利用对比增强超声和剪切波弹性成像评估乳腺癌生物标志物

摘要

本实验方案的目的是 评估剪切波弹性成像(SWE)和对比增强超声(CEUS)所得定量参数与乳腺癌生物学预后标志物之间的关系。研究纳入68例乳腺癌患者和68例健康对照者。所有患者均接受SWE和CEUS成像检查,乳腺癌组的手术标本通过免疫组织化学分析雌激素受体(ER)、人表皮生长因子受体2(HER2)和Ki-67的表达水平。

结果显示,乳腺癌组织与正常乳腺组织在弹性及灌注特性方面表现出显著差异。具体而言,癌症组的Emean值较低,而Emax、Emin、峰值强度和平均通过时间(MTT)值较高。相关性分析表明,Ki-67表达与Emean、Emax、峰值强度及MTT呈正相关,而雌激素受体(ER)除与峰值强度无负相关外,与其他所有参数均呈负相关。HER2表达与所有测得的影像学参数均呈正相关。这些发现提示,剪切波弹性成像(SWE)和 Contrast-Enhanced Ultrasound(CEUS)可提供无创、互补的信息,有助于支持乳腺癌的分子特征分析和临床评估。

引言

乳腺癌是全球女性中最常见的癌症之一,其发病率持续上升1。近期的流行病学研究表明,乳腺癌的发病率仅次于宫颈癌,总体而言仍是女性癌症相关死亡的首要原因2,3。乳腺癌的发病机制涉及多种相互作用的因素,包括遗传特征、内分泌影响以及环境暴露4。随着精准医学的发展,人们日益关注影像表型与乳腺癌分子特征之间的关系,以优化乳腺癌的管理策略5。

本研究的目的是 探讨剪切波弹性成像(SWE)和增强超声(CEUS)是否可作为无创影像学生物标志物,用于表征乳腺癌关键分子标志物,包括雌激素受体(ER)、人表皮生长因子受体2(HER2)和Ki-67。本研究旨在 确定影像学检查是否能够与这些指导治疗决策并具有预后意义的生物标志物相关联,并可能作为活检的辅助手段或替代方法。验证这种相关性可提供一种便捷且可重复的方法,通过先进的影像技术在分子水平上评估乳腺癌。

临床表现通常始于发现无痛性乳腺肿块,约80%的女性患者会出现此症状6。随着疾病进展,其他症状可能包括乳头溢液、由于库珀韧带牵拉引起的皮肤凹陷、因淋巴管阻塞导致的橘皮样改变,以及在晚期病例中出现的乳头回缩,并伴有瘙痒、水疱形成和溃疡等皮肤改变7。

目前用于乳腺癌诊断的影像学方法包括超声检查、乳腺X线摄影、磁共振成像(MRI)以及核医学技术8,9,10。在这些方法中,超声检查仍是最为普遍使用的,因其具有无创性、不使患者暴露于辐射,且对区分脂肪组织与致密腺体组织具有高重复性和高分辨率。剪切波弹性成像(SWE)通过基于剪切波速度计算杨氏模量,提供组织硬度的定量测量,从而增强了传统超声的功能11。该技术利用声辐射力激发剪切波,降低了操作者依赖性,提高了结果的可重复性12。对比增强超声(CEUS)可显示肿瘤的血管结构和灌注模式,有助于病灶的检出与特征描述,并辅助鉴别良恶性肿瘤13。

分子免疫组织化学标志物在乳腺癌的预后判断和治疗指导中具有关键作用14。雌激素受体(ER)、孕激素受体(PR)、Ki-67 和 HER2 是在临床决策中具有明确作用的关键标志物15。既往研究表明,超声成像特征可能反映这些分子标志物的表达水平,支持影像表型可反映肿瘤内在生物学特性的观点16,17,18。本研究旨在 采用适当的方案,评估CEUS与SWE成像参数与一系列生物学预后标志物之间的相关性,从而为放射基因组学这一新兴领域提供支持。

方案

本研究遵循赫尔辛基宣言以及临床研究的机构指南。   该研究方案已提交并获得锦州医科大学附属第三医院机构伦理审查委员会批准。 

所有参与者在入组前均已签署知情同意书 ,同意允许使用其去标识化的临床、影像和病理信息用于未来的出版发表。在整个研究过程中,始终严格保护患者的隐私信息。

患者参与的招募和评估过程也遵循了《中国抗癌协会指南》2021年版。

1. 患者选择与准备

  1. 纳入具有明确乳腺癌诊断、超声图像质量良好且临床资料完整的患者。
  2. 排除具有以下任一情况的患者:近期接受过乳腺手术、乳腺假体植入、乳腺炎、复发性乳腺癌、病灶邻近瘢痕、心血管或肝肾疾病、妊娠,或病理检查在影像检查前30天以上进行者。
  3. 在影像检查前记录每位参与者的年龄、体重及临床病史。
  4. 将患者置于仰卧位,双臂上举过头。
  5. 在乳腺表面充分涂抹耦合凝胶,以确保最佳的声学传导和图像清晰度。

2. 剪切波弹性成像(SWE)检查

注意:参见材料表中的超声系统和探头。

  1. 首先进行常规的B型超声检查,以定位并表征病灶。
  2. 启动SWE模式,将探头垂直置于病灶正上方的皮肤表面,避免施加外部压力。
  3. 调整采样框,确保完全包含病灶及其周围足够的组织边缘。
  4. 监测系统的质量控制图,以确保足够的剪切波传播。
  5. 图像稳定后,指导患者屏气3-5秒。
  6. 在此期间保持探头稳定,以维持图像稳定。
  7. 保存彩色编码的弹性成像图像,并使用系统的定量分析工具勾画病灶区域。
  8. 记录病灶的最大(Emax)、最小(Emin)和平均(Emean)弹性模量值。
  9. 每项测量重复三次,并计算平均值。
  10. 遵循已发表的国际指南进行剪切波弹性成像的数据采集和质量控制,以确保测量结果的一致性和可重复性19。

