慢病毒（LV）载体工程技术是目前对CD34+造血干细胞和祖细胞（HSPCs）等细胞类型进行基因修饰并用于后续体内移植的特征最明确的方法。本文所述方案旨在最大化在体内长期存留的基因修饰HSPCs数量，从而为患有多种恶性疾病、感染性疾病和遗传性疾病的患者提供临床获益。尽管过去十年中基因编辑策略不断涌现，但LV修饰的细胞在体外、动物模型以及患者中的研究最为充分1,8,20,21,22。

干细胞基因治疗是应对多种人类病理状况的有力手段。造血干细胞和祖细胞（HSPC）基因治疗是一种特别有吸引力的方法，原因包括：i）从患者体内收集这些细胞相对容易；ii）目前已有大量关于细胞表面表型和体外培养参数的知识积累；并且随着该领域的发展，iii）科学家可获得的、针对不同目标疾病定制的基因修饰策略工具箱也在不断扩大。我们正在从多个角度积极研究HSPC基因治疗，包括HSPC生物学的基础科学、基因修饰HSPC在临床前体内模型中的植入效果，以及在相关患者人群中的应用。我们及其他研究团队已对功能上不同的HSPC亚群的细胞表面表型1,2,3、可最大化HSPC产量和植入效率同时最小化毒性的动员与预处理方案4,5，以及针对多种恶性肿瘤、遗传性疾病和感染性疾病的基因修饰与基因编辑策略6,7,8,9,10进行了系统表征。基因修饰HSPC的功能和植入能力可在多种小型和大型动物模型中进行评估，包括小鼠、犬类和非人灵长类动物（NHP）。特别是，NHP模型具有优势，因为许多试剂（例如，针对CD34和CD90等HSPC细胞表面蛋白的特异性抗体）可在人和NHP细胞中互换使用。此外，与小鼠相比，大型动物如NHP能更准确地模拟实现临床疗效所需的基因修饰规模。最后，NHP是模拟人类疾病（如HIV-1感染11）的金标准模型，也是候选抗癌和抗HIV免疫疗法的新兴模型系统12,13。
本方案旨在概述纯化、基因修饰及制备非人灵长类动物造血干细胞和祖细胞（HSPC）输注产品的相关方法。尽管本方案未涵盖以下内容，但我们先前的研究已证明，这些产品可在自体非人灵长类动物宿主体内成功植入，生成所有造血谱系，并在多种疾病模型中表现出治疗效果1。我们还对植入的HSPC的克隆性进行了表征，并建立了平台，用于追踪自体移植后单个HSPC及其子代细胞的动力学、迁移路径和表型特征1,14。本文所介绍的方法基于以下目标开发：i）分离高纯度的HSPC以及具有长期植入能力的造血干细胞（HSC）亚群；ii）在体外培养过程中维持原始HSC的未分化状态；iii）高效地对整体HSPC群体或具有长期植入能力的HSC亚群进行基因修饰。我们采用磁珠辅助细胞分选（MACS）以及荧光激活细胞分选（FACS）技术，分离具有不同表型/功能特征的HSPC群体，该策略与多个研究团队所采用的方法一致2,15,16。在体外培养中维持原始HSC的状态（即最大限度减少这些细胞向产生成熟淋巴系和髓系细胞的定向前体细胞分化）是本方案的关键环节。尽管我们此前已报道过在保持原始表型的同时扩增HSPC的方法17,18，但本文描述的方案侧重于通过极短（48小时）且明确的体外培养条件来维持HSC特性。
高效修饰HSPC及其HSC亚群是本方案的核心目标。在我们已报道的多种方法中，目前在临床试验中研究最为广泛的有两种：慢病毒载体（LV）介导的基因修饰和核酸酶介导的基因编辑1,6,19。&#160;基因编辑策略利用多种核酸酶平台之一，特异性地修饰目标基因，例如，靶向C-C趋化因子受体5型（CCR5）用于治疗HIV感染7,19，或靶向Bcl11A用于治疗血红蛋白病6。本文重点介绍LV介导的基因修饰，该方法中转基因载荷以半随机方式整合入基因组1,8,20。LV方法的一个关键优势在于其能够递送大量遗传物质（最多可达8至9 kb）。尽管当前正在开发基于同源供体重组（HDR）的基因编辑策略，以实现将目标转基因特异性地整合至指定基因座，但这些方法仍需在体外及小型动物模型中进一步优化。相比之下，慢病毒载体已在非人灵长类动物（NHP）和患者中得到广泛应用21,22。重要的是，本文所述方案以预激活的骨髓作为起始的HSPC来源，该方法可便捷且广泛地进行调整，例如用于分离外周血干细胞（PBSCs）。如上所述，我们充分利用了非人灵长类动物与人类之间高度的遗传相似性，从而使用适用于两个物种的实验试剂。最后，该方法也已被拓展用于修饰其他造血细胞亚群，特别是T细胞12,23,24；高效T细胞免疫治疗策略的发展在很大程度上依赖于本方案中所采用的相同LV平台。这些方法适用于所有对HSPC生物学或LV介导的基因修饰感兴趣的研究人员。例如，本文所展示的HSPC纯化方案可用于表征新发现的富集HSC的细胞亚群，如先前研究所述1,15,25。同样，本文所介绍的LV转导方法也可进一步拓展并应用于多种其他细胞类型及实验问题，无论是在体外还是体内模型中。
综上所述，我们介绍了分离和遗传修饰非人灵长类动物造血干细胞和祖细胞（HSPC）的方法。这些方法可轻松适用于其他物种及其他来源的HSPC。该经过充分验证的实验方案在模拟多种人类疾病的有效疗法方面展现出巨大前景。

