本文所述所有方法均已获得北京医院伦理委员会批准。手术适应证依据欧洲泌尿外科学会关于非神经源性男性下尿路症状（LUTS）的指南。禁忌证包括怀疑前列腺癌或逼尿肌功能障碍。
1. 手术器械
<ol>
	<li>确保配备波长为980 nm的半导体激光设备，功率可达连续模式（80-100 W）。</li>
	<li>术中采用激光光纤及0.9%氯化钠溶液进行膀胱冲洗。</li>
	<li>使用26 F激光电切镜以获得良好的视野并提高手术效率。</li>
</ol>
2. 手术准备
<ol>
	<li>在手术当天进行皮肤准备。</li>
	<li>所有患者术前应给予静脉抗生素10。 
	注意：应在手术前30分钟给予头孢呋辛钠（1.5 g溶于100 mL 0.9%氯化钠溶液中）。</li>
	<li>麻醉前，让患者平卧于手术台上。 
	注意：全身麻醉对此手术是有效且合适的。麻醉医师应根据患者的一般状况决定麻醉方式。</li>
	<li>以无菌方式将患者置于截石位并铺巾。</li>
</ol>
3. 实验步骤
<ol>
	<li>观察
	<ol>
		<li>使用电切镜（15-30°）直接观察尿道、精阜、膀胱颈、输尿管开口、膀胱黏膜及小梁增生情况。</li>
	</ol>
	</li>
	<li>设计剜除范围（丘比特之箭）
	<ol>
		<li>使用激光在膀胱颈黏膜处做环形切口（深度达腺体层，宽度约3-4 mm）（图1）。</li>
		<li>使用激光在精阜近端的前列腺尿道黏膜处做环形切口（深度达腺体层，宽度约3-4 mm）（图2）。 
		注意：切开前需确认解剖标志，以避免损伤膀胱颈黏膜及尿道括约肌。除止血外，术中激光功率设定为80-100 W。</li>
		<li>在后尿道12点钟位置，将膀胱颈与前列腺尖部的同心圆切口相连。</li>
		<li>使用激光切开左侧叶和右侧叶（图3）。 
		注意：当前尿道12点钟位置预先切开后，保留远端黏膜。在膀胱颈和前列腺尖部形成的两个同心圆称为“丘比特之箭”。</li>
	</ol>
	</li>
	<li>建立通道
	<ol>
		<li>在前列腺尖部5点和7点钟位置寻找外科包膜。</li>
		<li>确认前列腺尖部5点和7点钟位置的外科包膜。</li>
		<li>使用激光连接前列腺尖部5点和7点钟位置的外科包膜（图4）。</li>
		<li>在前列腺尖部6点钟位置，使用激光从前列腺尖部至膀胱颈将中叶与外科包膜分离（图5）。 
		注意：若前列腺体积&#62;80 mL，则在前列腺尖部5点和7点钟位置将中叶与侧叶完全分离。</li>
	</ol>
	</li>
	<li>剜除左、右叶
	<ol>
		<li>使用电切镜逆时针方向从前列腺尖部至膀胱颈在6点和12点钟位置剜除右侧叶（图6）。</li>
		<li>顺时针方向从前列腺尖部至膀胱颈在6点和12点钟位置剜除左侧叶（图7）。</li>
		<li>剜除完成后，将所有腺体组织推入膀胱内。 
		注意：若前列腺体积&#62;80 mL，应按适当顺序依次剜除中叶和侧叶，然后推入膀胱。</li>
	</ol>
	</li>
	<li>止血
	<ol>
		<li>使用较低激光功率（50 W）对术区周围进行止血，保持适当距离（即1-3 mm）。</li>
	</ol>
	</li>
	<li>粉碎前列腺组织
	<ol>
		<li>使用组织粉碎器将剜除的前列腺组织粉碎成尽可能小的碎片，随后从膀胱中取出（图8）。</li>
	</ol>
	</li>
	<li>导尿
	<ol>
		<li>将组织粉碎器从尿道中取出。</li>
		<li>充分使用利多卡因凝胶润滑尿道后，轻柔地将带有30 mL水囊的22 F Foley导尿管经尿道口置入膀胱腔内。</li>
	</ol>
	</li>
	<li>术后护理10

		<li>术后患者在护理单元内平卧约3小时，直至完全从麻醉中清醒。此期间应确保监护仪和医用氧气设备可用。</li>
		<li>患者完全清醒后，将其送回病房。允许患者开始饮水和进食。记录尿量，并注意尿液颜色。</li>

