摘要

引言

方案

本研究涉及对已发表数据的分析,无需伦理审批或患者同意。

1. 制定检索策略 9

  1. 检索以下数据库:PubMed、Cochrane Library、Embase、Google Scholar 和 OVID。所有检索均需同时使用医学主题词(MeSH)和自由文本关键词,不限制语言和发表时间。
  2. 记录检索日期以及从每个数据库获取的文献记录数量。
  3. 使用 PICOS 框架(研究人群、干预措施、对照、结局和研究设计)10 构建检索式。

2. 研究的筛选与选择 11

  1. 将所有检索结果导入文献管理软件(例如,EndNote X20)。先进行自动去重,再通过人工核对以删除重复记录。
  2. 使用专用的系统评价软件(例如,Covidence)进行筛选过程12。指派两名独立的作者根据预先设定的纳入与排除标准对标题和摘要进行筛选。作者之间如有分歧,应通过讨论解决,必要时可咨询第三位作者。
  3. 采用以下纳入标准:随机对照试验或比较性研究;成年胃癌患者(≥18岁);营养干预措施与对照组或替代干预措施的比较;报告至少一项相关的营养状态结局指标(体重、白蛋白、前白蛋白、转铁蛋白)。
  4. 采用以下排除标准:非比较性研究设计;非胃癌人群;未报告营养相关结局;缺乏可用于荟萃分析的足够数据。
  5. 获取所有通过标题/摘要筛选的记录的全文文献13。指派两名独立的作者依据纳入标准对每篇全文文献进行评估。记录此阶段每篇被排除文章的具体原因。必要时联系相关研究的通讯作者以获取缺失数据或澄清信息。

3. 提取数据 13

  1. 制定标准化的数据提取表。使用 Microsoft Excel 等软件创建该表格,内容应包括:研究特征(作者、年份、国家、设计类型);参与者特征(样本量、年龄、性别、癌症分期);干预措施细节(类型、组成、持续时间、实施时机);对照措施详情;结局数据(各组及各时间点的均值、标准差和样本量);以及方法学质量评价指标。
  2. 指派两名独立的研究人员,使用标准化表格从纳入的研究中提取数据。通过共识讨论或参考原始发表文献来解决分歧。
  3. 提取以下指标在指定时间点的结局数据:干预后1个月相对于基线的身体重量变化;术后第7天(±2天)的白蛋白、前白蛋白和转铁蛋白水平。
  4. 将所有提取的数据转换为标准化单位:体重以千克(kg)表示,白蛋白以克每升(g/L)表示,前白蛋白以毫克每升(mg/L)表示,转铁蛋白以克每升(g/L)表示。若原文未报告均值和标准差,应使用公认的转换方法,根据可获得的统计量进行计算。

4. 评估纳入研究的质量 10

  1. 评估每项研究的偏倚风险。对于随机试验,使用Cochrane偏倚风险评估工具2.0版(RoB 2)。
  2. 指派两名独立的作者,从以下五个方面对每项研究进行评估:随机化过程导致的偏倚;干预措施偏离预期导致的偏倚;结局数据缺失导致的偏倚;结局测量中的偏倚;以及报告结果选择中的偏倚。将每个领域的偏倚风险分类为“低风险”、“存在一定问题”或“高风险”。通过讨论解决任何评估分歧。
  3. 对每项研究做出整体质量判断。将研究分类为:低风险(所有领域均为低风险)、存在一定问题(至少一个领域存在一定问题)或高风险(至少一个领域为高风险,或多个领域存在一定问题)。

5. 统计学分析

  1. 准备数据分析。将提取的数据输入元分析软件(例如,Review Manager 5.4)。对于连续性结局指标,计算每项研究的均数差(MD)及其95%置信区间(CI)。若同一结局指标使用了不同的测量量表,则采用标准化均数差(SMD)。
  2. 评估统计学异质性。计算I²统计量,以量化由异质性导致的总变异所占比例。I²值的解释如下:0–25%(可能不重要),25–50%(低),50–75%(中等),75–100%(高)。使用Cochran's Q检验(p < 0.10)评估是否存在显著异质性。
  3. 选择合适的统计模型。当I² > 50% 或存在明显的临床异质性时,采用随机效应模型;当I² ≤ 50% 且研究间临床特征均质时,采用固定效应模型。将两种模型的结果均作为敏感性分析的一部分进行报告。
  4. 进行亚组分析和敏感性分析。根据干预类型、干预时机、地理区域和癌症分期进行亚组分析。通过排除高偏倚风险的研究、使用替代效应量指标、应用不同统计模型以及进行逐一剔除分析(leave-one-out analyses)来实施敏感性分析。
  5. 评估发表偏倚14。针对包含10项或以上研究的任何结局指标,绘制漏斗图以直观检查其不对称性。对包含10项或以上研究的结局指标进行Egger回归检验,以统计学方法检测漏斗图的不对称性。若某结局指标所纳入的研究少于10项,则应谨慎解释结果。

检索结果与研究筛选
2025年7月1日进行的全面文献检索从五个数据库中共识别出3675条记录。去除1226条重复记录后,对2449条记录进行了标题和摘要筛选。其中,148篇全文文章被评估是否符合纳入标准,130篇因不符合纳入标准而被排除。最终共有18项研究15,16,17,18,19,20,21,22,23,24,