Preparation and Gene Modification of Nonhuman Primate Hematopoietic Stem and Progenitor Cells

造血干细胞和祖细胞（HSPC）移植作为治疗白血病和其他癌症的核心疗法已有近半个世纪的历史，是目前唯一已知可治愈人类免疫缺陷病毒（HIV-1）感染的方法，并在β地中海贫血等遗传性疾病的治疗中展现出巨大前景。我们团队已建立了一种在非人灵长类动物（NHP）中模拟HSPC基因治疗的实验方案，使科研人员能够优化与临床应用相同的一系列试剂和技术。本文描述了从预处理后的骨髓（BM）中纯化CD34+ HSPC以及长期存在的造血干细胞（HSC）亚群的方法。相同的技术也可用于其他HSPC来源（例如，动员后的外周血干细胞[PBSCs]）的纯化。本方案为为期两天的流程，包括细胞纯化、体外培养、慢病毒（LV）修饰，以及为回输至自体宿主做准备。评估成功的关键指标包括CD34+ HSPC群体的纯度、纯化后HSPC在半固体培养基中形成形态学上可区分的集落的能力，以及最重要的——基因修饰效率。HSPC基因治疗的关键优势在于其能够提供可长期存活的细胞来源，并分化为所有类型的造血细胞。因此，这些方法已被用于模拟癌症、遗传性疾病和感染性疾病的治疗策略。在各类疾病模型中，均通过增强特定HSPC子代细胞（包括红细胞、T细胞、B细胞和/或髓系亚群）的功能来确立治疗效果。HSPC产品的分离、修饰和制备方法可直接应用于人类患者的多种疾病治疗，具有高度的可转化性。

上述方案旨在分离并进行基因修饰非人灵长类动物（NHP）的CD34+造血干细胞和祖细胞（HSPCs），随后可将这些细胞回输至自体宿主体内（图1 和 图2）。按照本方案操作时，我们通常可从经动员的食蟹猴骨髓中获得最多约 8 × 109 个总白细胞（WBCs），有时从恒河猴中获得的数量可达此值的两倍。在这两个物种中，我们富集得到的CD34+ HSPCs数量与输入的总WBC数呈正比（图3）。先前的研究表明，总的CD34+ HSPC产物中包含并非真正具有长期植入能力的造血干细胞（HSCs）。因此，我们开发了基于流式细胞术的分析方法（图4），并利用集落形成细胞（CFC）检测（图5-6）来鉴定培养物中的长期HSCs及定向祖细胞亚群...

