1. 术前方案
<ol>
	<li>在门诊评估脊柱病变患者，并确定其是否符合脊柱手术的适应证。进行神经系统评估，并获取CT或MRI扫描以明确脊柱病变。</li>
	<li>纳入患有神经鞘瘤、室管膜瘤、脑膜瘤、星形细胞瘤等硬膜内病变的患者， 等等。或存在腹侧硬膜外压迫性病变的患者，例如胸段椎间盘腹侧突出、腹侧骨折碎片，或伴有腹侧压迫的脊柱骨性肿瘤。 
	注意：病理诊断需通过脊柱CT或MRI影像确定。排除标准包括无法耐受手术的患者，或预后极差的患者。</li>
</ol>
2. 手术准备
<ol>
	<li>手术前夜午夜后禁止患者经口进食任何食物或饮水。 
	注意：患者将由麻醉医师实施全身麻醉并进行气管插管。</li>
	<li>根据外科医生对脊柱手术的要求，将患者摆放至背部暴露的体位。</li>
	<li>使用聚维酮碘对手术区域进行擦洗，完成手术部位的消毒。</li>
</ol>
3. 手术
注意：本部分实验方案遵循通用的脊柱外科手术技术，相关内容可参考任何权威的脊柱外科手术技术教科书19。
<ol>
	<li>使用手术刀沿脊柱相应椎体水平的纵轴做切口，并继续直线切开直至暴露骨组织。 
	注意：切口大小取决于病变范围。例如，若肿瘤累及两个椎体节段，则至少需要暴露两个椎体节段。当骨组织暴露后，可使用便携式X光机进行X线检查，以确认正确的椎体水平。</li>
	<li>使用电外科烧灼器进行骨膜下剥离，暴露棘突（可见为膨大的骨性突起）。将切割缘转向腹侧，并横向清扫椎板区域。</li>
	<li>联合使用Leksell骨钳和高速磨钻切除骨性椎板及棘突，以暴露其下方的黄韧带。</li>
	<li>使用角度刮匙和Kerrison咬骨钳切除黄韧带，显露其下方的硬脑膜。</li>
	<li>使用双极电凝和止血材料实现止血。 
	注意：获得高质量超声图像的前提是保持术野清洁。</li>
</ol>
4. 术中超声
<ol>
	<li>使用便携式超声设备及直径为 20 mm 的换能器探头。 
	注意：探头频率范围应为 10 至 4.4 MHz。任何具有相似探头直径和频率范围的可比设备均可使用。</li>
	<li>完成骨质切除并暴露硬脑膜后，在手术区域注入足够的生理盐水，使超声探头可完全浸入其中。 
	注意：通常需要 100–500 mL 生理盐水。生理盐水可实现声学耦合。</li>
	<li>开启超声设备，将超声探头置于感兴趣区域水平的盐水环境中，开始获取图像。 
	注意：探头无需直接接触硬脑膜或脊髓。图像将在超声屏幕上实时显示，外科医生可立即进行判读。随时按下 冻结 按钮可捕获屏幕图像，并通过按下 保存 按钮进行存储。</li>
	<li>将超声探头沿脊髓管走行方向对齐，于纵切面获取实时图像，以观察脊髓及病灶，其成像效果类似于磁共振成像（MRI）的矢状位图像。</li>
	<li>将超声探头垂直于脊髓管放置，于横切面获取实时图像，以观察脊髓及病灶，其成像效果类似于磁共振成像（MRI）的轴位图像。</li>
	<li>获取实时图像，以确认无法直接观察到的病灶位置，与术前 CT 或 MRI 图像进行对照，指导手术器械的定位，和/或确认病变的清除情况。 
	注意：必要时，可使用一小块无菌压缩海绵（约 0.5 cm × 0.5 cm）作为强回声手术标记物，将其置于手术区域，以帮助将手术位置与图像位置相对应。这有助于术中定位病灶，并有助于识别肿瘤边界。</li>
</ol>
5. 术后随访
<ol>
	<li>出院后，让患者在一个月内返回门诊进行随访。</li>
	<li>进行神经系统评估以及CT或MRI扫描，以确认症状和病理的缓解情况。</li>

