1. 猪的外科麻醉与术前准备。
<ol>
	<li>对50 kg的长白-约克夏商用猪进行肌肉注射预麻醉，药物混合物包含盐酸氯胺酮（22 mg/kg）、盐酸赛拉嗪（2.2 mg/kg）和盐酸格隆溴铵（10 µg/kg）。</li>
	<li>在靠近猪头部的实验台末端设置所有用于监测临床参数的设备，确保设备不会妨碍对猪的操作。通过将压力换能器置于与心脏水平的水平面上，以获得准确的动脉血压测量值。</li>
	<li>使用吸入式异氟烷气体麻醉猪（500–1000 mL/min 氧气中含 4%–5% 异氟烷）2使用合适尺寸的面罩。用兽用喉镜（17–25 cm 长直型镜片）观察声带，并对声带喷洒局部10%利多卡因喷雾，以降低喉痉挛和气道阻塞的风险。</li>
	<li>通过声门插入带套囊的气管插管（9.0 mm 内径）对猪进行插管，并使用异氟烷气体（0.5%–3.0% 异氟烷，氧气流速 1000–2000 mL/min）维持麻醉2使用机械呼吸机（18–22 次/分钟）为猪通气，并确保所有呼出的麻醉气体均被有效回收并排至手术室外。通过下颌张力以及肢蹄反射和眼睑反射来评估麻醉深度。 
	注意：给予静脉乳酸林格液（LRS，10–50 mL/kg/h；参见步骤1.6），以改善血流动力学功能，并减轻异氟烷气体麻醉对猪心血管输出的抑制作用26.</li>
	<li>用医用胶带将脉搏血氧仪固定在舌体黏膜表面，以监测心率和外周血氧饱和度（SpO₂）。2）。将温度探头插入鼻腔约2-4 cm以监测体温。手术过程中将猪置于加热台上，以维持正常体温（38-40 °C）。</li>
	<li>通过适当的组织准备确保手术无菌性。
	<ol>
		<li>用10%聚维酮碘外科洗手液清洁耳部外表面，为静脉插管做准备，并让溶液自然风干。</li>
		<li>使用20 G、1英寸静脉导管穿刺耳缘静脉，以输注静脉液体（乳酸林格液；10–50 mL/kg/h）或追加其他麻醉药物。</li>
		<li>如有创操作需要，可通过持续静脉输注盐酸瑞芬太尼（0.05–0.14 µg/kg/min）加强猪的麻醉和镇痛效果。</li>
	</ol>
	</li>
	<li>将猪置于侧卧位，轻轻将前肢从肩部向外伸展约10–12 cm。剪除上臂内侧皮肤表面的毛发。通过触诊定位远端肱动脉搏动点。 
	注意：该动脉的标志性位置位于一个斜切面上，大约距离鹰嘴9 cm，距离肘关节屈侧5 cm。肱动脉向近端走行，穿过肱骨，延伸至肩胛骨的尾侧三分之一处。</li>
	<li>与步骤1.6类似，确保手术无菌操作并进行适当的组织准备。使用10%聚维酮碘外科洗手液清洁皮肤表面，并让溶液自然风干。用四个小型一次性手术巾覆盖肱动脉插管部位。</li>
</ol>
2. 肱动脉组织解剖与插管
<ol>
	<li>使用手术刀片做一个6 cm的皮肤切口，以暴露下方组织。用梅兹鲍姆剪刀沿肱二头肌内侧表面进行钝性分离，并加深解剖层次，直至识别出搏动的动脉。</li>
	<li>使用棉签轻轻分离肱动脉、正中神经和肱静脉周围的外膜；这些结构彼此靠近，位于同一筋膜平面内。解剖过程需轻柔操作，尤其应注意在操作过程中尽量减少对正中神经的损伤。肱动脉位于皮肤下方约2.0–2.5 cm处，位于喙肱肌内侧、前臂筋膜张肌外侧，并覆盖肱三头肌内侧头的一小段区域27,28。 
	注意：放置牵开器以保持切口开放，便于更方便地接触肱动脉。可选择性地放置第二个牵开器，以进一步协助血管暴露。</li>
	<li>在整个解剖过程中，使用温热的生理盐水（37 °C）润湿所有组织，以维持更好的结构完整性，并改善术中组织的操作性。</li>
	<li>用钝头镊子在动脉下方创建一个隧道，然后将三根2-0聚乳酸-羟基乙酸共聚物（polyglactin）缝线穿过动脉下方。有意将这些缝线的末端留得相对较长（3–4 cm），以便将导管固定于动脉上。在距离彼此约1.0 cm的前两根缝线上添加一个&#34;松结&#34;，以便快速固定导管，这两根缝线距离第三根远端缝线近端约1.5–2.0 cm。首先结扎最远端的缝线以闭塞动脉。</li>