与传统的经尿道前列腺切除术（TURP）相比，激光手术由于患者耐受性更好、术中出血量更少、疗效显著以及术后恢复时间更短，已逐渐得到更广泛的应用4,5。
近年来，发展最快的技术涉及使用不同波长的激光。目前，多种具有不同特性的激光可用于完成前列腺剜除术。自2007年二极管激光被美国FDA批准用于前列腺增生治疗以来，因其出色的切割能力和止血效果，其在良性前列腺增生（BPH）治疗中的应用逐渐增多6。激光波长决定了水和血红蛋白对其吸收的程度。波长为980 nm的二极管激光对水和血红蛋白的综合吸收率最高。其组织穿透深度为0.5 mm，可在较深组织中产生凝固效应，从而具备良好的组织消融和止血能力7,8。
许多研究中心已开始采用二极管激光前列腺剜除术（DiLEP）治疗良性前列腺增生。在临床实践中，传统的DiLEP学习曲线较长，且在尿道括约肌保护方面并无明显优势9。基于上述原因，本中心对传统DiLEP术式进行了改良，以提升二极管激光治疗良性前列腺增生（BPH）患者的临床价值。

Enucleation of the Prostate for the Treatment of Benign Prostatic Hyperplasia Using a 980 nm Diode Laser

在老年男性人群中，由良性前列腺增生（BPH）引起的下尿路症状（LUTS）是一种常见问题。本文介绍一种名为980 nm半导体激光前列腺剜除术（DiLEP）的新技术，用于治疗BPH1。半导体激光可同时被水和血红蛋白吸收，因此在切割和止血方面具有优良性能2。该半导体激光于2007年获得美国食品药品监督管理局（FDA）批准，因其有效的切割和止血效果，已被用于BPH的治疗3。与经尿道前列腺电切术（TURP）、钬激光前列腺剜除术（HoLEP）和光选择性汽化术（PVP）等其他技术相比，DiLEP具有多项优势。在手术过程中，我们利用半导体激光通过烧灼两个环形线和一条沟槽（形似丘比特之箭）来界定大体积前列腺的边界，并将其分为三个叶。与其他手术方式相比，mDiLEP术中并发症更少，学习曲线更短，并能实现更充分的组织切除。

由良性前列腺增生引起的下尿路症状在老年男性中较为常见。我们的新技术——980半导体激光剜除术，可用于治疗该疾病。半导体激光可同时被水和血红蛋白吸收。与其他手术方法相比，二极管激光手术的术中并发症更少，学习曲线更短，且能实现更强的T细胞应答。在开始手术前，需确认已备妥波长为980纳米、连续输出功率为80至100瓦的二极管激光器。手术开始前30分钟，应为患者静脉注射抗生素，并让患者仰卧于手术台上。患者体位固定后，使用电切镜观察尿道、精阜、膀胱颈、输尿管开口、膀胱黏膜以及小梁化增生情况。使用激光在膀胱颈黏膜做3至4毫米宽的环形切口，并在前列腺部尿道黏膜精阜近端做3至4毫米的环形切口。然后在后尿道12点位置连接膀胱颈与前列腺尖部的同心圆，切开膀胱左右两叶。然后，为了建立通道，定位前列腺尖部5点和7点位置的外科包膜，以及5点和7点位置的外科包膜。接着用激光连接前列腺尖部5点和7点位置的外科包膜。在前列腺尖部6点位置，从前列腺尖部至膀胱颈分离中叶与外科包膜。进行叶状剜除术时，使用切除镜从前列腺尖部至膀胱颈，在6点和12点位置以逆时针方向剜除右侧叶。随后在6点和12点位置从前列腺尖部至膀胱颈以顺时针方向剜除左侧叶。然后将所有腺体组织推入膀胱内。为控制手术区域内的出血点，将50瓦的激光器置于出血部位1至3毫米处，以实现止血。为切除已剥离的前列腺组织，使用组织粉碎器将组织尽可能切碎，并从膀胱中移除组织。当所有已剥离的组织均被清除后，将组织粉碎器从尿道中取出，然后轻柔地将一支22法式、经利多卡因凝胶润滑的弗利导尿管经尿道口插入膀胱腔内。然后用30毫升水充盈球囊。在本项临床研究中，所有40例接受半导体激光剜除术的良性前列腺增生患者均成功完成了手术，几乎所有患者均在术后五天内拔除导尿管。所有患者分别在术后第1、第3和第12个月返回医院进行随访检查。采用国际前列腺症状评分和生活质量评估工具对良性前列腺增生的症状进行筛查、快速诊断、症状监测及治疗方案建议。与基线值相比，术后1个月、3个月和12个月时，国际前列腺症状评分和生活质量评分均显著降低。平均最大尿流率（Qmax）增加了近三倍，平均残余尿量减少了近四倍。与术前数值相比，前列腺体积也显著减小。根据改良的Clavien-Dindo分级系统，所有患者在术中均未发生并发症，仅有四例报告了术后一级并发症。关键在于必须在正确的位置并以正确的方式建立通道，以降低对尿道括约肌和膀胱颈部造成损伤的风险。此外，也可使用其他类型的激光，但其性能不如二极管激光精确。二极管激光为良性前列腺增生的治疗提供了一种新工具，并促使泌尿科医生在改善手术效果时考虑不同激光的物理特性。
