机械分离基质血管组分与纤维蛋白水凝胶的联合应用:一种处理方案

摘要

机械分离的基质血管组分(SVF)与纤维蛋白水凝胶联用,可为具有多种适应症的活性脂肪来源基质细胞提供一种简便且高效的载体,适用于组织工程或伤口愈合等用途。本文介绍了一种用于转化研究和临床应用的机械法SVF(mSVF)-纤维蛋白水凝胶构建物的制备方法。

摘要

脂肪来源的基质细胞(ASCs)的再生潜能已在再生医学和转化研究中受到广泛关注。过去,从脂肪组织中提取这些细胞需要采用多步骤的酶解法,最终获得由ASCs及其他细胞组成的异质性细胞混合物,统称为基质血管组分(SVF)。近年来新出现的机械法SVF(mSVF)分离技术更为省时,并可避免相关监管问题。我们近期提出了一种基于乳化与离心联合方法的mSVF制备方案,可获得富含基质细胞的产物。目前在mSVF应用于创面治疗时存在一个实际问题,即缺乏可保护细胞免受机械操作和干燥损伤的支架材料。既往研究表明,纤维蛋白水凝胶在细胞移植促进创面愈合过程中可作为有效的辅助材料。本文中,我们详细阐述了一种mSVF-纤维蛋白水凝胶复合物的制备步骤,为转化研究和临床应用提供一种新策略。

引言

近年来,再生整形外科已成为整形外科的又一重要支柱1。再生整形外科旨在通过转移从患者体内获取的可溶性因子、细胞和组织,以微创方式促进受损组织的修复2。脂肪来源的干细胞(ASCs)因其能够分化为多种间充质谱系,已成为再生医学研究中极具前景的候选细胞3。其细胞因子谱具有促血管生成、免疫抑制和抗氧化作用4

传统上,ASCs 是通过使用胶原酶的酶消化法从脂肪组织中分离获得,从而得到基质血管组分(SVF),随后通过培养获取 ASCs。这些基于实验室的技术成本高昂、耗时较长,且重要的是,受到严格的监管限制,使临床转化变得复杂5,6,7。相比之下,机械法分离的基质血管组分(mSVF)方案不仅能够规避监管问题,还能最大限度降低污染风险,具有显著的临床优势8,9

已报道多种机械分离基质血管组分(SVF)的实验方案10其中,Tonnard 等人发表的移位术式. 在再生外科医生中获得了最多的关注11通过标准抽脂手术获取的脂肪,即抽脂物,可在连接装置上于两个手持注射器之间进行转移,从而形成一种称为纳米脂肪的液态物质。这些微型脂肪来源基质细胞(mSVF)分离方案的显著优势包括:处理时间缩短、污染风险极低(因整个操作在高度可控的环境中完成),以及具备即时临床应用的潜力。12.

临床前和临床证据表明,mSVF的特性(包括细胞活力和促进伤口愈合的能力)与标准的酶法分离方法相当12通过大鼠和小鼠模型验证了mSVF在促进伤口愈合方面的潜力 体内 陈等人和孙等人的研究13,14然而,关于临床环境中伤口愈合的可用数据仍然缺乏。当研究团队对一名83岁患者进行自体脂肪移植时,取得了令人鼓舞的结果,该患者下肢开放性骨折后伤口存在植入物外露。15此外,Lu 等人对20例慢性伤口患者进行了 mSVF 与负压伤口治疗的比较研究。16他们的研究结果表明,与负压伤口治疗相比,mSVF治疗可实现更高的伤口愈合率。16两项研究均将mSVF单独或与凝胶混合后注射至目标创面区域15,16.

在实际应用中,由于微碎片化脂肪组织(mSVF)在受体部位的吸收率不可预测,其临床应用受到限制17,18。支架材料有望解决这一问题,因其有助于细胞滞留、血管化以及与周围组织的整合19,20,21。纤维蛋白水凝胶是一种常用于外科领域的、经美国食品药品监督管理局(FDA)批准的工具,已被证明是mSVF的有效载体19。纤维蛋白凝胶是一种生物高分子材料,作为细胞载体具有多项优势:其具有优异的生物相容性,可促进细胞黏附,并能以可控方式降解22,24,25。此外,其引发的炎症反应和异物反应极轻微,且可在伤口自然愈合过程中被轻易吸收22。我们认为,上述mSVF细胞所具备的多种再生能力,结合纤维蛋白水凝胶的优势,可为促进伤口愈合过程提供一种创新性策略。总体而言,该方法可实现活性mSVF细胞的高效局部递送。本文旨在介绍将mSVF与纤维蛋白水凝胶联合用于伤口愈合的实验操作方案。

方案

本研究遵循《赫尔辛基宣言》的规定进行。所有成年供体均签署书面知情同意书,同意所采集的组织样本用于进一步研究。本方案遵循本机构人体研究伦理委员会的指导方针。

1. 脂肪组织的采集

  1. 采用既往文献中描述的标准脂肪抽吸术和常规脂肪采集技术获取脂肪组织26,27。确保使用肿胀液,其成分为普通乳酸林格液与肾上腺素,比例为1:200,000。
  2. 使用4 mm吸引套管,在负压和振动条件下将脂肪抽吸至无菌袋中。立即将采集的脂肪组织转运至实验室。