	<li>将一根22 G、1英寸的外周静脉导管插入动脉，然后将导管（完全推进至导管接头）沿着导丝推入血管。部分撤出导管内的导丝，观察到动脉血回流，确认导管正确位于血管内。随后，通过结扎中间的缝线牢固固定导管。取出导丝，并迅速盖上导管帽以减少出血。</li>
	<li>用温热的生理盐水（37 °C）冲洗切口和导管。结扎最近端的缝线，并特别注意确保远端缝线紧密固定在导管接头上，这有助于提高导管稳定性，并减少导管意外滑脱（例如在猪体位调整过程中）的风险。 
	注意：如果导管初次插入动脉失败，或血管受损，可将导管重新插入至初始插入点近端约0.25 cm的位置。</li>
	<li>迅速将装有乳酸林格液（LRS）的静脉延长管连接至动脉压换能器，并与导管连接；然后用温热的生理盐水（37 °C）冲洗手术区域，保持组织湿润，并清除渗入周围组织的血液。用生理盐水冲洗导管，以确保导管通畅，并防止血液在导管壁上形成血栓。 
	注意：检查换能器动脉血压监测线路是否出现故障（如泄漏），通过将动脉压监测仪归零建立基线，并确认动脉血压波形正常。</li>
	<li>通过使用加压输液袋维持延长管冲洗端压力高于250 mmHg，持续以3–5 mL/min的速度输注乳酸林格液（LRS），以确保导管持续通畅。
	<ol>
		<li>可选：在导管接头或静脉延长管接头周围放置两根2-0聚丙烯缝线或两根2-0聚乳酸-羟基乙酸共聚物（polyglactin）缝线，以进一步增强导管在动脉内的稳定性。</li>

3. 组织闭合与体位摆放
<ol>
	<li>使用带切割针或锥形针的2-0聚乳酸羟基乙酸缝线，以单纯连续缝合法关闭肌肉层；使用带切割针的2-0聚丙烯缝线，以单纯间断缝合法关闭皮肤。 
	注意：关闭肌肉层和皮肤时，可交替使用2-0聚乳酸羟基乙酸缝线或2-0聚丙烯缝线。</li>
	<li>将侧卧位的猪腹部转向手术台，使其呈腹卧位。左侧卧位的猪顺时针旋转，右侧卧位的猪逆时针旋转。</li>
	<li>将已插管的前肢置于与猪脊柱中线成40°角的位置。此前肢定位可获得最佳的动脉血流和最准确的动脉血压测量值。</li>

在外科实验研究中，通过手术干预建立动物模型，以促进科学发展。科学文献中包含大量新型外科动物模型的实例1,2,3。手术操作是一个复杂的过程，不仅涉及解剖结构的处理，还包括麻醉和镇痛所需多种药物引发的复杂生理相互作用。这种相互作用可能引起动物体内生理过程的重大变化，因此需要对动物进行严密监测4。临床上，成功的手术结果与动脉血气和动脉血压的监测密切相关5。这些临床参数的获取依赖于准确测量动脉血压和有效采集动脉血样的能力，而这些操作的前提是成功完成动脉插管6,7。
动脉插管用于采集动脉血并测量血压，已在多种动物物种中应用5,6,7,8,9,10,11,12,13,14,15,16,17,18,19,20,21，且已应用于不同发育阶段的动物19,20,21，并适用于恢复性（临床和诊断）操作4,5,6,7,8以及非恢复性（实验性）操作14,15,16,17,18。此外，在选择用于动脉血压测量的动脉时，还需考虑动脉穿刺的难易程度以及该动脉在手术操作中的位置。例如，犬的中尾动脉、马的面动脉，以及犬和马的足底动脉，常用于恢复性操作期间的诊断性测量与监测6,7,8。相比之下，猪则常对颈动脉和股动脉进行插管，用于非恢复性实验或长期导管植入实验14,15,18。
在猪体内，为测量动脉血压或采集动脉血，常规采用颈动脉、股动脉、内侧隐静脉或耳动脉进行动脉插管22,23。对于特殊非常规操作，也曾使用其他较为罕见的动脉，例如锁骨下动脉和髂动脉，分别用于测量肱动脉解剖扭曲度17和成像主动脉16。无论选择哪条动脉进行插管，每条动脉在应用时均具有其固有的优势与局限性。例如，耳动脉在解剖上易于接近，但其应用可能受限于其与耳缘静脉的邻近关系11,12。相比之下，颈动脉相对粗大且坚固24，但其位于颈静脉沟深处，需进行较广泛的组织分离25。因此，有必要寻找另一条可用于测量动脉压和采集动脉血的动脉。本视频及文稿详细描述了在猪体内对远端肱动脉进行插管的技术，该技术可应用于非恢复性操作。值得注意的是，猪肱动脉插管曾用于腰椎手术期间监测动脉血压和动脉血气参数，并同时进行后肢相关测量（该部分手术的数据未在此展示）。
