1. 心脏摘取与灌注
注意：作为移植物供体，使用7-22周龄的雌性或雄性大鼠。
<ol>
	<li>通过异氟烷吸入麻醉供体大鼠（诱导浓度5%，维持浓度3%，以氧气作为载体）2 以1 L/min的流速通入气体。术中及术后镇痛按每公斤体重5 mg剂量皮下注射卡洛芬，并检查趾夹回缩反射是否消失。</li>
	<li>涂抹眼用润滑剂，并使用机械式剪毛器去除腹部和胸部的毛发。</li>
	<li>将供体置于仰卧位，使用弹性绷带将其四肢固定在手术台基座上，并用70%乙醇或其他足够有效的替代品对皮肤进行消毒。</li>
	<li>沿纵行方向切开皮肤，在局部麻醉（例如利多卡因0.2%）后，使用剪刀行正中剖腹术。</li>
	<li>插入拉钩，将供体左侧的肠管游离，并用无菌棉签暴露下腔静脉。</li>
	<li>为抗凝，将500 I.U.肝素溶于1 mL冰冻等渗生理盐水中，通过穿刺下腔静脉静脉注射。拔出针头后，用棉签轻压穿刺部位以止血。图3A).</li>
	<li>切开膈肌，并对供体双侧进行侧胸廓切开术。</li>
	<li>将活动的胸壁腹侧固定在手术台上。</li>
	<li>使用两把显微镊子通过钝性分离去除心包膜和迷走神经。</li>
	<li>切断腹部血管以放空供体血液并减轻心脏负荷。</li>
	<li>将探针剪的钝端插入横窦心包，利用湿纱布对心脏施加轻柔的尾向牵引，在尽可能远的位置分离升主动脉与肺动脉图3B).</li>
	<li>在上腔静脉、下腔静脉和肺静脉周围放置一根5-0结扎线，并尽可能向背侧收紧图3C).</li>
	<li>结扎线背侧切断组织并取出心脏（图3D).</li>
	<li>通过升主动脉和肺动脉，使用静脉导管的18 G套管对离体心脏灌注30 mL含1000 I.U.肝素的冰冻等渗盐水溶液，随后将心脏置于装有盐水溶液并置于冰上的15 mL离心管中（图3E-F).</li>
</ol>
2. 心脏移植
注意：受体为 10-14 周龄的雌性或雄性大鼠。供体与受体的体重相近。
<ol>
	<li>使用异氟烷吸入法对受体大鼠进行麻醉（诱导浓度为5%，维持浓度为1.5–2%，以O₂为载体）2 以1 L/min的流速注入。术中镇痛按每千克体重皮下注射5 mg卡洛芬。&#160;并检查趾部捏压回缩反射是否消失。</li>
	<li>涂抹眼用润滑剂，去除腹部毛发，固定四肢，并参照供体准备方法对皮肤进行消毒。为获得最佳术后效果，应在加热垫上进行手术，以防止术中低体温。</li>
	<li>在皮肤做纵行切口后，于腹膜筋膜处应用局部麻醉剂（如利多卡因，0.2%）。通过正中剖腹术打开腹腔，并置入牵开器。</li>
	<li>将肠管游离至受体左上方，并置于温热的湿敷料中。</li>
	<li>在使用手术显微镜（或放大眼镜）以5-7倍放大倍率分别游离十二指肠和近端空肠后，用棉签钝性分离暴露腹主动脉和下腔静脉。切勿分离腹腔血管。</li>
	<li>使用两个显微持针器抬起腹腔血管，避免损伤腰静脉，并放置Cooley血管钳（图4A).</li>
	<li>用27 G弯头针以30-45°穿刺腹部血管图4B).</li>
	<li>使用Potts剪刀扩大穿刺部位，形成一个与供体血管管腔大小相匹配的纵向切口图4C-D）并用生理盐水灌注受体血管，以清除血凝块并预防术后血栓形成。</li>
	<li>将移植物放入受体位置，并使用两个间断单针缝合（8-0 单股不可吸收缝线）将供体升主动脉固定于受体腹主动脉，缝合位置为纵行切口的头侧和尾侧角图4E).</li>
	<li>将供体的升主动脉与受体的腹主动脉使用8-0单股缝线分两步连续缝合吻合：首先，将移植物置于受体血管右侧，完成吻合的第一半部分（图4E随后，将移植物置于受体血管的左侧，并完成吻合的第二部分（图4F).</li>
