NETs相关炎症性状与2型糖尿病及其并发症的孟德尔随机化分析

摘要

中性粒细胞胞外诱捕网(NETs)相关的炎症特征在2型糖尿病(T2DM)及其并发症中发挥重要作用。值得注意的是,作为关键炎症细胞因子的IL-6,与NETs的形成以及T2DM及其并发症的发病机制密切相关。本研究旨在采用孟德尔随机化(MR)方法,探索NETs相关炎症特征与T2DM及其并发症之间的因果关联。研究采用两样本MR设计,数据来源于全基因组关联研究(GWAS),涵盖基于大样本人群的欧洲人群荟萃分析,用于T2DM及其并发症的研究。主要分析方法为逆方差加权法(IVW),并辅以MR-Egger回归、加权中位数法和加权众数法。敏感性分析包括MR-Egger、MR-PRESSO、Cochran’s Q检验和留一法,以评估研究结果的稳健性。研究结果表明,遗传预测的白细胞介素-6(IL-6)水平与糖尿病性冠状动脉疾病(CAD)呈负相关(OR = 0.8997,95% CI:0.8257–0.9803,P = 0.0158)。此外,NETs与伴有肾脏并发症的T2DM(OR = 0.97,95% CI 0.9428–0.998,P = 0.0358)以及伴有外周循环系统并发症的T2DM(OR = 1.0342,95% CI 1.002–1.0673,P = 0.037)存在显著关联。显著的IVW关联结果未显示异质性或水平多效性的证据。本研究提示,遗传预测的NETs相关炎症特征与特定T2DM并发症相关。遗传预测的IL-6与糖尿病性CAD呈负相关,而NETs则与T2DM的肾脏及外周循环系统并发症相关。

引言

2型糖尿病(T2DM)以胰岛素抵抗为特征,常伴随进行性β细胞功能障碍,在全球范围内已达到流行病水平1,2。T2DM的并发症包括心血管疾病、肾功能衰竭、神经病变和视网膜病变,不仅严重影响患者的健康和生活质量,也给全球医疗系统带来沉重负担,导致资源紧张、医疗成本上升,并对经济造成影响3,4。鉴于T2DM的多因素特性,有必要对其病因进行深入细致的探讨。越来越多的证据表明,中性粒细胞胞外诱捕网(NETs)参与了多种代谢性疾病,尤其是T2DM的病理生理过程。

中性粒细胞胞外诱捕网(NETs)是由去浓缩的DNA和颗粒蛋白构成的胞外网络结构,具有强大的杀菌特性5,6。无细胞DNA和瓜氨酸化组蛋白H3等标志物是NETosis的关键指标,可反映多种疾病中的炎症活动7,8。越来越多的证据表明,糖尿病可促进NETs的形成,从而加剧炎症反应和组织损伤9,10。临床上,升高的NETs标志物与高血糖以及多种糖尿病并发症的不良结局相关。例如,在ST段抬高型心肌梗死(STEMI)患者中,双链DNA(dsDNA)水平升高与血糖调节紊乱相关11;NETs组分与糖尿病视网膜病变的进展相关12,并可预测糖尿病足溃疡患者的截肢风险13。尽管存在这些关联,NETs与2型糖尿病(T2DM)及其并发症发生发展之间的确切因果关系仍有待进一步阐明。越来越多的证据表明,IL-6与NETs在代谢性炎症中存在双向相互作用。高血糖可使中性粒细胞处于易于发生NETosis的状态,而IL-6是这一依赖能量过程的强效诱导因子14。临床上,IL-6可独立预测2型糖尿病患者NETs标志物(如H3Cit和无细胞DNA)的升高,这些标志物与血栓前状态以及糖尿病视网膜病变等微血管并发症密切相关15,16。此外,感染诱导的NETosis可增强IL-6的转信号传导,形成一种相互调控的反馈环路17。综上所述,这些研究结果强调了IL-6与NETs在糖尿病病理环境中的紧密病理生理关联。

