该研究方案已获得同济大学口腔医学院/口腔医院伦理委员会的批准。所有患者均签署了书面知情同意书。
1. PDL 细胞分离
<ol>
	<li>制备牙周膜细胞（PDL细胞）增殖培养基：在α-MEM培养基中添加10%胎牛血清（FCS）和100 U/mL青霉素-链霉素溶液（pen/strep）。</li>
	<li>准备一个装有冰的容器，用于转运分离的第三磨牙。</li>
	<li>使用高压灭菌器对手术器械进行灭菌处理。</li>
	<li>在同济大学口腔医学院附属口腔医院牙科诊所，从成年患者（18–28岁）中拔除正常的阻生第三磨牙。</li>
	<li>将第三磨牙放入增殖培养基中，并在4 °C条件下转运至实验室。</li>
	<li>将拔除的牙齿置于无菌的100 mm培养皿中，在生物安全型超净工作台内操作。</li>
	<li>加入10 mL磷酸盐缓冲液（PBS）清洗牙齿，重复清洗步骤两次。</li>
	<li>使用无菌手术刀轻轻分离牙根表面的牙周膜。 
	注意：应从牙根中三分之一区域刮取牙周膜，以避免牙龈组织或根尖组织的污染。</li>
	<li>借助无菌刀片将牙周膜组织切成1–2 mm大小的组织碎片。</li>
	<li>将牙周膜组织碎片放入T-25培养瓶中，加入3–4 mL增殖培养基。</li>
	<li>将培养物置于37 °C、5% CO2及湿润空气的培养箱中进行培养。</li>
	<li>当观察到牙周膜细胞开始向外生长时，更换培养基。牙周膜细胞通常需要1–2周时间开始外生长，可通过倒置相差显微镜观察细胞生长情况。</li>
	<li>此后每周更换培养基两次。</li>
	<li>当牙周膜细胞达到汇合状态时，按1:4比例进行传代培养（第一代）。</li>
	<li>每日观察细胞生长情况，并每周更换培养基两次。</li>
	<li>后续每次传代均在细胞达到80%汇合度后按1:4比例进行。用于细胞接种的牙周膜细胞应选用第三代至第五代细胞。</li>
</ol>
2. 壳聚糖薄膜的制备
<ol>
	<li>配制1% (v/v) 乙酸溶液时，将5 mL乙酸加入495 mL双蒸水中，置于洁净的玻璃烧杯中混合。</li>
	<li>配制1% w/v 壳聚糖溶液时，称取5 g壳聚糖粉末（平均分子量400 kDa，脱乙酰度85%），加入495 mL的1% (v/v) 乙酸溶液中。</li>
	<li>将配制好的溶液置于60 °C条件下，使用磁力搅拌子和磁力搅拌器连续搅拌24小时。</li>
	<li>将配制好的溶液进行高压灭菌，以确保壳聚糖完全溶解。 
	注意：实验可在此步骤暂停。</li>
	<li>为制备壳聚糖薄膜，将1% w/v 壳聚糖溶液以0.25 mL/cm2的量加入组织培养级聚苯乙烯（TCPS）培养皿中。例如，在24孔板的一个孔中加入0.5 mL的1% w/v 壳聚糖溶液。</li>
	<li>将TCPS培养皿置于60 °C烘箱中干燥24小时，以形成壳聚糖薄膜。</li>
	<li>配制0.5 N氢氧化钠（NaOH）溶液。将10 g NaOH分批少量加入大量双蒸水中，使用磁力搅拌子搅拌溶解，然后定容至0.5 L。 
	注意：必须将NaOH加入水中，切勿将水加入固体NaOH中。切勿直接接触NaOH，因其可能引起化学灼伤。</li>
	<li>用0.5 N NaOH溶液中和壳聚糖薄膜。在24孔板的一个孔中加入0.5 mL 0.5 N NaOH溶液，作用2小时。</li>
	<li>用双蒸水清洗壳聚糖薄膜三次。 
	注意：实验可在此步骤暂停。</li>
	<li>在接种细胞前，将涂覆壳聚糖的24孔板置于70%乙醇中，在室温下过夜灭菌。</li>
	<li>在室温下用PBS冲洗涂覆壳聚糖的培养板三次。</li>
	<li>将涂覆壳聚糖的培养板在室温下经紫外光照射过夜，进行灭菌处理。</li>
</ol>
3. 细胞接种
<ol>
	<li>将增殖培养基 PBS 溶液和胰蛋白酶/EDTA 溶液预热至 37 °C。</li>
	<li>弃去 T-75 细胞培养瓶中的上清液。</li>
	<li>用 10 mL PBS 溶液洗涤汇合的 PDL 细胞。</li>
	<li>弃去 PBS 溶液。</li>
	<li>向 T-75 细胞培养瓶中加入 1 mL 胰蛋白酶/EDTA 溶液。</li>
	<li>在 37 °C 下，用胰蛋白酶/EDTA 溶液孵育细胞 3 分钟。</li>
	<li>向 T-75 细胞培养瓶中加入 3 mL 增殖培养基，终止胰蛋白酶的消化作用。</li>
	<li>将制备好的细胞悬液转移至 15 mL 圆锥形离心管中。</li>
	<li>在室温下，以 300 x g 离心 5 分钟。</li>
	<li>弃去 15 mL 圆锥形离心管中的上清液，并将细胞沉淀重悬于 500 µL 增殖培养基中。</li>
	<li>使用血细胞计数板对 PDL 细胞进行计数。</li>
	<li>以 0.5 x 104、1 x 104、3 x 104 和 6 x 104 个细胞/cm2 的密度接种 PDL 细胞。</li>
	<li>将 PDL 细胞在壳聚糖膜上接种后，置于含 5% CO2 的湿润空气中，于 37 °C 培养箱中培养。</li>
	<li>此后每周更换两次培养基。</li>
	<li>在第 1、2、3、4 和 5 天，每天通过倒置相差显微镜观察细胞形态。</li>
</ol>
4. 细胞存活率
<ol>
	<li>培养 1、3 和 6 天后，使用商用细胞活力/细胞毒性检测试剂盒测定细胞存活率。</li>
	<li>在 15 mL 离心管中，将 2 mM 钙黄绿素-AM 和 4 mM 碘化丙啶同型二聚体溶于 PBS 中，配制 2 mL 新鲜溶液，涡旋混匀 5 秒。 
	注意：操作过程需避光。</li>
	<li>移除培养基，并用 PBS 清洗类球体。</li>
	<li>将类球体置于含 2 mM 钙黄绿素-AM 和 4 mM 碘化丙啶同型二聚体的 PBS 中，在室温下避光孵育 30 分钟。</li>
	<li>用 PBS 漂洗类球体两次。对于来自 24 孔板一个孔的类球体，每次使用 2 mL PBS。</li>
	<li>使用荧光显微镜观察样品，放大倍数为 10× 或 20×。</li>

