1. LSCI 设置
<ol>
	<li>打开计算机及所有外设。</li>

2. 系统验证

3. 受试者准备

	<li>在开始任何测量前，让患者静息15分钟。</li>

4. 微循环图像测量
<ol>
	<li>在工具菜单中，选择并点击项目编辑器。将打开一个新窗口，可在其中保存常用设置。</li>
	<li>在项目框中，点击新建以创建一个新项目。输入“Vestibulum”，然后点击确定。</li>
	<li>在站点框中，点击新建以创建一个新站点。输入“Tooth 14”，然后点击确定。</li>
	<li>在Tooth 14 内容面板下，将工作距离设为所需的“10 cm”，并在测量框中输入宽度 3 cm 和高度 2 cm。</li>
	<li>将点密度分辨率设为“正常”，帧率设为 16 张/秒，并从持续时间下拉菜单中选择时间，将录制时长设为 0:30。</li>
	<li>选择“无平均值录制”，并将彩色照片捕获速率设为每秒 1 次。</li>
	<li>然后点击“应用”和“确定”以保存项目参数。</li>
	<li>在文件菜单中，选择并点击&#160;新建录制。将打开一个新的图像窗口&#160;，同时显示设置面板&#160;。</li>
	<li>在录制设置下，为项目选择“Vestibulum”，为 4.9. 站点选择“Tooth 14”。</li>
	<li>打开受试者下拉菜单，在选择受试者对话框中点击“新建”，并输入患者姓名。</li>
	<li>点击确定，并在录制名称字段中输入本次录制的名称：例如，术后第 1 天（术后经过天数），并在操作员字段中输入操作员姓名。</li>
	<li>在开始微循环图像测量之前，测量患者的血压和脉搏。</li>

	<li>重复步骤 4.14&#160;至 4.21&#160;两次。</li>
	<li>关闭文件。数据将自动保存。</li>
	<li>在 LSCI 测量完成后，再次测量血压和脉搏。&#160;</li>

5. 离线分析

	<li>使用内置软件分析LSCI图像。进入图像或分屏视图&#160;（图2）。</li>
	<li>定义感兴趣区域（ROI）。注意：ROI内各像素的灌注值将被取平均，并定义为该ROI的血流值，以一个称为激光散斑灌注单位（LSPU）的任意单位表示。</li>
	<li>在右侧的ROI工具面板中选择所需的ROI形状。</li>
	<li>在ROI工具面板中选择应用选项，该操作会将ROI处理应用于记录中的所有图像。</li>
	<li>在强度图像中点击并按住鼠标按钮，拖动以调整ROI至所需大小，然后释放鼠标按钮（对于自由形状的ROI，请点击并双击）。如有需要，可调整ROI的位置、大小或旋转角度。</li>

激光散斑衬比成像（LSCI）方法对工作距离的设置和运动伪影均具有高度敏感性。本研究中，工作距离固定为10 cm。测量区域为2.7 cm × 2 cm，相当于约三个牙齿宽度的牙龈区域。有效帧率为16幅图像/秒，即每幅图像0.06秒。由于动脉脉搏会引起牙龈微循环的搏动性变化9，必须通过平均化处理从记录中消除该影响。快速成像也有助于降低运动伪影的风险。然而，若设置不当或患者发生移动，应停止记录并在最佳条件下重新进行。

在临床情况下监测人体牙龈微循环的长期变化是口腔与牙周外科领域的热点话题。然而，对灌注情况进行可靠评估可能存在困难。目前仅有少数几种方法能够无创地测量人体黏膜血液循环的变化。其中两种方法均采用激光束1,2,3,4，但原理不同。激光多普勒血流仪（LDF）利用激光束的多普勒频移5,6，而激光散斑对比成像（LSCI）方法则基于背向散射激光光形成的散斑图样来测量红细胞流速7。
LDF 仅能进行单点测量，而传感器位置的可重复标准化是一项理想但难以实现的任务。另一个问题是，LDF 探头的直径较小（1 mm）2）。术前在预定位置进行测量过于局限，可能无法察觉术后循环变化，而水肿、组织切除、组织移位或植入移植物等因素会导致受累软组织术后几何形态发生显著改变。激光多普勒流量仪（LDF）的测量深度小于1 mm，因此当组织体积发生变化时，无法使用带有预置探头孔的牙科夹板。激光散斑成像（LSCI）无需特殊定位工具，可在数厘米范围内的区域进行测量2因此，可以全程监测手术部位的伤口愈合情况。此外，激光散斑血流成像（LSCI）能够以每秒数帧的速度生成彩色编码图像，显示血流灌注情况，空间分辨率高达20 μm。
本文介绍的LSCI设备主要用于需要在小测量区域实现高分辨率的动物研究应用。然而，由于人体口腔黏膜在不同区域（附着龈、边缘龈、前庭黏膜）的结构和组织学特征各不相同，血流循环也具有异质性8。因此，与通常用于人体检测的常规分辨率LSCI相比，高分辨率LSCI具有显著优势。
激光散斑成像（LSCI）仪器采用一种不可见的激光（波长785 nm）。光束发散后照射测量区域，形成散斑图案。CCD相机对被照射区域内的散斑图案进行成像。本系统所用的CCD相机具有1386 x 1034像素的有效成像区域，其分辨率为20–60 µm/像素，具体数值取决于测量区域的大小以及软件设置（低、中、高）。该相机每秒可采集16帧图像，若减小图像尺寸，速度可进一步提高，最高可达每秒100帧。血流灌注由内置软件计算，软件分析散斑图案的变化并量化对比度，所得的血流值通过颜色编码生成灌注图像。根据我们先前的研究结果，LSCI在评估牙龈血流灌注方面具有良好的重复性和再现性9。这表明，LSCI不仅适用于短期实验中对口腔黏膜微循环变化的监测，也适用于长期研究中对疾病进展或伤口愈合过程的追踪10。
本文通过一例临床病例报告表明，激光散斑衬比成像（LSCI）的高空间分辨率能够揭示异种胶原移植物的新生血管化模式。此外，该病例表明，LSCI 由于具有高可靠性，可灵敏地检测个体间的差异。这一点尤为重要，因为在牙周手术的临床试验中，不同病例之间显著的局部解剖变异以及不同的全身背景使得手术干预难以实现标准化。