3. 超声造影(CEUS)评估

  1. 确定显示病灶最大横截面积的最佳成像平面。
  2. 按照制造商说明重悬造影剂:加入 5 mL 生理盐水,并抽取 2.4 mL 至注射器中。
  3. 在肘前静脉建立静脉通路,使用合适的导管。
  4. 将超声系统设置为造影成像模式,并调整机械指数以利于微泡显像。
  5. 通过快速团注给予 2.4 mL 造影剂,随后用 5 mL 生理盐水冲管。
  6. 立即开始连续数字录像,以捕捉增强的流入和流出相。
  7. 至少记录 3 分钟,以完整记录增强动力学过程。
  8. 识别达峰增强,并在病灶周围间隔约 3–4 mm 处放置四个测量点(上、下、内、外侧)。
  9. 使用定量分析软件分析时间-强度曲线。
  10. 通过定量分析生成峰值强度和平均通过时间(MTT)等参数。
  11. 遵循既往临床研究中已验证的对比增强超声(CEUS)图像采集与定量分析方法,以确保结果的一致性和可重复性15。
  12. 利用 CEUS 成像及定量分析作为标准乳腺影像的补充,用于评估血管生成和灌注情况。

4. 组织取样与免疫组织化学分析

  1. 根据临床标准,通过粗针穿刺活检或手术切除获取组织样本。
  2. 在室温下用10%中性缓冲福尔马林固定样本6-24小时。
  3. 根据标准的组织病理学处理方案,对样本进行脱水、透明和石蜡包埋20.
  4. 使用切片机将石蜡包埋的组织块切成4 µm厚的切片。
  5. 将切片置于带正电荷的玻片上。
  6. 在室温下用二甲苯脱蜡(每次10分钟,共2次),随后通过梯度乙醇系列(100%、95%、85%和70%乙醇;每步5分钟)水化,并用蒸馏水冲洗。
    注意:所有步骤均在室温下进行,且未进行搅拌。
  7. 使用柠檬酸盐缓冲液(pH 6.0)或EDTA缓冲液(pH 9.0)进行抗原修复。
  8. 为阻断内源性过氧化物酶活性,将组织切片置于3%过氧化氢溶液中孵育10分钟。
  9. 在湿盒中,将用含1% BSA的PBS稀释的一抗(ER 1:100;HER2 1:200;Ki-67 1:200)加入,于4 °C孵育过夜,不振荡,具体操作遵循制造商说明书。
  10. 在湿润孵育盒中,将辣根过氧化物酶标记的二抗用含1% BSA的PBS按1:200稀释后,室温孵育30分钟,期间不进行振荡。
  11. 使用3,3'-二氨基联苯胺(DAB)显色剂显色以观察抗体结合情况,并用苏木精复染30秒。
  12. 在400×放大倍数下,使用标准判读标准评估染色结果:ER阳性 >10% 核染色;HER2 依据 ASCO/CAP 指南进行评估;Ki-67 以阳性肿瘤细胞核所占百分比表示。
  13. 采用标准化方案进行免疫组织化学染色,包括热诱导抗原修复(柠檬酸盐缓冲液,pH 6.0,或EDTA缓冲液,pH 9.0,95–98 °C,20 min)、室温下用3%过氧化氢封闭内源性过氧化物酶10 min、一抗孵育,随后孵育辣根过氧化物酶标记的二抗,DAB(3,3′-二氨基联苯胺)显色,苏木精复染,梯度乙醇脱水,封片;所有样本均采用相同流程,遵循既定的免疫组织化学方法21,22.

5. 统计分析

  1. 以均值 ± 标准差(SD)表示连续变量。
  2. 在正态性检验后,采用独立样本 t 检验比较癌症组与对照组之间的 SWE 和 CEUS 参数。
  3. 采用 Spearman 相关分析评估超声参数与免疫组织化学标志物(ER、HER2、Ki-67)之间的关联性。
  4. 以癌症状态为分类变量,为每个超声参数绘制受试者工作特征(ROC)曲线。
  5. 计算曲线下面积(AUC)值,并根据约登指数(J = 敏感性 + 特异性 − 1)确定的最佳截断值,推导 敏感性和特异性。
  6. 基于选定的截断值,计算阳性预测值(PPV)和阴性预测值(NPV)。
  7. 以双侧 P 值 < 0.05 为具有统计学意义。结果以表格和图示形式呈现。

结果

患者特征

在68例乳腺癌患者和68例健康对照者中实施该方案,结果显示两组在弹性成像和灌注参数方面存在显著差异。所有乳腺癌病例均为单侧病灶,平均直径为20.78 ± 5.37 mm。组织病理学分析显示,49例为浸润性导管癌,9例为小叶癌,6例为导管内癌,3例为黏液癌,1例为髓样癌。

超声造影检查结果

增强超声检查显示出典型的恶性特征,包括形态不规则、内部回声不均、无包膜以及边界不清且呈角状。对比剂注射后,44例(64.71%)表现为快速高增强模式。30例(44.12%)观察到灌注缺损,而21例(30.88%)显示肿块内存在放射状或穿入性血管。

讨论

本方案明确展示了定量超声参数与乳腺癌分子预后标志物之间的关系,为无创性疾病表征提供了清晰的框架。影像参数与免疫组织化学标志物之间的相关性反映了潜在的生物学变化,这些变化同时影响成像过程中观察到的机械特性(组织硬度)和灌注特征。
