揿针疗法:一种用于缓解机械通气患者疼痛的中医治疗手段

本文详细阐述了如何以标准化方式使用按压针缓解接受机械通气患者的疼痛。

摘要

机械通气需要进行插管和导管置入,这些操作可能对人体造成明显损伤,容易加重患者的疼痛,并导致人机不同步。因此,有必要探索一种安全、可靠且更高效的镇痛和镇静措施,以减轻患者的疼痛刺激,确保各项治疗措施有序实施。过去,临床上常采用药物进行镇痛和镇静,虽然效果尚可,但长期大剂量使用可能引发多种并发症或不良反应,从而影响预后质量。揿针疗法,又称皮内针疗法,是一种针灸治疗方式,主要通过将针具埋置于穴位中,促进血液循环,达到缓解疼痛的目的。本文将详细阐述如何规范使用揿针,以减轻接受机械通气治疗患者的疼痛。

引言

危重患者,尤其是接受插管和机械通气(MV)的患者,常伴有疼痛1。疼痛是重症监护病房(ICU)住院患者中常见且令人痛苦的症状。疼痛还可由多种原因引起。昏迷患者可能因侵入性操作、机械通气以及ICU内的物理环境而经历疼痛2。就机械通气而言,众所周知,患者在治疗过程中可能感到疼痛或不适,因此需要足量的镇痛药和镇静药3,4。疼痛可增强神经内分泌系统的活动以及心动过速的交感张力,进而增加心肌氧耗,抑制免疫系统,并增强患者的凝血功能、分解代谢、活动受限、肺部并发症及发育迟缓。目前的疼痛管理方法包括药物治疗(药物干预)和一些非药物干预措施。镇静和镇痛药物常用于接受机械通气的危重患者,以缓解疼痛、焦虑和躁动,并帮助患者耐受在重症监护病房(ICU)中的侵入性操作5。阿片类药物是最常用的镇痛药,而苯二氮䓬类药物、丙泊酚或右美托咪定通常用于预防或减轻焦虑和躁动6。然而,这些药物的过度使用与更差的临床结局相关,例如机械通气时间延长、住院时间增加、精神状态改变的风险升高,甚至死亡率上升7。因此,在危重患者中,如何在提供必要镇痛效果的同时,减少镇静药和镇痛药的不必要用量及其不良反应,始终是一个关键目标。

揿针疗法,也称为 "针刺埋针疗法" (参见 图1 (按压针的图像)属于皮内针疗法。该疗法采用特制的微型针具,对人体特定穴位进行微弱而持久的刺激,具有以下意义 "长期保持平静"揿针疗法结合了皮部理论、腧穴理论和运动针灸理论,是一种创新性的应用。揿针疗法可刺激真皮层内的神经末梢,将信号传递至神经系统,从而缓解肌肉紧张,促进局部血液循环,有助于减轻症状并发挥治疗作用8当揿针留置于皮下时,其持续刺激可导致局部肥大细胞脱颗粒并释放介质,从而引发过敏反应。与此同时,表皮朗格汉斯细胞在识别外来物质后,将参与免疫应答并激活机体的免疫调节机制。揿针是一种浅刺激技术,通过将针具轻柔地刺入特定皮下穴位并加以固定,以延长留针时间,从而累积治疗效应。揿针疗法与经络理论密切相关 "十二个皮肤区域"即通过长期按压对皮肤区域提供持续且稳定的刺激9,10,11与普通针灸相比,具有操作简便、持续刺激时间长、患者依从性好的优点12.

从中医理论的角度来看,揿针疗法主要通过皮部-络脉-卫气-经筋的理论,调节气血、疏通经络、濡养筋脉,从而达到缓解疼痛的效果。现代研究表明,皮部与经络之间存在一定的物质基础,用于相互联系13,14,15。因此,通过揿针浅刺皮部,可借助这一物质基础影响经络及内脏功能,实现疏通经络、调和气血、缓解疼痛的作用。揿针刺激皮下络脉,调节络脉中气机的郁滞与紊乱,恢复经络及与其相连内脏的气血灌注,沟通内外,平衡阴阳,使营卫之气运行通畅、经络通利,从而缓解疼痛16。