孟德尔随机化(Mendelian Randomization, MR)是一种强大的统计学方法,利用自然界中存在的遗传变异作为工具变量,估计可改变的暴露因素对健康结局的因果效应,从而揭示复杂疾病的发生通路18。该方法基于孟德尔遗传定律,通过利用减数分裂过程中等位基因的随机分配,使研究人员能够在观察性研究中推断因果关系,从而克服传统流行病学方法在确立因果关联方面的一些局限性19。

迄今为止,关于利用孟德尔随机化(MR)研究基因预测的中性粒细胞胞外诱捕网(NETs)相关炎症性状与2型糖尿病(T2DM)及其并发症之间因果关系的报道仍十分有限。因此,本研究旨在采用两样本MR分析方法,探讨NETs相关炎症性状与T2DM及其并发症之间的潜在因果关联。

方案

研究设计

本研究采用双样本孟德尔随机化(MR)方法,探讨中性粒细胞胞外陷阱(NETs)相关炎症性状与2型糖尿病(T2DM)及其并发症之间的因果关系。MR分析框架依赖于三个关键假设20:1)遗传变异必须与暴露因素显著相关;2)遗传变异仅通过暴露因素影响结局(排除多效性);3)遗传变异不能与任何混杂因素相关。有关本研究流程的详细概述,请参见图1。本论文遵循MR-STROBE指南报告MR研究,以确保研究的透明度和可重复性达到严格标准21。

数据来源

本MR分析所使用的全基因组关联研究(GWAS)数据来源于GWAS公共数据库。关于2型糖尿病(T2DM)的汇总水平GWAS数据,包括针对欧洲人群的最大规模荟萃分析,涵盖74,124例病例和824,006例对照,以及一项在欧洲开展的前瞻性嵌套病例队列研究,包含9,978例病例和12,348例对照22。对于糖尿病并发症,包括伴有肾脏并发症的T2DM和伴有外周循环系统并发症的T2DM,本研究从相关研究中获取了GWAS汇总统计资料,其中病例为患有特定并发症的T2DM患者,对照为无T2DM的个体。数据来源的详细信息见补充表1。

与中性粒细胞胞外诱捕网(NETs)相关的炎症性状的全基因组关联研究(GWAS)数据来自GWAS数据库(见补充表2)。为解决生物学特异性方面的异质性问题,本研究根据所纳入暴露因素在中性粒细胞生物学中的功能作用,将其分为两个不同类别:(1)NETosis核心相关因子:该类别包括直接参与NETs结构形成或染色质去凝集酶促过程的标志物。具体包括NETs本身、髓过氧化物酶(MPO)、中性粒细胞弹性蛋白酶(NE)以及MPO-DNA复合物。MPO和NE是组蛋白降解和染色质去凝集的关键酶,而MPO-DNA复合物是NETs的特异性替代标志物23。(2)NETs相关炎症介质:该类别包括作为NETosis上游调节因子或下游效应分子的细胞因子和介质,这些因子虽与NETosis密切相关,但也参与更广泛的炎症通路。该组包括白细胞介素-6(IL-6)24、肿瘤坏死因子-α(TNF-α)25、中性粒细胞明胶酶相关脂质运载蛋白水平(NGAL)26以及细胞通讯网络因子1(CCN1)27。

本研究使用的数据来源于开放获取数据库或先前研究中发表的全基因组关联研究(GWAS),因此本研究无需伦理审批。

NETs 相关遗传工具的选择

鉴于中性粒细胞胞外诱捕网(NETs)的形成是一个动态的生物学过程,无法通过传统的全基因组关联研究(GWAS)直接测量,本研究采用基于基因的方法来鉴定与NETs相关炎症性状的遗传工具。研究从已建立的机制性研究和已发表的文献中系统整理出一组共257个已知在NETs形成与调控中起关键作用的基因,包括MPO、组蛋白编码基因以及中性粒细胞颗粒蛋白编码基因28。

