经皮下静脉通路港(TIVAP)的CT引导下锁骨下静脉植入术

摘要

在CT引导下经锁骨下静脉植入完全植入式静脉输液港(TIVAP)是一种高度安全且精确的方法,可有效降低并发症发生率,并有助于早期发现潜在并发症。

摘要

完全植入式静脉输液港(TIVAP)已被证实安全有效,常用于癌症患者。本文介绍在CT引导下经锁骨下静脉植入TIVAP的完整操作过程。共有283例鼻咽癌患者(包括215例男性和68例女性)接受了经右侧锁骨下静脉植入TIVAP,平均年龄为46.4岁(范围13–74岁)。其中1例患者右侧锁骨下静脉穿刺失败,随后在超声引导下成功完成左侧锁骨下静脉穿刺。平均手术时间为36.2分钟(范围20–90分钟),导管长度为17.1 cm(范围14–21 cm)。术中发生3例动脉穿刺、1例导管移位、2例血胸、2例气胸、2例血栓形成及2例伤口感染。总体并发症发生率为5.3%(15/283)。因此,经锁骨下静脉行CT引导下完全植入式静脉输液港(TIVAP)植入可被视为一种安全可靠的手术技术。

引言

大多数晚期癌症患者需要长期中心静脉通路,以安全输注化疗药物、全肠外营养、抗生素以及输血和血液制品1,2,3。完全植入式静脉输液港(TIVAPs)和经外周置入的中心静脉导管(PICCs)均被广泛用于此目的。与经外周置入的中心静脉导管(PICCs)相比,完全植入式静脉输液港(TIVAPs)因具有更高的置管成功率、更长的留置时间、更低的导管相关并发症风险(如深静脉血栓)以及更简便的维护方案,在癌症患者中日益受到青睐1,4。TIVAPs对患者生活质量无明显影响5,6

锁骨下静脉(SV)和颈内静脉(IJV)是该操作最常选用的血管。基于解剖标志进行静脉穿刺是一种安全且有效的操作方法。一些临床试验表明,透视和超声是极为有用且可重复性高的工具,可通过实时确认导管位置而提高操作安全性。7,8,9我们介绍了一种在CT介入手术室内通过CT引导下锁骨下静脉穿刺进行的完全植入式静脉输液港(TIVAP)置入术。CT引导下TIVAP植入可实现对导管位置的动态监测和导管尖端位置的精确定位,同时能够即时发现气胸或血胸等潜在并发症的征象。本文报道了CT引导下TIVAP植入术的实施过程。 通过 锁骨下静脉,解决传统成像方式的局限性。

本方案由一名经过培训且具备经外周静脉置入中心静脉导管(TIVAP)操作资质的医生在门诊部实施,同时配备一名熟悉手术流程并能够协助处理相关并发症的助手。本方案所需的设备列于表1中。可根据当地的设施、设备和材料情况进行适当调整。

病例报告:
一名56岁女性因进行性左侧鼻塞及间歇性鼻出血持续3个月,就诊于耳鼻喉科。经鼻咽镜检查及后续组织病理学检查,确诊为未分化型非角化性鼻咽癌(cT3N2M0,III期)。在治疗开始前,择期于右侧锁骨下区域植入完全植入式静脉输液港(TIVAP)。该方案可为化疗提供可靠的血管通路,同时避免干扰后续针对颈部及锁骨上区域的放射治疗射野。

方案

中山大学肿瘤防治中心的机构审查委员会批准了所述方案。由于本研究为回顾性研究,免除了知情同意要求。

1. 准备工作

  1. 手术操作前,应评估现有信息并明确手术指征。确保患者无活动性感染,且无影响中心静脉通路的颈胸部解剖畸形,例如锁骨骨折。
  2. 确保患者已完成血液学检查、血液检测及凝血功能检查。排除标准包括凝血功能障碍(定义为血小板计数 <50,000 mm³ 和/或国际标准化比值 INR >1.5)。
  3. 确保患者已签署知情同意书,并告知其手术风险及可能发生的并发症。
  4. 将患者置于手术台上仰卧位。连接心电监护仪,持续监测心电图和血氧饱和度,每3–5分钟测量一次血压。