为了评估有前景的新型基因治疗策略的有效性和安全性，在经过验证的临床前体内模型中对基因修饰细胞进行全面测试至关重要。非人灵长类动物模型具有相关细胞表面标志物的保守性、试剂的交叉反应性，并能够应用与患者相同的辅助治疗措施，从而紧密模拟临床参数。本次操作将由来自汉斯-彼得·基姆博士实验室的研究技术员Anai Perez进行演示。首先，将来自健康非人灵长类供体的10至12毫升骨髓等分试样加入50毫升锥形管中。用溶血缓冲液将管内液体体积补足至50毫升，并在室温下孵育细胞，时间不超过7分钟。通过离心去除细胞碎片，弃去上清液，仅保留最后2至5毫升。 通过轻弹管壁将沉淀物在残留的上清液中重悬，使用70微米孔径的细胞滤网将骨髓悬液转移至新的50毫升离心管中，以去除任何血凝块。加入新鲜的溶血缓冲液至终体积为50毫升，室温孵育5分钟，随后通过离心收集细胞。吸除全部上清液，并将细胞重悬于10毫升max缓冲液中。在将细胞悬液通过另一个70微米孔径的滤网过滤之前，用40毫升新鲜缓冲液冲洗试管，并将洗涤液与细胞悬液合并。根据标准的磁辅助细胞分选方案富集CD34阳性细胞后，用最多50毫升新鲜Max缓冲液洗涤磁珠分离的细胞，并将细胞沉淀重悬于HSPC培养基中。随后将10至20毫升细胞接种于透气的组织培养处理T75培养瓶中，在37摄氏度、5%二氧化碳培养箱中过夜培养。为评估骨髓细胞样本在富集前后的纯度，需建立一个流式细胞术检测方案，包含适当的图谱和细胞群层级结构，展示所有事件的散点图、CD34阳性细胞、造血干细胞、多能性非红系髓系祖细胞以及淋系-髓系祖细胞的设门策略。接着，运行一份未染色的富集前白细胞样本，以调整前向散射和侧向散射参数的电压，并校正所有荧光通道；随后使用单染补偿微珠，调节相邻荧光通道之间的补偿。最后，采用经电压和补偿调整后的检测方案，运行其余未染色和染色的样本，以记录CD34富集效率。当所有补偿设置完成后，配置细胞分选仪，用于将 CD34 亚群分选至集落形成细胞检测中，并将染色后的 CD34 富集样本管上机至流式细胞仪。随后记录 2000 至 3000 个事件，并根据信号强度和目标细胞群精细调整分选门。设置完成后，开始采集细胞，调节流速至每秒 500 至 1000 个细胞，并分别从侧向散射光、CD34 阳性、造血干细胞、多能非红系髓系祖细胞以及淋系-髓系祖细胞门中分选出 800 至 1200 个细胞，收集至各含有 3.6 毫升集落形成细胞培养基的独立试管中。分选完成后，收集离心管中的涡旋液，将1毫升细胞悬液分别加入三个无菌的3.5厘米非组织培养处理的培养皿中，每个培养皿置于一个15厘米培养皿内，每种细胞群体设置三份，于37摄氏度孵育10至14天。次日早晨，使用10毫升移液器，用无菌HBSS轻轻冲洗每个培养瓶底部的贴壁细胞，并通过离心收集收获的细胞。将细胞重悬于转导培养基中，调整浓度为每毫升1 × 10⁶个细胞。在重组纤连蛋白片段包被的12孔板中，向其中一个孔加入一毫升细胞，作为模拟对照，于细胞培养孵箱中过夜孵育。然后将剩余细胞分装为每份10毫升，加入重组纤连蛋白片段包被的T75培养瓶中，于细胞培养孵箱中孵育30分钟，瓶盖保持通气状态。孵育期间，解冻病毒条件培养基样品，并根据滴度及每毫升细胞的感染单位数计算所需添加的病毒量。从培养箱中取出培养瓶，加入适量的病毒条件培养基，然后将细胞放回37摄氏度、5%二氧化碳的环境中。次日早晨，将细胞从培养瓶转移至单独的50毫升锥形管中。每次用最多50毫升的HBSS洗涤细胞，并重复此步骤最多三次，以去除残留的病毒。将细胞在添加前列腺素 E2 的 HSPC 培养基中孵育两小时。然后洗涤细胞，并将细胞重悬于每管含 2% 自体血清的 10 毫升 HBSS 中。接着使用配备 16.5 号针头的 20 毫升注射器吸取细胞悬液。用新鲜的 HBSS 加 2% 自体血清冲洗管内壁，并吸入注射器中。在将细胞溶液置于冰上直至回输至自体宿主之前，请小心地用针头帽盖住注射器。为测定转导细胞的基因修饰效率，将每毫升HSPC培养基中保留的1 × 10⁶个模拟处理或转导细胞接种至非组织培养处理的培养板中。在转导后的第2、5和12天，收集细胞并进行计数。在第2天和第5天，将33%的细胞以每毫升1 × 10⁶个细胞的浓度重新接种至新鲜的HSPC培养基中。在第2、5和12天，将33%的细胞冻存于DNA提取缓冲液中，用于定量实时PCR检测；其余最后33%的细胞则按照标准操作流程进行流式细胞术分析，以评估造血后代的表型组成。为了通过流式细胞术定量检测转基因表达水平，额外增加三个侧向散射光 vs.转基因图谱；第一个图门控造血干细胞，第二个图门控多能性红系-髓系祖细胞，第三个图门控淋系-髓系祖细胞，均相对于转基因进行分析。然后记录2000至3000个事件，并微调分选门以适应信号强度和目标细胞群。采用本方案富集的非人灵长类动物 CD34 阳性 HSPC 数量通常与输入的总白细胞计数成正比。先前的研究表明，总的 CD34 阳性 HSPC 产物中包含并非真正具有长期重建造血功能的造血干细胞，因此可利用基于流式细胞术和集落形成细胞的技术，可靠地鉴定并区分富集的长期重建造血干细胞亚群与定向祖细胞。CD34 阳性人干细胞富集细胞可利用慢病毒载体高效地进行基因修饰。携带非整合型载体的细胞在为期12天的液体培养检测过程中会逐渐稀释，而稳定整合到基因组中的载体则会传递给所有子代细胞。这一点已通过集落形成细胞实验中载体的表达得到证实。该临床前方法可用于分离经过基因修饰的、质量可控的非人灵长类造血干细胞和祖细胞，用于基因治疗。该方法可轻松适用于其他物种、造血干细胞和祖细胞来源、基因治疗工具及疾病。这一经过充分验证的方案在模拟多种人类疾病的有效疗法方面展现出巨大前景。请注意，操作非人灵长类动物组织时需要加强安全措施，包括使用个人防护装备。
