急性肠系膜缺血(AMI)是一种罕见的病理状态1,具有较高的死亡风险2,其特征是由于腹腔干、肠系膜上动脉或肠系膜下动脉内形成血栓,导致肠系膜血流减少。在60%–70%的病例中,肠系膜上动脉因血栓形成或栓塞引起的急性闭塞是导致急性肠缺血的主要原因3。相比之下,在5%–15%的病例中,肠系膜静脉血栓形成可导致肠系膜缺血,主要累及肠系膜上静脉,较少累及肠系膜下静脉4。缺血引起的缺氧和营养不良会导致细胞水平的能量代谢紊乱5。例如,线粒体功能受损会激活多种信号通路,启动细胞死亡机制6。尽管快速再灌注能够恢复有氧代谢,但血流恢复到缺血区域时常因活性氧(ROS)的产生7、钙超载8、内皮功能障碍9以及炎症反应10而导致广泛的组织损伤。

已建立多种采用暂时性肠系膜上动脉阻断的实验动物模型,以阐明缺血-再灌注(I/R)损伤过程中的分子机制11。将此类模型与活体显微技术(IVM)结合使用,对于深入了解细胞间相互作用及疾病进展至关重要11。肠道屏障功能障碍在缺血后很快出现12,表现为上皮完整性丧失13,导致细菌易位至肠外部位,如肠系膜淋巴结、脾脏、肝脏和肾脏12,14。再灌注会进一步加重黏膜损伤15。小动脉中的炎症反应表现为白细胞在内皮细胞层上的滚动和牢固黏附,这一过程依赖于L-选择素16、P-选择素(CD62P)17、血小板GPIIb/IIIa和纤维蛋白原等分子18。有趣的是,白细胞在肠系膜微静脉中的黏附受到肠道微生物群19和Toll样受体4信号通路10的影响。I/R损伤还可触发中性粒细胞胞外诱捕网(NET)的形成10。靶向来源于活化中性粒细胞等白细胞的胞外DNA可改善肠道I/R损伤的预后20。在肝脏中,I/R损伤由定居的巨噬细胞引发21,而微小RNA的抑制可显著减轻细胞凋亡22。值得注意的是,肠系膜I/R损伤会导致肠道菌群失调,这种失调是可逆的,在血管再通后可恢复正常23

内皮细胞也受到缺血/再灌注(I/R)损伤有害作用的影响,其中活性氧(ROS)在该过程中起关键作用24。可观察到白细胞黏附于内皮细胞层、内皮损伤以及一氧化氮(NO)代谢的改变9。然而,所涉及的分子通路尚未被充分研究。

蛋白酶激活受体1(PAR1,F2r)是一种由多种细胞(包括内皮细胞)表达的膜受体,在炎症条件下被激活25。当丝氨酸蛋白酶在受体的N端将其切割时,暴露出连接的配体,该配体继而通过与分子的第二个胞外环结合来激活受体26。内皮蛋白C受体(EPCR)-PAR1信号通路影响内皮一氧化氮(NO)的稳态,并促进外周缺血时血流的恢复27

1. 手术准备

  1. 将必要的手术器械(超细直剪、镊子、血管夹等)进行灭菌处理。
  2. 准备好基于氧气和异氟烷的麻醉系统,配备鼻罩和加热板。

2. 动物准备

  1. 称量动物体重。
  2. 实施麻醉:对小鼠进行麻醉 通过 按照机构批准的方案,腹腔(i.p.)注射含咪达唑仑(5 mg/kg)、美托咪定(0.5 mg/mL)和芬太尼(0.5 mg/kg)的溶液。

3. 手术步骤

  1. 颈静脉导管植入
    1. 在气管上方做一横向切口(约0.5 cm),去除覆盖颈部的皮肤,并分离右侧颈静脉。
    2. 使用约5 cm的丝线结扎静脉远端,并用手术镊固定。在静脉下方约2 cm处放置三条丝线:一条靠近颈静脉远端,两条靠近近端。打外科结,但不要闭合血管。
    3. 准备一根充满0.9% NaCl并用1 mL注射器封堵的聚乙烯导管(内径0.28 mm,外径0.61 mm)。使用剪刀在第二和第三个结扎结之间剪开一个切口,将导管插入颈静脉。回抽注射器,确认导管内有血液进入。
    4. 用丝线结扎固定静脉内的导管,确保牢固。使用医用胶带固定注射器。
    5. 制备吖啶橙:将2 mg/mL的储备液用无菌生理盐水稀释4倍(终浓度为0.5 mg/mL),置于1 mL注射器中。避光保存,并向小鼠注射50 µL。
    6. 制备核酸染料:将5 mM储存液用无菌生理盐水稀释1000倍(终浓度为5 µM),加入1 mL注射器中。将注射器避光保存,并向小鼠注射50 µL。
    7. 体内注射吖啶橙(50 µg/µL,每只小鼠50 µL) 通过 通过颈静脉导管注入染色的白细胞和一种细胞外DNA荧光染料,以类似方法标记中性粒细胞胞外诱捕网(NETs)(5 µM,每只小鼠50 µL)。NETs与白细胞同时成像。
  2. 肠系膜缺血-再灌注损伤模型
    1. 用70%乙醇对腹部皮肤进行消毒。
    2. 沿腹部进行3-5厘米的剖腹手术 白线 使用手术剪。
    3. 使用两个浸有生理盐水的棉签,轻轻移除肠道并找到脂肪覆盖较少的区域。将小肠分支置于黑色面团上,以降低背景信号。
    4. 在缺血/再灌注(I/R)前获取视频(缺血前)。