从现代医学理论的角度来看,揿针的刺激可传递至中枢神经系统,影响内分泌功能以及内源性镇痛物质的释放。揿针插入皮下组织后,埋针部位的游离铁、镍等离子含量发生变化,产生电流并引发神经冲动。这些冲动通过传入神经传递至中枢神经系统,激活中枢内分泌器官,释放5-羟色胺、去甲肾上腺素、血清素、食欲素等小分子神经递质,以及阿片肽和抗阿片肽等大分子肽类物质。这些物质通过抑制脊髓背角对伤害性信息的进一步传递,并影响痛觉信号的下行投射,从而发挥镇痛作用17,18。针灸可抑制神经细胞的自噬,减少肿瘤坏死因子-α和白细胞介素-6等促炎因子的生成,同时增强抗炎因子白细胞介素-10的活性19。针灸通过影响内分泌功能所产生的内源性阿片肽,还可调节促炎因子及炎症反应物质的释放,在炎症性疾病中发挥治疗作用20。此外,针灸还能影响血管调节因子、凋亡因子、自噬因子等的表达,改善病理性疼痛21。本研究旨在基于中医穴位理论,阐述如何正确使用揿针缓解接受机械通气患者的疼痛。这种皮内针疗法主要适用于已行经口插管、接受轻至中度镇静、且预计需机械通气超过24小时的患者。明确排除存在皮肤感染、凝血功能障碍、孕妇以及对针灸有恐惧心理的患者。有关详细的纳入与排除标准,请参见“方案”部分。

方案

本研究为回顾性队列研究,研究对象的入组时间为2024年1月至2025年1月。本方案已获得成都中医药大学附属医院临床试验伦理委员会的批准(伦理备案编号:KY202420)。需确保环境清洁无污染、光线充足且安静。须获取患者的知情同意,允许使用和发表本文所呈现的数据。

1. 患者选择

  1. 纳入标准
    1. 纳入所有接受经口气管插管并连接呼吸机进行辅助通气的患者。
    2. 纳入预计机械通气时间超过24小时的患者。
    3. 纳入已签署研究知情同意书的患者。
    4. 纳入病例资料完整的患者。
  2. 排除标准
    1. 排除患有皮肤感染的患者。
    2. 排除有针灸恐惧的患者。
    3. 排除妊娠患者。
    4. 排除患有凝血功能障碍的患者。
    5. 排除因特定原因退出研究或在治疗期间转至其他医院的患者。
    6. 排除治疗前后镇静目标被调整的患者。
      注:所有患者需每日进行两次 Richmond 躁动-镇静量表(RASS)评估22。治疗前后镇静方案保持不变。本试验中,RASS 评分为 -2 至 +1 的患者定义为轻度镇静患者,评分为 -3 至 -5 的患者定义为中度或深度镇静患者。

2. 术前评估

  1. 核对患者基本信息:包括性别、年龄、病情、主要症状、病变部位、局部皮肤状况、凝血功能、临床症状、既往病史、有无出血性疾病、妊娠或月经情况以及过敏史。
  2. 评估患者心理状态:了解患者对皮内针操作的认知程度、配合程度、疼痛耐受能力等;对于意识清醒的患者,应向其说明操作目的,以取得配合。
  3. 操作前对所有患者进行重症监护疼痛观察工具(Critical Care Pain Observation Tool, CPOT)评估23。
  4. 环境准备:确保光线和温度适宜。
  5. 器械准备:检查器械的有效期,确认性能良好、处于备用状态,并摆放整齐。

3. 医护人员的准备工作

  1. 着装整洁,洗手,并佩戴口罩。
  2. 环境要求:保持适宜的温湿度,注意保护患者隐私,避免空气交叉流通。
  3. 器械准备:准备治疗盘、皮内针、镊子、敷料、无菌棉签、碘伏消毒棉签、弯盘和锐器盒。

4. 实施步骤

  1. 将设备带至患者床旁,再次核对患者信息,取得清醒患者的配合。
  2. 准备患者:协助患者取舒适体位,暴露皮内针注射部位,清洁局部皮肤,并拉上帘子保护患者隐私。
  3. 选取穴位(使用患者自身的身体测量方法)

