本研究完全由美国国立卫生研究院（NIH）护理研究所院内研究部资助，马里兰州贝塞斯达。

本研究（NCT00852111）已获得美国国立卫生研究院（NIH）机构审查委员会（IRB）的批准。纳入本研究的受试者年龄须在18岁或以上，诊断为非转移性前列腺癌，无论是否曾接受过前列腺切除术，并计划在NIH临床中心放射肿瘤学门诊接受外照射放射治疗。若潜在受试者患有可导致显著疲劳的进行性疾病、过去五年内有精神疾病史、存在未纠正的甲状腺功能减退或贫血，或患有第二种恶性肿瘤，则被排除。使用镇静剂、类固醇或非甾体抗炎药物的个体也被排除。所有受试者均在NIH Magnuson临床研究中心招募。在参与研究前，均已签署书面知情同意书。
1. 握力准备与测试体位
<ol>
	<li>在安静的房间内，放置一把带扶手的椅子。</li>
	<li>打开手持式测力计。
	<ol>
		<li>软件将提示对测力计进行校准。校准时应确保设备平稳放置于水平表面上。</li>
	</ol>
	</li>
	<li>让受试者坐于椅子上，保持躯干直立，双脚完全接触地面，臀部尽量靠紧椅背。
	<ol>
		<li>确保受试者的髋关节和膝关节角度接近90°，肩关节处于中立位的外展/内收状态且无旋转，肘关节屈曲90°，腕部无支撑，符合美国手疗协会（American Society of Hand Therapists）手册推荐的体位17。</li>
	</ol>
	</li>
	<li>完成测力计校准后，指导受试者握住测力计，使背侧中节指骨朝向前方。
	<ol>
		<li>根据受试者的手部尺寸调整握持位置，并记录该位置7。</li>
		<li>在后续所有测试中保持相同的握力测试位置。</li>
		<li>每次测试前，提供标准化的指导语，并要求受试者进行一次模拟尝试，以确认其理解操作指令。</li>
		<li>告知受试者轻微不适属于正常现象，但如果出现异常剧烈的疲劳或疼痛，可随时终止测试。</li>
		<li>若受试者报告严重不适或出现意外情况，应立即停止测试。</li>
		<li>每次测试之间应保证2分钟的休息时间，以使肌肉充分恢复18。</li>
	</ol>

</ol>
2. 最大自主等长收缩（MVIC）测试
<ol>
	<li>向受试者提供标准化指导语。例如：“在测试中，您将首先用非优势手尽可能用力握压5秒。每只手将重复此测试三次。每次测试前，我会倒数3、2、1……开始。听到‘开始’时，请立即尽可能用力握压测试装置。”</li>
	<li>在“开始”口令时，点击 GO 按钮启动程序。</li>
	<li>重复最大自主等长收缩力（MVIC）测试，共进行三次，每次之间休息30秒。</li>
	<li>每只手三次测试中最大力量的平均值即为MVIC19。</li>
</ol>
3. 最大力静态疲劳试验
<ol>
	<li>指导受试者在静态疲劳测试期间尽全力达到最大收缩。</li>
	<li>当听到“开始”时，点击GO按钮启动程序。使用标准化的鼓励语句（如反复说用力挤压），直至测试结束。</li>
	<li>持续进行静态疲劳测试35秒，以便留出最多5秒的时间以达到Fmax（最大握力）。</li>
	<li>静态疲劳指数（SFI）12,20,21
	<ol>
		<li>使用以下公式计算SFI：

		<li>将SFI版本2定义为最后5秒内的最大力（Fmax 25-30s）与最初5秒内的最大力（Fmax 0-5s）的比值，使用以下公式计算：

4. 次最大力静态疲劳试验
<ol>
	<li>在屏幕的透明覆盖层上画一条水平线，标出受试者非优势手最大自主收缩力（MVIC）50%的数值。</li>
	<li>在覆盖层上用另一种颜色画第二条线，标出目标值下降10%的位置。</li>
	<li>确保受试者能够清晰看到屏幕及50% MVIC的标线。</li>
	<li>指导受试者尽可能长时间地维持50% MVIC的目标值。</li>
	<li>进行倒数，在“开始”时点击GO按钮启动程序。</li>
	<li>当力量下降超过目标值的10%，且持续时间超过5秒（以透明覆盖层上的第二条线为指示）时，停止测试。</li>
	<li>计算完成的总功7，即在维持目标力量（T50% MVIC）期间，力-时间曲线下方的面积： 
	总功 = T50% MVIC期间的曲线下面积（AUC） 
	 注意：耐力可测量为完成任务所需的时间22。总功数值越高，表明疲劳程度越低。</li>
</ol>
5. 动态疲劳试验
<ol>
	<li>指导受试者每秒进行一次最大力度握紧，持续 30 s。使用节拍器提供节奏引导20。</li>
	<li>启动设定为每秒发出一次提示音的节拍器。</li>
	<li>开始倒计时。当提示“开始”时，点击 GO 按钮启动测试。确保倒计时速度与节拍器节奏一致。</li>
	<li>在测试进行到一半以及剩余 5 s 时，及时告知受试者。</li>
	<li>30 s 结束后停止测试。</li>
	<li>动态疲劳指数

