1. 运行检测前的准备
注意：甲型和乙型流感检测适用于鼻咽拭子样本以及保存在病毒运输培养基中的鼻咽拭子。试剂盒中包含拭子，应使用这些拭子以获得最佳性能。然而，也可使用棉质、泡沫、植绒或聚酯鼻拭子来采集鼻拭子样本。

	<li>采集鼻拭子样本时，将拭子插入分泌物最明显或堵塞最严重的鼻孔。轻轻旋转并推进拭子，直至遇到阻力，然后在鼻腔壁上多次旋转，再缓慢从鼻孔中取出。将拭子收集后置于含有 3 ml 病毒运输介质（VTM）的试管中进行储存和运输。 
	注意：必须谨慎规范地进行样本采集，因为不充分的样本采集可能导致错误结果。 
	注意：采集后的拭子样本应尽快进行检测。若无法立即检测，可在室温下保存最多 2 小时，或在 2–8 °C 冷藏最多 24 小时。新鲜采集的样本最有利于实现最佳检测性能。不恰当的样本处理、储存和/或运输可能导致错误结果。 
	注意：作为预防措施，执行本检测的人员在处理样本时应遵循普遍性防护原则，将所有样本视为可能具有传染性。</li>
	<li>检测前将所有样本恢复至室温。 
	注意：所有检测试剂组件仅限一次性使用，不得用于多次检测。不得混用不同批号的试剂盒组件。不得使用超过有效期的试剂盒试剂。</li>
	<li>检测前将蓝色样本接收器恢复至室温。橙色测试底座无需预热至室温即可使用。</li>
	<li>所有测试组件应在使用前一刻才从铝箔包装中取出。按下仪器侧面的电源按钮以开启仪器，输入用户 ID 后按“确定”。</li>
</ol>
2. 运行检测
<ol>
	<li>开始测试流程时，触摸仪器屏幕上的“Run Test”按钮。使用屏幕上的键盘或条形码扫描器输入患者编号，然后按“OK”。打开盖子，将橙色测试底座插入橙色测试底座支架中。</li>
	<li>确认屏幕上显示的是正确的测试项目，然后按“OK”。将蓝色样本接收器插入蓝色样本接收器支架中。 
	注意：请勿在将样本接收器放入仪器前打开其盖子，否则会导致洗脱缓冲液无法达到所需的工作温度，可能影响测试性能。</li>
	<li>等待样本接收器预热。当仪器提示时，撕开铝箔封口，将待检测的患者拭子放入样本接收器中。 
	注意：撕开铝箔时，请用两根手指按住样本接收器边缘，以确保其保持原位。</li>
	<li>用力将拭子在液体中混合10秒，混合过程中将拭子头部反复按压样本接收器内壁，以帮助将样本从拭子上脱离。按“OK”继续。若检测的是保存在病毒运输培养基（VTM）中的拭子，请先将VTM涡旋震荡10秒，然后取200 µl加入样本接收器中。</li>
	<li>将白色转移 cartridge 压入蓝色样本接收器中。持续按压样本接收器，直到橙色指示条上升。</li>
	<li>提起转移 cartridge 并将其连接至测试底座。当 cartridge 正确连接后，观察橙色指示条下降。关闭盖子，在屏幕显示&#34;Test Complete&#34;信息前请勿打开。</li>
</ol>
3. 质量控制

	<li>在主屏幕上点击“运行质控测试”。选择要运行的质控测试。通过点击“确定”确认测试类型与拟用于测试的质控样本一致，并按照屏幕提示完成测试。</li>

4. 结果解读

5. 维护与清洁

每年流感病毒感染都会导致大量的发病和死亡1,2,3。单纯性流感的特征是出现全身性和呼吸道症状，例如发热、肌痛、头痛和干咳4,5。老年人、幼儿、免疫功能低下者以及患有基础合并症的患者发生肺炎、心肌炎、中枢神经系统疾病或死亡等严重并发症的风险更高6,7。
流感感染与其他呼吸道病毒不同，症状出现后48小时内及时给予抗病毒治疗可减轻疾病严重程度并缩短病程8。快速识别流感还被证实可减少不必要的抗生素使用9,10。此外，感染流感的住院患者必须安置在隔离病房，并采取适当的感染控制措施。然而，由非流感病毒引起的呼吸道疾病在临床上难以与流感区分。因此，快速且准确的流感诊断检测对于临床患者管理至关重要。
目前有多种检测和鉴定流感病毒的实验方法。快速流感抗原检测（RIDT）因其操作简便，且可在15至30分钟内获得结果，已被广泛用作临床实践中的即时检测手段11,12；然而，其敏感性因生产厂家、检测人群以及流感病毒类型和亚型的不同而存在较大差异（10–80%）13,14,15。直接荧光检测法（DFA）的敏感性优于RIDT，但检测耗时较长（约3小时），且必须由经验丰富的技术人员完成16,17。病毒培养曾是流感诊断的金标准，其敏感性高于RIDT和DFA18。然而，流感病毒培养通常需要2至14天才能完成，限制了其在患者管理中的实际应用价值19。最后，核酸扩增检测（NAAT）已取代培养技术，成为流感诊断新的金标准。NAAT被认为具有最高的检测敏感性，可在数小时内完成检测。但NAAT也是成本最高的检测方法，且需要专用设备和技术人员进行操作5,20,21,22,23,24,25。
本文所述的甲型和乙型流感检测是首个获得CLIA豁免且易于获取的分子快速流感检测方法。该检测采用切刻内切酶扩增反应（NEAR），利用等温扩增技术及针对流感病毒的特异性引物，随后通过分子信标探针进行靶标检测。该检测可区分甲型与乙型流感，单个样本的设置和处理仅需2分钟，总检测时间共需15分钟。

Rapid Molecular Detection and Differentiation of Influenza Viruses A and B

本研究中，于2012年12月15日至2013年3月1日流感暴发期间，在纪念斯隆-凯特琳癌症中心（MSKCC）因流感样症状住院的患者中收集了存档的鼻咽拭子（NPS）标本。NPS标本采集于3 ml病毒运输液（VTM）中，并作为常规临床检测的一部分，使用一种可检测多种呼吸道病毒（RP）的分子检测方法进行检测，包括甲型流感病毒、A-1、A-3和乙型流感病毒。在研究期间，共有3,675份NPS标本被送至MSKCC临床实验室进行检测。其中，分别有45份、425份、37份和77份标本经RP检测呈甲型流感病毒A-1、A-3、未分型甲型流感病毒（A-u）和乙型流感病毒阳性。共选取360份阳性标本，其中每种甲型流感病毒亚型各40份，A-u型37份。此外，针对每份阳性标本，选择在实验室接收时间上紧邻的1至2份阴性标本，用于后续甲型和乙型流感病毒检测。