	<li>将供体肺动脉以类似于主动脉吻合的方式固定于下腔静脉（使用8-0单股不可吸收缝线）。从血管腔内侧缝合静脉吻合口的前半部分。图4G-H).</li>
	<li>在收紧结之前，直接用生理盐水冲洗吻合口，以防止外周栓塞。</li>
	<li>在两处吻合口周围放置止血纱布，小心松开Cooley血管钳，以开始移植物的再灌注。若吻合口沿线出现出血，可用无菌棉签轻轻压迫止血。 
	注意：移植组织应在大约 60 秒后开始跳动。</li>
	<li>以蜿蜒曲折的方式将肠管复位。确保肠系膜根部无扭转，以防止肠坏死或机械性梗阻。</li>
	<li>分别使用连续的3-0聚丝线缝合腹肌/筋膜和皮肤。</li>
</ol>
3. 术后护理
<ol>
	<li>术后镇痛处理：在术后第一天（POD），按每公斤体重5 mg的剂量，对受体动物进行一次额外的皮下注射卡洛芬（Carprofen）。此外，从术后第一天起至术后第三天，将1 g的美他力酮（Metamizol）加入500 mL饮用水中供其饮用。</li>
	<li>从术后第三天开始，通过每日腹部触诊监测心脏移植物功能。 
	注意：若在术后第三天之前发生移植物失功，应考虑为手术相关失败而非免疫性排斥。但此判断仍取决于所选用的小鼠品系组合及相应的免疫学模型（例如，经预先免疫后发生的超急性排斥反应）。</li>
	<li>在移植物发生排斥后，采集引流腹膜后淋巴结（位于吻合口头侧）、脾脏、血液、胸腺及移植物等组织，用于后续通过流式细胞术或免疫组织化学方法进行免疫学分析。</li>
</ol>
4. 心脏的酶消化及心脏细胞向耳部皮下注射
<ol>
	<li>进行心脏摘除和灌注，操作方法参照异位心脏移植（见步骤1）。</li>
	<li>使用无菌手术刀或无菌剪刀将心脏切成3 mm × 3 mm的小块，并在含有0.5 mg/mL胶原酶的培养基中于37 °C孵育30分钟。 
	注意：必须使用含有青霉素、链霉素和谷氨酰胺但不含胎牛血清（FCS）的培养基，因为FCS会抑制胶原酶的消化作用。</li>
	<li>将消化后的组织加入大孔径筛网中，弃去培养基，并充分剪碎，以获得含有活性心肌细胞、大部分已死亡的单个心细胞及残留血细胞的悬液。用无菌等渗盐溶液将细胞悬液洗涤两次。 
	注意：离心条件：10分钟，200 × g，20 °C</li>
	<li>使用40 µm细胞筛网过滤悬液，并用5–10 mL等渗盐溶液冲洗细胞筛网，收集活性细胞团块。</li>
	<li>离心后，将心肌细胞重悬于等渗盐溶液中，浓度调整为5×105 个细胞/mL，并将细胞悬液吸入1 mL注射器中。</li>
	<li>对受体动物实施麻醉，操作方法参照异位心脏移植中受体麻醉方案（见步骤2）。</li>
	<li>将受体动物置于侧卧位，并用双面胶带固定耳朵，同时用手指固定耳部（图5A）。</li>
	<li>通过27 G套管，在受体耳部毛细血管可见区域附近皮下（s.c.）注射20 µL心肌细胞悬液（含1×104个细胞）（图5B）。</li>
	<li>经过设定的观察期（根据所选品系组合和排斥反应强度而定），取出引流颈部淋巴结，并进行进一步分析，如流式细胞术或共培养实验（图5C）。 
	注意：此外，可对耳廓进行组织学分析，以评估细胞浸润情况。</li>

异位心脏移植是一种常用于研究移植耐受性、急性和慢性同种移植物排斥反应、缺血-再灌注损伤、机械灌注或心脏重构等多种问题的实验模型。与其他器官相比，该模型具有多项优势：移植物功能可通过触诊进行无创监测，且移植物失功不会导致受体生命功能受损，而肾脏或肝脏等器官移植则可能直接影响受体生存。