		<li>使用最后 5 s 的最大力量（Fmax）和前 5 s 的最大力量（Fmax）计算动态疲劳指数20。

癌症相关性疲劳（CRF）是一种常见且致残的症状，高达80%的癌症患者报告存在此症状1。美国国家综合癌症网络（NCCN）将CRF定义为一种持续存在的躯体、情绪和认知方面的耗竭感1。CRF的主要特征是其严重程度与近期活动量不成比例，且休息后无法缓解1。因此，CRF严重影响患者参与日常活动的能力及其与健康相关的生活质量1。
目前对癌因性疲乏（CRF）的评估主要依赖于自评问卷2。因此，通过自评方式测量的症状严重程度容易受到回忆偏差和报告偏差的影响，并可能因所使用的特定问卷及其截断评分标准而产生差异3。作为一种多维构念，CRF的生理维度已被证明与日常活动变化及白天需要小睡的需求相关4，而CRF对生理功能的影响则研究较少。迄今为止，CRF仍是一种诊断不足和治疗不足的症状，其潜在机制和治疗方案均尚未明确1。为了更好地理解这一致残性病症，迫切需要对CRF及其各个维度进行客观且定量的测量。
身体疲劳是指在持续收缩活动中无法维持所需力量的状态5。由于无法完成日常任务（例如提购物袋、举起并持握物体），导致日常功能受损，严重影响健康相关的生活质量，尤其在老年人群中更为显著，并可能增加未来受伤的风险6,7。目前已开发出多种工具用于量化身体功能障碍，包括身体机能测试（如6分钟步行测试（6MWT）和坐-立测试（STS））以及可穿戴式身体活动监测设备（如活动记录仪和健身追踪器）8,9,10。6MWT和STS等身体机能测试操作简便，无需特殊设备10。然而，此类测试的可靠性和成功实施依赖于临床医生的培训，以及诸如30米走廊等后勤条件10。可穿戴活动监测设备可实现自动数据采集和长期症状监测11。然而，这些设备通常需要连续佩戴多日，患者依从性可能存在问题11。此外，使用活动监测设备采集的大量数据在处理上具有挑战性，难以提取具有临床意义的信息11。
手持式测力计，或称配备计算机辅助数据采集系统的仪器化握力装置，是一种用于测量握力的便携式设备。手持式测力法已被用于检测通常涉及运动系统（包括运动神经元和肌肉问题）的疾病状态下的运动疲劳和功能损害12。近期研究显示，患者自述的主观慢性疲劳（CRF）评分与通过握力静态疲劳测试所测得的运动疲劳之间存在关联13。由于握力疲劳测试具有良好的可靠性和时间效率，通常仅需几分钟即可完成，因此特别适用于临床应用14,15。此外，握力疲劳测试可预先编程，确保数据的可重复性7。在标准化操作规程下，实施握力测试对测试人员的培训要求较低，易于在临床环境中推广。将自述疲劳问卷与握力疲劳测试结合使用，可为临床医生提供额外工具，用于癌症患者疲劳症状的筛查、监测和管理。
缺乏标准化的共识方法限制了握力疲劳测试在临床中的应用16。在本研究中，我们提出了三种不同的方法，利用手持式测力计客观地量化运动疲劳。每种方法都应在不同的癌症人群中进行验证，以确保其能够准确区分疲劳与非疲劳受试者。我们还介绍了针对每种握力疲劳测试计算疲劳指数的方法。本研究的目标是提供一套全面的工具包，以补充基于自我报告的问卷，并实现对癌症相关疲劳（CRF）身体功能表现的准确、客观和标准化测量。

Measuring the Motor Aspect of Cancer-Related Fatigue using a Handheld Dynamometer
