Dechallenge Rechallenge

Dechallenge-rechallenge is a pharmacovigilance approach used to evaluate whether a medicine caused a suspected adverse drug reaction. During dechallenge, the drug is discontinued or its dose is reduced, and improvement is assessed as drug exposure and pharmacologic effects decline; during rechallenge, the drug is reintroduced to determine whether the reaction returns. A positive dechallenge followed by a positive rechallenge strengthens the assessment of causality, although drug half-life, disease fluctuation, and alternative explanations can affect interpretation. This method supports adverse-event investigation, regulatory safety monitoring, and clinical decisions, but rechallenge may be inappropriate or unethical when the reaction is severe.

In this video, we demonstrate the repetitive tumor challenge assay to evaluate the CAR T cells’ repetitive tumor killing potential during high tumor cell loads. Further, the CAR T cells can be analyzed using flow cytometry to determine the different subpopulations of CAR T cells in the sample.