1964年，Abbott 等人首次描述了大鼠异位腹腔心脏移植模型1。随后在1966年，Tomita 等人提出了端侧吻合法进行血管吻合的技术2。Ono 和 Lindsey 于1969年报道了目前常用模型的基础工作3。在过去的几十年中，已有多种改良方法被发表，用于构建不同类型的心脏移植物，包括无负荷、部分负荷或完全负荷的左心室移植物，以及联合异位心脏-肺移植模型4,5,6。在免疫学分析中，最常采用的是非容量负荷的心脏移植物移植模型。在此模型中，血流逆向进入供体升主动脉，随后流入冠状动脉。静脉回流则经冠状窦进入右心房和右心室（图1A-B）。因此，左心室基本不参与血流循环，仅有极少量来自Thebesian静脉的血液。该模型也适用于研究左心室辅助装置治疗期间的病理生理机制7。
异位心脏移植已在多种物种中实施，包括小鼠、兔子、猪，并且甚至被用作人类的单心室或双心室辅助装置8,9,10,11。大鼠仍然是移植模型中常用的实验动物，尤其是因为过去已明确不同大鼠品系组合的移植物存活时间，且可获得大量免疫学试剂12,13。与小鼠不同，大鼠体型较大，使手术操作以及获取淋巴组织进行免疫学分析更为可行12。此外，近年来商业化的克隆技术在大鼠中的应用，很可能将重新激发人们对实验性大鼠模型的兴趣14。
通常，异位心脏移植可以通过颈部分支或腹部吻合连接到受体血管。然而，一些研究表明，股动脉吻合由于更便于手动触诊或经股动脉超声心动图监测，有利于改善监测效果，从而能够更精确地检测移植物功能衰竭15,16。
已有研究表明，两种吻合技术在手术时间、并发症发生率、治疗效果及移植物存活时间方面均无显著差异17。显然，颈段吻合术的一个优势在于可获得数量充足的引流淋巴结；然而，该术式需要更长的培训周期。相比之下，腹段吻合术操作更为简便，且在免疫学研究中具有同等价值，尤其是与新型同种异体心肌细胞耳部注射技术及后续颈淋巴结切除术联合应用时。两种模型的结合可为干预后的免疫学分析提供广泛的研究空间。
以下实验方案涉及两名外科医生协作操作，以缩短缺血时间。然而，所有实验步骤也可由单人完成。心脏摘取与移植所需器械和材料的准备如图2A-B所示。

An Immunological Model for Heterotopic Heart and Cardiac Muscle Cell Transplantation in Rats

大鼠异位心脏移植模型已被广泛用于多种免疫学研究超过50年。自1964年首次报道以来，已有多种技术改良方法被提出。在进行了30年的大鼠异位心脏移植手术实践后，我们建立了一种简化的外科手术方法，该方法易于教学和操作，无需额外的外科培训或背景知识。
在解剖升主动脉和肺动脉，并结扎上、下腔静脉及肺静脉后，获取供体心脏，随后用添加肝素的冰冻生理盐水溶液进行灌注。夹闭并切开受体腹腔血管后，使用连续缝合线将供体的升主动脉和肺动脉分别与受体的腹主动脉和下腔静脉进行吻合。
根据不同的供体-受体组合，该模型可用于分析同种移植物的急性或慢性排斥反应。通过耳部注射活体心肌细胞并随后分析引流的颈部淋巴组织，该模型的免疫学意义得到了进一步提升。

移植模型是免疫学研究中重要的工具，通常需要外科技术。本文介绍了一种在大鼠中进行异位心脏及心肌细胞移植的可行方法。通过突出并简化本模型的所有关键步骤，无外科背景的研究人员也能在合理的时间内掌握该技术。两种移植模型均可为研究耐受与排斥机制提供重要见解，并可应用于多种物种。我们在向无外科手术经验的人员教授该模型方面拥有丰富经验。在掌握关键步骤后，我们建议保持耐心，并使用死亡动物进行充分练习。对于供体心脏摘取，在确认麻醉后的7至22周龄大鼠供体动物对足底反射无反应后，涂抹眼部润滑剂，并使用电动剪毛器去除腹部和胸部的毛发。将大鼠仰卧位固定于加热底座上，并用弹性绑带将四肢固定在手术台基部。用70%乙醇对暴露的皮肤进行消毒，然后行正中腹切口。