匙和刮铲清理椎间隙,彻底清除手术节段内残留的椎间盘组织。
  11. 清理椎间隙后,使用撑开器打开塌陷的椎间隙。
  12. 使用骨锉去除终板表面的软骨层,直至骨面渗血。
  13. 将适当大小的椎间隙探查装置置入椎间盘内。进行术中X线透视,以确定合适的融合装置尺寸(图3D、E)。
  14. 确定合适装置尺寸后,将同尺寸的椎间融合器内填充同种异体骨,并在X线透视引导下植入融合器(图3F、G)。
  15. 植入椎间融合器后,逐层缝合伤口。分别使用10号、4号和1号线缝合肌肉、筋膜和手术切口(图3H)。用纱布加压包扎手术切口。
  16. 患者返回病房后,每日给予一次抗炎药物(头孢唑林),连续3天,以预防感染。术后第2天起,每日使用一次抗血栓药物(低分子量肝素钠),连续3天,以预防下肢深静脉血栓形成。

4. 术后护理

  1. 指导患者卧床休息。术后第3天进行下肢深静脉彩色多普勒超声检查。
  2. 若下肢深静脉彩色多普勒超声未显示下肢深静脉血栓形成,通常于术后3天内指导患者在腰围辅助下下地行走。
  3. 若下肢深静脉超声显示存在肌间静脉血栓,则将抗血栓药物调整为每日两次。
  4. 若下肢深静脉超声显示存在深静脉血栓,应联系血管外科进行治疗。下肢康复锻炼按先前报道的方法进行19,20

结果

其他研究团队的研究表明,与其他方法相比,侧方腰椎椎间融合术(OLIF)具有手术时间更短、术中出血量更少的优势(表1)。尤为重要的是,接受OLIF手术的患者恢复速度优于常用的经椎间孔腰椎椎间融合术(TLIF)手术组12,21,22。本研究团队对42例接受OLIF手术的退行性腰椎滑脱症(DLS)患者进行了超过1年的随访。结果显示,与术前视觉模拟评分(VAS)相比,采用OLIF治疗的DLS患者VAS评分得到改善,且椎间盘前、后方高度、节段性腰椎前凸角以及整体腰椎前凸角均较术前显著改善16图4)。与既往研究结果相似,OLIF组患者的椎间隙高度和椎间孔高度均高于TLIF组12,21

讨论

自2012年Silvestre正式提出以来23,OLIF因其在治疗腰椎疾病方面相较于其他方法的多种优势,逐渐受到脊柱外科医生的关注6,12,13,16,21,22。OLIF显著改善了患者的椎间隙高度和腰椎前凸角,这可能是由于与传统后路手术相比,OLIF能够更彻底地清除病变椎间盘,并植入更大的椎间融合器,从而有助于恢复脊柱的生理曲度12,16,21。对OLIF患者进行长期术后随访发现,其在临床症状缓解方面与后路TLI...