2. mSVF 的分离

  1. 在细胞培养操作台中进行以下步骤(第2和第3部分),以提供无菌工作环境。穿戴常规实验服和手套,确保达到生物安全二级水平。
  2. 准备培养基:在500 mL高糖 Dulbecco 改良 Eagle 培养基(DMEM)中加入50 mL胎牛血清(FBS)和5 mL青霉素-链霉素溶液。
  3. 将脂肪抽吸物转移至一个50 mL离心管中。
  4. 将脂肪抽吸物转移至一个无菌的20 mL鲁尔锁紧注射器中,并连接一个1.4 mm连接器。确保注射器内无空气存在。
  5. 将第二个20 mL鲁尔锁紧注射器连接至1.4 mm连接器的对侧。
  6. 将脂肪组织在两个注射器之间来回推注,共30次。
  7. 将乳化后的脂肪转移至一个新的50 mL离心管中。
  8. 在500 × g 条件下离心乳化脂肪10分钟。
  9. 离心后,弃去上层油相,收集中间的纯化mSV层。将其转移至一个新的50 mL离心管中,并弃去水相。
  10. 用步骤2.2中配制的培养基将离心管加液至40 mL刻度线处。
  11. 将离心管放入离心机中,再次在500 × g 条件下离心5分钟。
  12. 收集所得的mSVF层,并将其转移至一个新的50 mL离心管中。

3. mSVF-纤维蛋白水凝胶的制备

  1. 在无菌的 1.5 mL 离心管中,将 100 µL mSVF 与 10 µL 凝血酶(100 U/mL)、10 µL CaCl2(80 mM)和 70 µL 氨甲环酸(100 mg/mL)混合。
  2. 使用新的移液枪头,在即将使用前加入 10 µL 纤维蛋白原(100 mg/mL)作为最后组分。注意,水凝胶聚合反应将在约 10–30 秒内开始。
  3. 用于临床时,应在局部给药前立即进行此最后一步,最好在床旁操作。用于分析目的时,可用移液器将混合物转移至 12 孔板中。

4. 活性检测与组织学分析

  1. 从步骤 3.3 中吸取 200 µL mSVF-水凝胶混合物,加入 12 孔板的一个孔中。
  2. 吸取 100 µL mSVF-收集液(步骤 2.12)加入另一孔中作为阳性对照。
  3. 仅将 200 µL 纤维蛋白水凝胶加入一个孔中作为阴性对照。
  4. 将 12 孔板置于 37 °C、5% CO2 的常规培养箱中孵育 30 分钟。
  5. 孵育结束后,向每孔加入 1 mL 刃天青(Alamar Blue,10% 浓度,用培养基稀释)。
  6. 加液后,将 12 孔板在 37 °C、5% CO2条件下继续孵育 24 小时。
  7. 24 小时后,使用细胞成像多功能酶标仪检测荧光强度,激发波长为 555 nm,发射波长为 596 nm。
  8. 在第 3 天和第 7 天连续进行荧光强度检测。
  9. 如需进行组织学评估,分别在第 1、3、7 天终止实验,将纤维蛋白水凝胶置于 4% 多聚甲醛中,4 °C 固定 24 小时;固定后用 1% 磷酸盐缓冲液(PBS)洗涤,并于 4 °C 保存。
  10. 将水凝胶包埋于最佳切割温度(OCT)包埋剂中,使用冷冻切片机将冷冻组织块切为 10 µm 厚的切片,并根据标准操作流程28用苏木精-伊红(H&E)溶液染色。

结果

刃天青检测
我们首先考察了 体外 mSVF细胞的细胞活力。为此,我们在第0、3和7天进行了刃天青细胞活力检测。总共四个样本在第0、3和7天的细胞活力如图所示。 图1第0天的数值作为基线,设为100%。第3天时,阳性对照组(mSVF)略有下降,为78.92%(±5.33%),而mSVF-纤维蛋白水凝胶组合组保持在96.48%(±1.89%)。第7天也观察到类似趋势,mSVF组为77.46%(±3.90%),而mSVF-纤维蛋白水凝胶组合组稳定在109.02%(±8.92%)。我们采用Welch校正的非配对t检验对第3天和第7天获得的两组数据进行比较,结果显示无统计学显著差异(p ≥ 0.05)。

组织学

讨论

SVF的机械分离法为传统的酶解方法提供了一种简洁的替代方案,并更广泛地适用于临床应用29。事实上,如本论文所述,mSVF目前已用于瘢痕的软组织治疗,或作为整形美容手术的辅助手段30。本文所提出的方案提供了一种简便的方法,可实现活性mSVF细胞的有效局部递送。尽管仅使用mSVF细胞的阳性对照组在7天后吸光度值呈下降趋势,但mSVF-纤维蛋白水凝胶组合组的数值保持稳定。虽然该差异未达到统计学显著性,但观察到的趋势仍具有重要意义。组织学数据也显示,在7天的培养期间,染色细胞数量保持恒定,表明本方法具有良好的细胞相容性。纤维蛋白水凝胶的降解程度极低,第3天和第7天的总体积与第1天相比均无明显变化。体外实验中,可通过调整细胞培养基中抗纤溶添加剂(如氨甲环酸)的浓度,控制水凝胶的降解时间。如前所示,乳化过程并未完全破坏所有脂肪细胞,组织学切片显示,完整的脂肪细胞碎片仍嵌于完整的细胞外基质中。该现象在大鼠脂肪组织中亦有观察到19。总体而言,本研究结果证实,纤维蛋白水凝胶可作为活性mSVF的有...