将拉钩插入切口，使用无菌棉签将肠管向供体左侧移动，以暴露下腔静脉。接着，穿刺下腔静脉，注入溶解于1毫升冰冻等渗盐水中的500国际单位肝素，进行静脉注射。拔针后，用棉签轻轻按压穿刺部位，止住出血。切开膈肌，对供体双侧进行侧胸廓切开术，并将胸壁固定。使用两个显微持针器钝性剥离以去除心包膜和迷走神经。为放血，横断腹部血管，并将探针剪刀的钝端插入横窦。在轻柔向尾侧牵引心脏的条件下，使用压钳尽可能远端分离升主动脉和肺动脉。用一根5-0缝线环绕上腔静脉、下腔静脉及肺静脉，并逐步尽可能向背侧收紧缝线。切断缝线背侧的组织，取出心脏。为了灌注离体心脏，使用静脉导管的18号套管针，将30毫升冰冻等渗盐水（含1000国际单位肝素）经升主动脉和肺动脉冲洗心脏，随后将心脏置于盛有盐水溶液的15毫升离心管中，并置于冰上。按照前述方法准备10至14周龄的受体大鼠后，将受体动物的肠管移至左上方，并放置在温热的湿敷料上。调节手术显微镜或性能足够的替代设备。分别使用5至7倍全视野放大倍率游离十二指肠和近端空肠后，用棉签和钝性分离法暴露腹主动脉和下腔静脉。使用两个显微持针器提起腹腔血管，避免损伤腰静脉，将Cooley血管夹置于血管上。采用30至45度弧形27号套管针穿刺腹腔血管。使用Potts剪刀将穿刺部位扩大为与供体血管管腔大小相匹配的纵行切口。为清除血栓并预防术后血栓形成，用生理盐水灌注血管，并将移植物置入手术部位。使用两针独立的8-0单股不可吸收缝线，将供体升主动脉固定于受体腹主动脉纵行切口的头侧和尾侧角处。将供体升主动脉与受体腹主动脉吻合时，在进行吻合口前半部分缝合时，将移植物置于受体血管右侧，使用连续的8-0单股缝线进行缝合。然后将移植物置于受体血管左侧，完成吻合口的后半部分。以类似方式将供体肺动脉固定于下腔静脉，分别在头侧和尾侧角各打两个角结。在用缝线于供体血管每个角固定后，从血管外向内进针缝合，以完成吻合口前半部分的腔内吻合。在打结之前，再从血管内向外进针缝合一针。为防止完成吻合口后半部分后发生周围栓塞，在收紧后半部分吻合口的缝线结之前，需用生理盐水冲洗吻合口。在两处吻合口周围放置止血纱布，小心松开Cooley血管钳以恢复移植物的灌注。使用轻柔按压和无菌棉签处理任何出血。约60秒后，心脏应开始搏动。仔细将肠管复位，并分别使用连续的3-0多股缝线缝合腹筋膜和皮肤。在适当的实验时间点，取出供体大鼠心脏，按所示方法操作，使用无菌手术刀或无菌剪刀将心脏切成3毫米×3毫米的组织碎片。在37°C条件下，用20毫升含有0.5毫克/毫升胶原酶的培养基对组织碎片消化30分钟。消化结束后，将组织加入大孔径筛网中，同时去除培养基。加入5至10毫升等渗盐溶液，将组织匀浆通过筛网过滤。随后将细胞悬液离心两次，再通过40微米细胞滤筛过滤。用 5 至 10 毫升等渗盐水溶液冲洗滤网，以收集残留的细胞。离心后，将分离的大鼠心肌细胞重悬于盐水溶液中，浓度为每毫升 5 × 10⁵ 个细胞。用 1 毫升注射器吸取细胞悬液，将麻醉后的受体动物置于侧卧位，使目标耳朝上。用手指和双面胶固定耳朵，使用27号套管针将20微升心肌细胞溶液皮下注射至受体耳部毛细血管可见区域附近。在适当的实验观察期后，取出引流颈部淋巴结，并进行相应的下游分析。同系移植体存活长达100天，未出现移植物失效的迹象。特定的供体-受体组合可能导致异位心脏移植排斥反应的速度不同。排斥心脏的冷冻切片显示免疫效应细胞浸润增加，而同系移植的心脏则基本无免疫细胞浸润。在异基因移植受体中，经口服心肌细胞注射后进行颈部淋巴结切除及引流淋巴结细胞再刺激实验，可揭示针对异位心脏组织的不同品系特异性免疫反应，提示需要进一步开展细胞因子谱分析等免疫学研究。插入和取出针头时，请小心操作血管。不要过度关注缝线，以免造成狭窄。每次吻合时不要缝合过多针数。
