Interictal Epileptiform Discharges

Interictal epileptiform discharges (IEDs) are abnormal, brief patterns of synchronized neuronal activity that appear on an electroencephalogram (EEG) between epileptic seizures, making them important biomarkers in epilepsy evaluation. They typically appear as spikes, sharp waves, or spike-and-wave complexes, reflecting transient hypersynchronous activity in cortical networks without producing an overt seizure. In neuroscience and clinical research, EEG analysis of IEDs supports epilepsy diagnosis, helps characterize seizure types and potential seizure-generating regions, and provides an objective measure for studying network excitability. Because IEDs can be infrequent and may occur without clinical symptoms, interpretation requires appropriate recording conditions and expert correlation with patient history.

High Frequency Oscillations (HFOs) have emerged as presurgical biomarkers for the identification of the epileptogenic zone in pediatric patients with medically refractory epilepsy.