工具变量选择

为了确定与中性粒细胞胞外诱捕网(NETs)相关炎症特征的可靠遗传工具,本研究使用全基因组关联研究(GWAS)汇总统计数据实施了逐步筛选流程。具体操作步骤如下:

初步筛选:根据显著性阈值 P < 5 × 10⁻6,提取与暴露性状显著相关的遗传变异29。

次要等位基因频率(MAF)过滤:为确保统计功效,排除次要等位基因频率(MAF)≤ 0.01 的单核苷酸多态性(SNPs)30。

连锁不平衡(LD)剪枝:为减少连锁不平衡(LD)混杂的影响,采用剪枝函数在10,000 kb窗口范围内筛选独立的SNP,参数设定为r2 < 0.00131。

工具变量强度评估:使用 F 统计量对每个剩余的工具变量(IV)的强度进行量化,计算公式为 F = R2 × (N-2) / (1-R2)。仅保留 F 统计量 > 10 的 SNP,以尽量减少弱工具变量偏差32。

代理 SNP 鉴定与数据整合

为解决结局数据集中缺失的SNP,本研究进行了代理搜索和数据整合过程:

替代位点选择:当暴露性状的全基因组关联研究(GWAS)中的目标单核苷酸多态性(SNP)在结局GWAS中不可用时,本研究基于1,000基因组计划的欧洲人群参考面板,使用LDproxy()函数寻找替代位点。仅当候选替代SNP与原始SNP具有高度连锁不平衡(r2 > 0.8)时,才被选为替代位点。若未找到符合条件的替代位点,则该SNP被剔除。

数据标准化:使用 TwoSampleMR 软件包(R 版本 4.0.5)中的 harmonise_data() 函数对暴露和结局数据集进行对齐。本研究将参数 action 设置为 2,以自动将所有 SNP 校正至正链,并移除具有模糊链方向的回文 SNP。

验证:在完成数据整合后,本研究通过人工检查整合后的数据集,确认暴露数据与结局数据之间的效应等位基因频率具有一致性。

MR 分析与敏感性检验

使用 TwoSampleMR 软件包(R 版本 4.0.5)进行因果推断。

主要分析与次要分析:本研究采用逆方差加权法(Inverse Variance Weighted, IVW)作为主要分析方法33。为确保结果的稳健性,还使用MR-Egger34、加权中位数法和加权众数法进行了补充分析35。

敏感性分析:通过 mr_heterogeneity() 函数使用 Cochran’s Q 检验36 评估工具变量(IVs)间的异质性。使用 MR-Egger34 截距检验(mr_pleiotropy_test())评估水平 pleiotropy。

异常值检测:本研究采用 MR-PRESSO 软件包检测潜在的异常值37。使用 mr_presso() 函数进行 1,000 次模拟分析。移除识别出的异常值 SNP(P < 0.05),并重新计算因果效应估计值,以验证结果的稳定性。此外,还进行了留一法分析,以确保因果关联并非由单个 SNP 所驱动38。

统计学校正

为校正多重检验,使用错误发现率(FDR)方法对MR分析得到的P值进行校正。该方法通过R语言中的P.adjust()函数实现,参数设置为method = "fdr"。校正后的P值(PFDR)< 0.05的关联被认为具有统计学显著性。

结果

选定的工具变量

本研究采用与中性粒细胞胞外诱捕网(NETs)相关的炎症性状作为暴露因素进行孟德尔随机化分析。共确定了106个工具变量用于此目的,具体信息见补充表3。这些工具变量的F统计量平均值为26.46,最小值为20.6,最大值为250.4。

以2型糖尿病及其多种并发症(糖尿病酮症酸中毒、肾脏并发症、眼科并发症、神经系统并发症和外周循环并发症)作为结局时,三个SNP的信息无法在汇总数据中匹配。本研究使用了替代SNP(补充表4)。