2. 实验步骤

  1. 充分暴露上胸部,头部偏向非手术侧。使用碘伏对手术区域消毒2-3次,并采取最大无菌屏障防护措施。
  2. 确定穿刺点和输液港植入位置。使用1%利多卡因对囊袋区域及穿刺路径进行局部麻醉。
    注:穿刺点位于右锁骨中点下方2-3 cm处,穿刺针方向指向胸骨上窝。输液港位置位于穿刺点下方1 cm处。
  3. 保持穿刺针斜面向上,根据解剖标志穿刺锁骨下静脉(图1)。确认回血后固定穿刺针,旋转针体使斜面朝向足侧,沿穿刺针导入导丝(图2)。
  4. 行CT扫描以确认导丝位于上腔静脉内(图3)。
  5. 拔出穿刺针,做2 mm切口,沿导丝插入8 French可撕开鞘。
  6. 撤出导丝和扩张器,将导管经可撕开鞘置入。
  7. 再次行CT扫描,确认导管尖端位于腔静脉-心房交界处(图4)。
  8. 在穿刺点下方约1 cm处切开皮肤及皮下组织,采用钝性分离法制作皮下囊袋(图5)。
  9. 使用隧道针引导导管至皮下囊袋,确保导管完全位于皮下。
  10. 将固定夹穿过导管。
  11. 在适当位置剪断导管,确保导管尖端位于腔静脉-心房交界处。
  12. 将导管与输液港连接,并用锁扣固定(图6)。
  13. 将输液港置入囊袋内,将外露的导管部分置入鞘内。
  14. 向输液港内注入生理盐水,确认导管通畅及输液港功能正常,随后移除可撕开鞘。
  15. 使用可吸收缝线逐层缝合切口(图7)。
  16. 插入蝶形针并测试输液港功能(图8)。
  17. 最后覆盖伤口。
  18. 必要时,行最终CT扫描以确认导管尖端准确位于腔静脉-心房交界处,并评估是否存在气胸或出血等潜在并发症。

3. 术后管理

  1. 术后每3-5天更换一次伤口敷料。
  2. 通过观察、触诊及主动询问患者,评估是否存在红肿、疼痛等情况,判断输液港港体是否与导管分离、港体是否翻转,以及同侧胸颈部是否出现肿胀、同侧上臂臂围增粗等疑似血栓症状。
    注:间歇期治疗期间建议每4周进行一次TIVAP维护。

结果

共有283例鼻咽癌患者通过右侧锁骨下静脉植入输液港,平均年龄为46.4岁(范围13-74岁),其中男性215例,女性68例(表1)。1例患者右侧锁骨下静脉穿刺失败,随后在超声引导下成功实施左侧锁骨下静脉穿刺。平均手术时间为36.2分钟(范围20-90分钟),导管长度为17.1厘米(范围14-21厘米)。术中发生动脉穿刺3例,导管移位1例,血胸2例,气胸2例,血栓形成2例,切口感染2例。如表2所示,总体并发症发生率为5.3%。

讨论

1982年,John Niederhuber采用手术技术经头静脉首次完成了TIVAP的植入10。目前该技术已普遍应用于肿瘤学领域。TIVAP的植入主要有两种方法:手术法和经皮法。由于开放手术技术需要专门的外科技能,而经皮法被广泛认为操作更简便、更安全,因此手术法的应用逐渐减少,经皮植入已成为全球首选的技术11,12,13,14,15

一些专家倾向于通过颈内静脉(尤其是右侧入路)进行穿刺,因为从该静脉入路到上腔静脉的解剖路径更为直接,可减少导管与血管壁的接触及内皮刺激,从而降低相关的血栓形成风险。另一些专家则更偏好锁骨下静脉入路,因为其到腔静脉和右心房的距离更短,无需隧道式置管,且操作所需时间更少。此外,与锁骨下...