然而，该方法的一个局限性在于，牙周膜细胞在形成球状体后增殖能力下降14，这一现象与之前在脂肪基质细胞...

牙周炎主要由牙菌斑引发1，其特征是牙周组织（包括牙周膜（PDL）、牙槽骨和牙骨质）的破坏。目前对牙周炎的治疗通常能有效阻止活动性疾病的进展，但已丧失的牙周组织的再生仍是临床难题。近年来，在基于细胞的牙周组织再生策略方面取得了重要进展，以克服现有治疗方法的局限性2,3,4。
我们之前的系统性综述表明，牙周膜细胞在牙周组织再生方面具有巨大潜力5。传统上，牙周膜细胞在二维（2D）基质（如组织培养用聚苯乙烯，TCPS）上进行培养。然而，在体外培养过程中已观察到牙周膜细胞出现特征性改变6。这一现象可能是由于2D TCPS与体内三维（3D）微环境存在差异所致7。与在2D基质上培养的细胞相比，在3D微环境中生长的细胞更接近体内状态的细胞8。因此，3D细胞培养模型为传统的2D单层细胞培养提供了一种极具前景的替代方案。
传统的三维培养方法是将细胞包埋于三维生物材料中。与包埋在三维生物材料中的细胞相比，细胞球体因由细胞聚集形成且不依赖外来材料生长，能更接近地模拟体内环境9,10,11,12。已有报道指出，细胞球体可通过保留纤连蛋白和层粘连蛋白等细胞外基质（ECM）成分，促进间充质干细胞（MSC）的生物学活性13。为了改进传统的牙周膜细胞（PDL细胞）培养模型，我们近期开发了一种基于壳聚糖薄膜上PDL细胞形成球体的三维培养方法14。球体形成可增强PDL细胞的自我更新能力及成骨分化潜能14。本文提供了基于壳聚糖薄膜的PDL细胞球体培养的详细实验方案。该PDL细胞三维球体培养体系克服了传统组织培养聚苯乙烯表面（TCPS）培养方式的一些局限性，因此可能适用于制备具有更强治疗效能的PDL细胞，用于未来的牙周组织再生。

