Gastric Fundus Mobilization

Gastric fundus mobilization is a surgical technique that frees the upper portion of the stomach so it can be repositioned during operations involving the esophagus or gastroesophageal junction. The procedure typically releases the fundus from surrounding attachments, including the gastrosplenic ligament and short gastric vessels, while preserving adequate blood supply through the gastric and gastroepiploic vessels. Mobilization improves the fundus’s reach and flexibility, allowing surgeons to create a tension-free fundoplication or use the stomach in reconstructive procedures. Careful dissection helps limit bleeding, preserve tissue viability, and support functional outcomes in treatments for gastroesophageal reflux disease, hiatal hernia, and selected esophageal conditions.

Fundus photography normally requires specialized fundus cameras that are not always available in all clinical settings. Here, a simple method to record ocular fundus images using a smartphone camera and a conventional high-plus handheld indirect ophthalmoscopy lens is described.

The retinal pigment epithelium (RPE) supports the sensory retina through recycling visual cycle byproducts, which accumulate as lipofuscin. These products are autofluorescent and can be qualitatively imaged in vivo. Here, we describe a method to quantitatively image RPE lipofuscin using confocal scanning laser ophthalmoscopy.