    3. 保持双手温暖并选取穴位。采用患者自身的“寸”测量法进行定位,参考《经络腧穴学标准操作规范》41。合谷穴位于第二掌骨桡侧中点,人迎穴位于喉结旁开1.5寸,内关穴位于腕横纹上2寸。对于清醒患者,可通过询问其感觉(如酸、热、麻、胀或痛感)来确定穴位的准确位置。
    4. 消毒:使用75%医用乙醇消毒剂喷洒穴位2–3次。取一次性无菌棉签蘸取75%乙醇,以穴位为中心由内向外螺旋式擦拭,消毒区域直径不小于3 cm。待乙醇完全挥发后方可进行针刺。

5. 皮内注射针操作

  1. 检查皮内针:确保皮内针在有效期内,打开包装后检查包装完整性,并确认皮内针处于无菌状态,无破损或变形。
  2. 合谷穴针刺:用非优势手的拇指和食指固定皮肤,用优势手持镊子夹取皮内针,保持针尖无菌。将针尖对准合谷穴,以30°或45°角进针54,55。接触皮肤后,减小皮内针与皮肤之间的角度,继续将针体刺入穴位。用镊子夹取固定贴片,沿针体方向粘贴,确保固定贴片边缘无翘起、针体无松动。用非优势手按压并固定贴片(图2)。
  3. 操作要求:操作过程中应避免抖动或突然移动,以防增加患者疼痛或造成损伤。针刺后,针刺部位皮肤应略显苍白,无出血或渗出,针体应垂直于皮肤表面,无倾斜。
  4. 人迎穴针刺:按照步骤5.2所述方法操作(图3)。针刺部位外观要求同步骤5.3。
  5. 内关穴针刺:按照步骤5.2所述方法操作(图4)。针刺部位外观要求同步骤5.3。
  6. 询问患者感觉:对于轻度镇静患者(RASS评分-2至+1),轻轻按压已插入的针体三次,询问患者是否感到酸、麻、胀或痛。若患者有得气感,则留针;若无,则调整进针方向直至出现得气感后再留针。对于中度或深度镇静患者,通过CPOT评分(观察面部表情和肢体活动)评估镇痛效果24,25。
  7. 起针方法:2天后取出针具,让患者休息1天,再于相同穴位重新贴敷皮内针。一个疗程持续2周,共进行2个疗程。
  8. 留针期间护理:留针期间,保持室温22–24 °C,湿度50%–60%。患者应避免针刺部位剧烈活动,以防针体脱落或移位。每隔约3小时按压针体一次,每日3次,每次约1分钟。按压力度以患者耐受为宜(以轻度镇静患者报告的轻微酸胀感为标准),避免用力过大导致皮肤损伤。
  9. 起针操作:用非优势手的拇指和食指固定皮肤,用优势手持镊子夹住针体贴片,垂直于皮肤方向拔出针体。起针后,用无菌棉签按压针孔30秒,直至无出血。再用酒精棉签局部消毒。起针后针孔处应无红肿、出血,皮肤完整性良好。
  10. 疗效评估:起针后再次对患者进行CPOT评估。

结果

共有60名符合条件的患者参与并完成了本研究。研究过程中,3名患者的气管导管被提前拔除,4名患者自愿出院,另有4名患者因病情变化而调整了镇静方案。本试验以患者的CPOT评分作为主要评价指标,并采用配对t检验对患者治疗前后的自我控制情况进行比较。

除了CPOT评分外,还监测了阿片类药物的日均消耗量(以芬太尼为例)作为评估非药物镇痛效果的客观指标。结果显示,治疗前患者芬太尼的日均剂量为(80 ± 15)µg,治疗后下降至(59 ± 12)µg,差异具有统计学意义(P < 0.05)。

讨论

对于接受机械通气的危重症患者,常规镇痛和镇静是必要的。然而,过度使用镇静和镇痛药物可能导致不同程度的不良反应,例如呼吸驱动减弱26。这些不良反应与临床预后恶化相关,包括机械通气时间延长、住院时间增加、谵妄风险升高,甚至死亡率上升27,28。通过大量复杂的临床尝试,迄今已确定优化镇痛和镇静策略可避免过度镇静和过度镇痛,通过减少镇静和镇痛药物的使用时长与剂量,从而改善临床预后29,30,31。因此,为危重症患者制定强化的镇静与镇痛药物策略已成为一项关键目标。

除了上述考虑因素外,本研究还存在若干局限性,需予以说明。首先,作为一项回顾性队列研究,其本质上易受选择偏倚的影响,因为患者入组是基于现有的临床记录而非随机分配,这可能引入了无法完全校...