Formation of Human Periodontal Ligament Cell Spheroids on Chitosan Films

牙周膜（PDL）细胞在牙周组织再生方面具有广阔的应用前景。传统上，PDL细胞在二维（2D）基质（如组织培养用聚苯乙烯，TCPS）上进行培养。然而，在体外培养过程中已观察到PDL细胞出现特征性改变。这一现象可能是由于2D TCPS与体内三维（3D）微环境存在差异所致。与在2D基质上培养的细胞相比，在3D微环境中生长的细胞更接近体内状态的细胞。因此，3D细胞培养模型为传统的2D单层细胞培养提供了一种极具前景的替代方案。为了改进传统的PDL细胞培养模型，我们近期开发了一种基于壳聚糖薄膜上PDL细胞形成球状体的3D细胞培养方法。本文详细介绍了基于壳聚糖薄膜的细胞球状体培养方案。PDL细胞球状体的3D培养系统克服了传统2D单层细胞培养的一些局限性，因此可能适用于制备具有更高治疗效能的PDL细胞，用于未来的牙周组织再生。

采用本实验方案，成功形成了具有活性的牙周膜细胞球体。图1显示，在壳聚糖膜上主要观察到悬浮的细胞或细胞球体，而非贴壁细胞。当接种密度为0.5 × 104 cells/cm2时，在第1天和第3天偶尔可见贴壁的牙周膜细胞，而牙周膜细胞球体极少被观察到。相反，当接种密度为3 × 104 和 6 × 104 cells/cm2时，自第1天起即可观察到不同大小的牙周膜细胞球体。在培养3天后，所有接种密度下均可见牙周膜细胞球体的形成。如图1所示，接种密度为3 × 104 cells/cm2时，牙周膜细胞球体的大小较为均一，因此该密度为最佳接种密度。培养5天后，所有接种密度下均形成了更大的细胞球体，而在第5天几乎未见悬浮的牙周膜细胞。

本研究详细描述了利用壳聚糖薄膜培养牙周膜细胞球体的实验方案。与传统培养体系相比，球体培养体系可显著增强牙周膜细胞的自我更新能力及成骨分化能力。首先，按照文中的说明配制1%（质量/体积）的壳聚糖溶液。向24孔组织培养聚苯乙烯板的每个孔中加入0.5毫升壳聚糖溶液。将该板置于60摄氏度的烘箱中干燥24小时，以形成壳聚糖膜。为中和壳聚糖膜，向24孔板的每个孔中加入0.5毫升0.5摩尔氢氧化钠溶液，并在室温下孵育2小时。然后，用双蒸水将壳聚糖薄膜清洗三次。将涂有壳聚糖的24孔板在70%乙醇中室温下过夜灭菌。用PBS冲洗该孔板三次，然后置于紫外光下过夜。细胞接种前，将增殖培养基、PBS 溶液和胰蛋白酶 EDTA 溶液预热至 37 摄氏度。弃去细胞培养瓶中的上清液，用 10 毫升 PBS 洗涤汇合的牙周膜（PDL）细胞。弃去 PBS，向培养瓶中加入 1 毫升胰蛋白酶 EDTA 溶液。将锥形瓶在 37 摄氏度下孵育 3 分钟，然后加入 3 毫升增殖培养基以终止胰蛋白酶消化。将细胞悬液转移至 15 毫升锥形离心管中，随后在 300 × g 条件下离心 5 分钟。弃去上清液，并将细胞重悬于 500 微升增殖培养基中。使用血细胞计数板对细胞进行计数，并以每平方厘米5,000、10,000、30,000和60,000个细胞的密度接种到培养板中。将PDL细胞置于37°C、5%二氧化碳及湿润空气的培养箱中培养，每周更换培养基两次。通过倒置相差显微镜每日观察细胞形态，持续5天。采用本方案可成功形成具有活性的PDL细胞球体。在接种密度较低的情况下，第1天和第3天极少观察到细胞球体；但在两种较高密度下，自第1天起即可观察到不同大小的细胞球体。最佳接种密度为每平方厘米30,000个细胞，因为该密度可形成均一的球状体。培养五天后，所有接种密度下均观察到较大的球状体。细胞活力检测表明，牙周膜细胞在第1、3、6天时大部分存活，但在第6天时死细胞数量有所增加。本方案的一个局限性在于类球体形成后细胞增殖能力下降，因此需要进一步研究以促进类球体形成后的细胞增殖。
