我们采用连续结局模型,采用固定效应或随机效应方法进行荟萃分析,估算每个结局的平均差(MD)和95%置信区间(CI)。我们选取了9项研究,共3648名护士进行荟萃分析。了解AI在护理实践中的应用的护士,其感知(合并原始MD,1.43;95% CI,0.86–1.99,p < 0.001)、态度(MD,1.80;95% CI,0.81–2.78,p < 0.001)和意图(MD,2.89;95% CI,1.61–4.16,p < 0.001)显著高于不了解AI在护理实践中的应用者。 然而,所有结局的异质性都非常高(I2 = 91–98%),表明各研究间存在显著差异。然而,由于这些结果使用量表范围差异很大(例如5点到100分量表)的仪器测量,合并后的原始MD并不代表一致的绝对差异。所有研究中效果一致(阳性)是主要发现,而非具体MD值。

具体来说,这项荟萃分析旨在比较两组护士之间的三项结果——感知、态度和意图——分别是报告了解人工智能在护理实践中的应用情况,另一组则不了解。

研究设计:

本研究为系统综述和荟萃分析,按照系统评价与荟萃分析首选报告项目(PRISMA)指南进行。完成的PRISMA 2020检查清单作为补充文件1提供。 该方案旨在定量综合护士对患者护理中使用人工智能(AI)的看法、态度和意图,比较已有AI知识者与无18岁护理人员。 图1 展示了研究选择过程。

资格标准
研究基于以下PICOS框架19进行筛选:

人口(P):任何临床专科或环境中的注册护士或护士经理。

干预(I)/暴露:拥有护理实践中人工智能应用的知识、培训或意识。

比较(C):报告不了解人工智能在护理实践中的应用的护士或护理学生。

结局(O):以带标准差(SD)的平均分或可转换为标准差的数据报告的对人工智能的感知、态度或意图的定量测量。

研究设计(S):观察性研究(横断面或队列)和基于调查的研究。介入性研究(如随机对照试验)被排除在外,因为兴趣的暴露——即对护理实践中人工智能如何使用的了解——是现有特征,而非随机干预。

排除标准:

本研究排除了定性研究、综述文章、社论、信件、无完整数据的会议摘要及非同行评审文献。排除了未提供两组(有无AI知识)比较定量数据(如均值、标准差、样本量)的研究。排除了人群非纯或主要为护士的研究(如无可分离护士数据的混合医疗专业人员组)。未排除任何基于语言的研究。

信息来源与检索策略

系统检索涵盖了五个电子数据库,从其成立起直到2025年7月31日:PubMed、Cochrane Library、Embase、OVID 和 GoogleScholar
搜索策略结合了受控词汇(如MeSH术语)和与三个核心概念相关的自由文本关键词:(1)护士,(2)人工智能,以及(3)感知/态度。布尔运算符“AND”和“OR”用于连接概念内及概念之间的术语。 表1提供了PubMed的示例搜索策略。所有纳入研究及相关综述的参考文献列表均经过人工筛选,以确定更多符合条件的出版物21。每个数据库的完整可重复搜索字符串如下所示。语法根据每个数据库的要求格式进行了调整,包括合适的字段标签、受控词汇(MeSH、EMTREE)和布尔运算符。没有语言或日期限制。搜索于2025年7月31日进行。

学习选择流程

研究选择过程遵循PRISMA流程图(见 图1)。所有检索到的记录都导入到 EndNote X9(Clarivate Analytics)进行重复删除。两位独立评审员根据资格标准筛选了标题和摘要。任一审稿人认为可能相关的研究进入全文综述。 同两位评审者独立评估了入围研究的全文。任何阶段的分歧都通过讨论和共识解决。无需第三位评审者。 最终符合所有标准的研究名单是通过共识达成的。

数据提取与管理22

在电子表格中开发了标准化的试点数据提取表。两位评审者独立提取了每项纳入研究的数据。提取的数据包括研究特征、人群细节、暴露定义和结局数据。

研究特征:第一作者、发表年份、国家、研究设计及样本量。

人口详情:护士类型(如临床护士、学生护士、管理者)、临床环境、平均年龄、性别分布。

暴露定义:如何定义和测量“护理实践中人工智能使用的知识”(例如,特定培训课程、李克特量表上的自我报告熟悉度)。

结局数据:针对每个相关结局(感知、态度、意图),提取了“具备AI知识”和“非AI知识”组的平均分、标准差(SD)和样本量(n)。如果均值和标准差(SD)未直接报告,则采用《Cochrane干预系统评价手册》(6.4版)中描述的方法,从现有统计数据计算得出。具体来说:

从中位数和四分位数区间(IQR):采用Wan等(2014)23 的方法估算均值和标准差。

根据95%置信区间(CI)和样本量:标准差(SD)采用公式反推计算:SD = √n ×(置信区间上限-下置信区间)/(2 × 1.96)。

根据p值和样本量:当有t统计量或精确p值时,使用Cochrane手册(第6.5.2.3节)描述的方法估计标准差。

在纳入的9项研究中,有3项因均值和标准差未以荟萃分析所需的格式报告,需要数据转换:

研究【作者,年份】:报告的中位数和IQR;采用Wan等人23 方法转换。

研究 [作者,年份]:仅报告95%置信区间;SDs已经回算过了。

研究 [作者,年份]:报告均值,但无标准差但提供组别比较的p值;根据p值估算的标准差。

其余6项研究直接报告均值和标准差,无需转换。所有转换后的数值均由两位评审独立验证。

偏倚风险(质量)评估24

两位评审者使用乔安娜·布里格斯研究所(JBI)分析横断面研究的批判性评估清单,独立评估了纳入观察性研究的方法学质量25。该工具评估样本代表性、暴露与结果测量、混杂因素以及统计分析等领域。每项题目都被评分为“是”、“否”、“不清楚”或“不适用”。根据共识,对整体研究质量评级(高、中、低)进行了评级。分歧如上所述得到解决。

数据综合与统计分析

效应指标:主要效应指标为均数差(MD),置信区间(CI)为95%。MD>0表示AI知识组得分较高(例如态度更积极)。

荟萃分析模型:由于预期各研究间的临床和方法学异质性,所有初级分析均采用随机效应模型。还应用了随机效应模型作为敏感性分析26,27

异质性评估:统计异质性采用I2 统计量量化。25%、50%和75%的数值 分别被解释为低、中度和高异质性。还参考了Cochran的Q检验(p < 0.10,显示显著异质性)。

亚组与敏感性分析:由于每个结局研究数量有限(<10项),计划中的亚组分析不可行。敏感性分析通过切换到固定效应模型进行。计划进行敏感性分析以评估合并估计值的稳健性,包括排除被评为低质量的研究并切换到固定效应模型。进行了不包括两项评为低质量(JBI评分≤4)27.28的研究的敏感性分析。其余七项研究的合并效应估计(n = 3,161)与所有结局的主分析相似,所有关联均保持统计学显著性(p < 0.001)。因此,所有9项研究均被保留在主要分析中。由于排除这些研究并未实质性改变结果,因此保留了它们作为主要分析,以最大化样本量和推广性。

发表偏倚评估:计划对漏斗图进行视觉检查,涵盖包括≥10项研究。由于所有结果均包含的研究少于10项,正式的检验如埃格尔回归检验被认为力度不足,通常不推荐使用。然而,对研究数量最多的结局(态度,n=9)进行了探索性埃格尔回归检验作为事后分析。由于统计检验力较低,结果应极为谨慎地解读。
