本研究通过文献计量学方法,系统描绘了2016年至2025年系统性红斑狼疮与巨噬细胞领域的研究格局及发展前沿。

摘要

本研究基于Web of Science核心合集,采用文献计量学方法系统分析了2016年至2025年发表的1,197篇关于巨噬细胞在系统性红斑狼疮(SLE)中作用的文章。分析内容包括出版趋势、合作网络、研究主题和知识基础,旨在阐明该领域的当前研究格局、核心研究力量及前沿演变。2022年发文量达到峰值,为154篇,而年度被引频次总体从2016年的184次上升至2025年的9,030次,表明该领域研究活跃且具有重要影响力。中国在发文数量上位居第一(379篇文章),而美国在总被引频次(18,838次)和国际合作中心性方面居首。上海交通大学和卡罗林斯卡学院是最具生产力的机构,各发表23篇文章。《免疫学前沿》(Frontiers in Immunology,影响因子 = 5.9;《期刊引证报告》Q1区)是最具生产力的期刊。关键词时间线聚类识别出13个主要研究主题,包括固有免疫的关键组成部分,如中性粒细胞胞外诱捕网、树突状细胞和Toll样受体,以及涉及T细胞、B细胞和浆细胞样树突状细胞的适应性免疫。这些主题还包括核心机制,如巨噬细胞极化、含NOD样受体家族吡啶结构域3(NLRP3)炎症小体和凋亡细胞清除,以及临床热点,如狼疮性肾炎、巨噬细胞活化综合征和类风湿关节炎。研究热点的演变反映了从早期关于凋亡细胞和免疫复合物清除的基本机制,发展到对细胞因子、固有免疫和适应性免疫的精细调控,最终转向针对信号通路的治疗、临床管理及多中心验证研究,“通路”和“多中心诊断”成为当前的研究前沿。

引言

系统性红斑狼疮(SLE)是一种慢性自身免疫性疾病,其特征是产生自身抗体、免疫复合物沉积以及多器官炎症1,2。尽管近几十年来在诊断方法和治疗策略方面取得了显著进展,SLE 仍持续对肾脏、血液系统和中枢神经系统造成严重损害3,4,5。现有的治疗方法,包括糖皮质激素、免疫抑制剂和生物制剂,通常疗效有限,并在不同程度上伴随不良反应6,7,8。细胞工程策略,例如嵌合抗原受体T细胞(CAR-T)疗法,已显示出通过深度清除自身反应性B细胞从而实现长期无药缓解的潜力9,10,11。作为先天免疫系统的关键成员,巨噬细胞在清除免疫复合物、吞噬凋亡细胞以及分泌细胞因子等方面发挥着重要作用,从而维持免疫稳态11,12。越来越多的证据表明,多种形式的巨噬细胞功能障碍参与了 SLE 的发病机制13,14,15。在狼疮性肾炎中,巨噬细胞向促炎型 M1 与抗炎型 M2 表型的极化失衡现象持续被观察到,其中 M1 巨噬细胞促进组织损伤,而 M2 巨噬细胞则参与组织修复及纤维化重塑16,17。中性粒细胞胞外诱捕网(NETs)的过度形成以及干扰素信号通路的异常激活进一步放大了巨噬细胞介导的炎症反应18,19,20。因此,巨噬细胞功能障碍不仅已成为 SLE 发病机制中的核心环节,同时也成为一个有前景的治疗靶点21

过去十年间,关于巨噬细胞在系统性红斑狼疮(SLE)中作用的研究文献迅速增长,涵盖多种研究方向,包括胞葬作用(efferocytosis)、中性粒细胞胞外诱捕网形成(NETosis)、巨噬细胞极化、代谢重编程以及信号通路的靶向调控22,23,24,25,26。然而,文献数量的激增也带来了诸多挑战,例如研究主题分散、核心研究力量识别不清、前沿演进脉络模糊,凸显出对当前研究格局进行系统性梳理的迫切需求。文献计量学采用数学与统计方法,对知识载体的分布特征与演化规律进行定量分析,可为论文发表趋势、合作网络、研究热点及前沿动态提供客观数据支持27,28,29,30,31。尽管文献计量方法已在SLE免疫治疗、生物标志物发现等相关领域成功应用于知识图谱构建,但针对SLE中巨噬细胞研究的专门性文献计量分析仍较为有限32,33,34。本研究从Web of Science核心合集数据库中检索2016年至2025年间相关文献,利用CiteSpace、VOSviewer和Scimago Graphica开展多维度可视化分析。通过考察论文产出情况、国家/地区与机构间的合作网络、作者与期刊关系网络、关键词共现与时间线聚类,以及引文突现检测,旨在系统阐明SLE中巨噬细胞领域的当前研究现状、核心研究力量、主题演变路径及新兴前沿方向,从而为后续研究提供客观的知识图谱与方向性参考。

方案

本研究无需伦理审批或知情同意,因为所有数据均来源于Web of Science核心合集数据库中公开可获取的文献记录。本研究未涉及人类受试者、动物或临床样本。

数据采集与检索策略

数据于单一日期(2026年4月27日)从Web of Science核心合集(WOSCC)数据库中检索获取,以确保数据的一致性和可重复性。检索式采用布尔运算符,结构如下:TS=("系统性红斑狼疮" OR "系统性红斑狼疮" OR "弥散性红斑狼疮") AND TS=("巨噬细胞*")。文献类型限定为文章和综述文章。发表时间范围为2016年1月1日至2025年12月31日。共保留1,197篇有效论文用于后续的文献计量分析。

分析工具

采用了一个整合多种软件工具的分析流程。使用CiteSpace(研究资源标识符[RRID]:SCR_020932)进行关键词时间线聚类和引文突现分析35。使用VOSviewer(RRID:SCR_016598)构建并可视化国家/地区、机构和作者之间的合作网络,以及生成关键词共现图谱36。使用Scimago Graphica对从VOSviewer导出的网络文件生成国家/地区合作网络的可视化图谱37。基础统计计算和图表制作使用WPS Excel和GraphPad Prism(RRID:SCR_002798)完成。整体文献计量分析流程如图1所示。

CiteSpace 和 VOSviewer 中的参数配置

在 CiteSpace 中,时间切片设置为从 2016 年到 2025 年,以 1 年为间隔。节点选择阈值采用 g 指数(k = 10)。Pathfinder 剪枝算法应用了两次:第一次用于简化每个独立的时间切片网络,第二次在合并所有时间切片后进行,以降低可视化复杂度。在 VOSviewer 中,选择 LinLog/模块度算法进行网络布局优化。节点大小根据文献数量或引用频次加权,具体取决于分析类型。在使用 Scimago Graphica 进行国家/地区合作图谱绘制时,节点直径反映发文量,连接线粗细表示国家之间的合作强度。

年度出版物产出

2016年至2025年间,共发表了1,197篇关于系统性红斑狼疮(SLE)中巨噬细胞的研究文章。年度发表数量有所波动,从2016年的112篇开始,于2022年达到154篇的峰值,随后在2025年略有下降至133篇。在同一时期,年度被引频次总体上从184次增长至9,030次,虽有小幅波动,但表明该领域的学术影响力持续上升(图2)。