    6. 用小型血管夹夹闭肠系膜上动脉。
    7. 使用浸有生理盐水的棉签轻轻将肠管推回腹腔,并用7-0缝线缝合腹壁,以防止水分和热量丢失。维持缺血状态60分钟,每20分钟至少检查一次麻醉深度。
    8. 1小时后,通过移除夹子开始再灌注。
      注意:正确执行缺血操作后会出现可见变化。最初,由于血流中断,受影响区域会呈现苍白或白色。移除血管夹后,血液将逐渐重新流入缺血区域,组织颜色随之恢复至正常,表明阻塞成功。在缺血阶段,必须持续密切观察小鼠状态。
    9. 缺血期结束后,在夹闭区域滴加少量生理盐水,然后使用动脉夹施加器轻轻移除微血管夹。
    10. 使用生理盐水湿润的棉签轻轻再次移除肠管,将静脉的小分支置于黑色琼脂上,再次注入50 µL吖啶橙,并在同一位置(缺血后)采集同一静脉的视频。

4. 活体高速视频落射荧光显微镜技术

  1. 使用配备长距离聚光镜、单色器、10倍水浸物镜和电荷耦合器件相机的高速宽场荧光显微镜观察白细胞和中性粒细胞胞外诱捕网(NETs)。图像采集的曝光时间为30 ms,使用Alexa Fluor 488(495/519)和Rhodamine Red(570/590)滤光片。
  2. 使用成像软件进行图像采集与分析。
  3. 在0.06 mm²的兴趣区域内定量细胞募集情况。
  4. 将黏附的白细胞定义为在20秒观察期间保持静止或附着于血管内皮表面的细胞,并对其进行定量。
  5. 通过白细胞标记物与细胞外核酸荧光染料双重标记的细胞外结构来识别NETs。
  6. 实验结束后,按照机构批准的方案,通过颈椎脱位法处死动物。

5. 肝脏内皮细胞(LEC)的分离与定量 RT-PCR

  1. 分离 LEC 通过 磁性细胞分选(MACS)29.
  2. 提取总RNA。
  3. 进行互补DNA(cDNA)合成27 将 2 µg 总 RNA 与 2 µL RT 缓冲液、0.8 µL dNTP 混合物、2 µL RT 随机引物、1 µL 逆转录酶以及 4.2 µL 无菌蒸馏水混合2O.
  4. cDNA 转录按以下条件进行:25 °C 反应 10 分钟,37 °C 反应 120 分钟,85 °C 反应 5 分钟。
  5. 使用20 ng cDNA进行qPCR。

为了研究内皮细胞依赖性对白细胞滚动和黏附以及中性粒细胞胞外诱捕网(NET)形成的影响,首先检测了在内皮细胞中诱导型 VE-Cadherin Cre 介导的 F2r 基因缺失的有效性。采用磁性细胞分选法分离肝脏内皮细胞,随后提取RNA,并进行定量实时荧光PCR(qRT-PCR)。在Cre重组酶受VE-Cadherin启动子调控而缺失的 F2rΔEC 小鼠中,F2r(PAR1)mRNA水平较野生型(WT)对照组显著降低(约降低三分之一)(图1)。

在确认小鼠品系的基因缺陷后,进行肠系膜缺血/再灌注(I/R)损伤实验,以检测血管内皮细胞表达的PAR1对白细胞滚动和黏附的影响,并通过活体显微镜观察(图2A,B)。缺血1小时后,移除夹闭血管的夹子,使血管恢复灌注。观察并记录白细胞的滚动和黏附情况(染成绿色,由白色箭头指示)。尽管两组在I/R损伤后均出现黏附白细胞数量增加,但内皮细胞F2r(PAR1)缺陷导致白细胞黏附于I/R激活的肠系膜内皮的程度明显降低,尽管差异未达到统计学显著性,提示内皮PAR1的激活在白细胞募集至缺血肠系膜微静脉过程中发挥积极作用(图2C)。

下一个目标是检测内皮细胞 PAR1 表达是否调控缺血/再灌注(I/R)损伤诱导的中性粒细胞胞外诱捕网(NET)形成。因此,使用细胞非渗透性染料对细胞外核酸进行染色30,并通过活体显微镜观察生成的 NETs(以黑色箭头指示)(图 2A、B)。在 I/R 损伤后,仅在内皮细胞表达正常水平 PAR1 的野生型(WT)组中观察到 NETs,表明 PAR1 在 NETosis 过程中具有调控作用(图 2C、D)。值得注意的是,当内皮细胞 F2r 表达被抑制时,在受损的肠系膜微静脉中未发现 NETs。

为了保持一致性,必须在缺血前后观察肠道节段的相同部位。由于缺血后肠道在腹腔内的位置可能发生改变,这一步骤具有挑战性。为解决此问题,可在缺血前观察过的肠段部位放置一小块湿润纱布作为标记。