超声是医学中最常见的诊断工具之一，尤其适用于腹部、四肢和颈部病变的可视化。然而，目前其在颅内和脊柱病变检查中的应用尚不广泛。1978年，Reid首次报道了使用超声成像技术观察颈髓囊性星形细胞瘤1。在该研究中，通过让患者屈颈以打开椎板间窗进行扫描。四年后，即1982年，Dohrmann和Rubin报道了在10例患者中术中使用超声观察硬膜内空间的应用2。这10例患者中通过术中超声识别出的病变包括脊髓空洞症、脊髓囊肿以及髓内和髓外肿瘤。他们进一步展示了术中超声在引导导管和探针进行肿瘤活检、囊肿引流以及脑室分流导管置入中的应用3。这使得探针/导管的位置能够实时监测并精确定位，从而减少放置过程中的误差和不准确性。在这些初步报道之后，陆续有其他研究发表，报道了术中超声在引导脊髓囊肿引流、髓内及髓外肿瘤切除以及脊髓空洞-蛛网膜下腔分流导管置入中的应用4,5,6,7,8,9,10。此外，研究还表明，术中超声可提高轴内实性脑肿瘤和脊髓硬膜内肿瘤全切除率11,12。术中超声还被证实有助于在组织操作前进行术中手术规划，并在脊柱骨折患者中对神经结构充分减压情况进行后续可视化评估7,9,13,14,15。
随着新型术中技术的发展，软组织的可视化效果得到显著提升，例如磁共振成像（MRI）和计算机断层扫描（CT），因此术中神经外科医生如今较少使用且更不倾向于采用术中超声作为术中影像技术16。然而，在某些手术病例中，术中超声相较于这些新技术仍具有优势（表1）。研究表明，与术中CT（iCT）或锥形束CT（cbCT）相比，术中超声在显示硬膜内结构的软组织方面具有更佳的可视化效果9,17。尽管术中MRI（iMRI）因其更高的软组织分辨率而在可用情况下具有实用价值，但其成本高昂、耗时较长，且无法提供实时图像6,16,18。例如，在硬膜囊腹侧存在一个外科医生无法直接观察到的硬膜内占位性病变时，术中超声便体现出其优势。此外，尽管术中超声的操作依赖于操作者经验，但根据我们的经验，该技术使用相对简便，且无需放射科医师即可轻松解读图像。

在正常脊柱超声成像中，硬脊膜表现为围绕无回声脊髓液的一层强回声结构。脊髓因其均匀的低回声表现而可被识别，其周围环绕着一层强回声环。该强回声环是由于从脊髓液到脊髓组织的密度变化所致。中央管表现为中央明亮的回声，而穿出的神经根则呈高度回声，尤其在马尾区域更为明显16。术中超声在硬膜内肿块病变切除术中可发挥重要作用。在常规情况下，术前CT或MRI可大致确定硬膜内肿块相对于已知邻近结构的位置。基于此估计，进行硬脊膜切开，通常需向两侧延长切口以充分暴露病变区域。在马尾肿瘤病例中，病变可能相对于术前影像发生头侧移位20。借助术中超声，可在打开硬脊膜前清晰显示病变位置，从而使硬脊膜切开更准确、更适当地定位至肿块的确切位置20,21。此外，对于需要切开脊髓以到达肿瘤的髓内病变，使用术中超声引导外科医生可降低神经损伤及后续神经功能缺损的风险22...

脊柱手术中的术中超声对于髓内病变或腹侧脊髓病变非常有用，可使外科医生在打开硬脑膜前即能观察到病变。这还能帮助外科医生定位病变位置，并在切除病变或肿瘤后确认脊髓是否得到充分减压。术中超声可提供实时信息，对外科医生在手术过程中的决策极具价值。该技术的主要优势在于，可在术中实时观察硬膜内的病变，然后再切开硬膜。在开始手术前，需进行磁共振成像（MRI）扫描，以确定脊柱病变的位置。全身麻醉后，将患者摆放到背部暴露的体位，并使用聚维酮碘溶液对术区进行消毒。接下来，使用手术刀在相应椎体水平的脊柱纵轴上做一切口。继续切开深层组织，直至暴露骨组织。采用电外科烧灼法进行骨膜下剥离，暴露棘突。将电刀的切割缘朝向腹侧，扫过椎板，结合使用Lexer骨钳和高速磨钻切除骨性椎板及棘突，以暴露其下方的黄韧带。使用角度刮匙和Karrison骨凿切除黄韧带，显露其下方的硬脑膜。随后使用双极电凝和止血基质实现止血。完成骨质切除并暴露硬脑膜后，用100至500 mL生理盐水填充手术区域，使直径为20 mm的超声探头能够浸入其中。打开便携式超声设备，将超声探头置于盐水浴中目标层面，开始采集图像。为了观察类似于MRI矢状位图像的脊髓及病变结构，需沿脊髓管走行方向移动探头，并在纵向平面采集实时图像。为了可视化脊髓及病灶，可将探头垂直于椎管放置，以获取与磁共振成像（MRI）类似的轴向图像，并在横断面实时成像。随后通过实时成像验证无法直接观察到的病灶位置，将其与术前计算机断层扫描（CT）或MRI图像进行对照，以指导手术器械的定位和/或确认病变的消除情况。术前影像可大致确定硬膜内肿块相对于其已知邻近结构的位置。此处，术中超声与术前 MRI 影像结果一致，显示病变部位上方存在液体聚集。术中轴位超声显示，该肿块累及脊髓的大部分区域，使用一块 0.5 厘米 × 0.5 厘米的无菌压缩海绵确认肿瘤的边界。随后在切除后再次进行术中超声检查，以确认肿瘤已被完全切除，且占位效应已解除。对于无症状的胸椎间盘突出，通过后路手术切除时，术中采用超声辅助评估减压效果，并确保所有压迫性椎间盘碎片均被彻底切除。类似地，在腰椎爆裂性骨折病例中，术中超声有助于确认减压充分以及所有碎片已被清除。因此，在脊柱手术中进行术中超声检查时，重要的是保持耐心，扩大扫描区域的视野，并使用较大的探头以获得清晰的影像。为患者进行检查，以获得脊髓清晰的轴向和矢状面图像，使超声图像能够清楚地显示脊髓与肿瘤或病变之间的关系。
