1. 动物
<ol>
	<li>从商业渠道获取妊娠的 Sprague Dawley 大鼠。</li>
	<li>将动物饲养在经批准的兽医设施中，保持 14 小时光照/10 小时黑暗的光暗循环，相对湿度为 45-60%。</li>
</ol>
2. 测试化合物的制备
<ol>
	<li>使用伊文思蓝染料作为测试化合物，以评估经气管滴注操作的有效性。</li>
	<li>用磷酸盐缓冲液（pH 7.2）配制0.25%（w/v）的染料溶液，并使用0.45 µm注射器滤器过滤除菌。</li>
</ol>
3. 麻醉给药
<ol>
	<li>使用改良的适用于新生小鼠的气体麻醉系统，以3%异氟烷（溶于100%氧气）对幼鼠进行麻醉。</li>
	<li>检查尾部和足底反射是否消失以及呼吸是否变浅，以确认麻醉深度适合进行操作。</li>
</ol>
4. 气管内灌注（ITI）
<ol>
	<li>使用出生后第5天（PN 5）的大鼠幼崽进行ITI实验。PN 5大鼠幼崽的平均体重为12克。</li>
	<li>将麻醉后的幼鼠仰卧位固定在倾斜的平板平台上，使用实验室标签胶带进行固定，固定角度约为45°。</li>
	<li>打开新生儿的口腔，用钝头镊子轻轻将舌头拉向一侧。</li>
	<li>使用直径为2 mm的微型耳镜窥耳器连接耳镜，轻柔地固定舌头，以清晰观察喉部。</li>
	<li>使用喉部照明系统，即手术耳镜，以及放大镜，以清晰观察声带图1).</li>
	<li>位置&#160;将动物以45°角置于倾斜平面上。使用小鼠笼具的金属丝网盖图2). 
	注意：将动物置于45°角度有助于更好地观察气管开口，同时避免会厌的干扰。</li>
	<li>取一支用于上样Western blot凝胶的长角度移液器吸头，使用手术刀片将其底部切去，使其能够紧密地插入1 cc注射器的前端。</li>
	<li>使用装有长弯移液器吸头的无菌1 mL注射器，将30–50 µL的物质注入肺部。将注射器倒置，先吸取约0.9 cc空气进入连接移液器吸头的1 mL注射器中，随后吸入待注入的染料或物质。这样可在染料注入后，使位于染料后方的空气一并推入气管，如图所示。 图3通过观察喉腔（声带）并将连接注射器的移液器尖端插入气管腔内，实现气管内给药。</li>
	<li>使用耳镜的窥耳器固定舌头并暴露声带。窥耳器起到喉镜片的作用。将移液器吸头弯曲成30°角，以便通过锥形窥耳器将试剂顺利导入气管开口。</li>
	<li>将移液器吸头插入气管开口，深入至超过声带约2毫米处。推动注射器活塞，通过手术耳镜的窥耳器将染料或药物注入，如图所示。 图3给药后立即将空气引入肺部，可防止该物质回流至喉腔。</li>
	<li>给幼崽注射染料或生理盐水后，将其置于集成循环液体加热垫（38°C）上，直至呼吸运动恢复正常。待麻醉完全恢复后，将幼崽与母鼠合笼。</li>
</ol>
5. ITI 递送的表征
<ol>
	<li>ITI 后，在给药后的适当时间点，通过给予过量麻醉剂（氯胺酮 100 mg/kg 和赛拉嗪 10 mg/kg）/硫喷妥钠，随后放血处死幼鼠。为收集肺组织以验证疗效，实验过程中需实施安乐死。</li>
	<li>将已处死的幼鼠固定于解剖板上，用 70% 乙醇擦拭胸部和腹部。</li>
	<li>为评估染料在肺部的分布情况，采用无菌技术取出动物的肺组织，并根据成像需要适当展示肺组织（图 4A、B）。</li>

气管内灌注是一种优异的方法，在呼吸系统疾病干预及疾病模型构建方面相较于现有方法具有多项优势。该方法操作快速，且随着经验积累，每只动物平均可在2-3分钟内完成。成功插管的关键因素包括动物的适当麻醉、正确体位（尤其是头部位置），以及开口器在口咽部插入深度或尺寸的准确性。充分的麻醉可为操作者（特别是初学者）提供充足的操作时间。将动物置于45°角体位，有助于清晰观察声带。开口器插入至合适深度，可在整个操作过程中有效牵开舌头，从而进一步确保声带的良好视野。两人团队可高效协作完成该操作：一人负责麻醉和动物固定，另一人实施灌注。ITI技术中最具挑战性的环节是准确将导管插入气管。插管成功可通过插管后注入染料来确认。确认插管初始步骤的准确性至关重要，因为导管极易误入食管，导致给药物质进入胃部而非肺部。
唯一需要特别注意的部分是误插管可能引起的创伤。操作时还必须非常轻柔谨慎，以避免穿透气管或声带周围组织。此外，若已发生2至3次插管失败，则不建议继续进行气管内插管2。

早产新生儿肺部发育不成熟，需要多种干预治疗，例如长期通气。这些干预措施使存活下来的新生儿面临发生后续并发症的高风险1。实验动物模型在模拟各种疾病状态、研究疾病的病理生物学机制以及评估治疗干预措施方面具有重要作用。尽管已有从小鼠、大鼠、兔到早产羔羊和猪等多种动物模型可供选择，但小鼠和大鼠是最常用的动物模型。
使用小鼠和大鼠的主要优势在于其相对较短的妊娠期和较低的成本。此外，它们易于获得，可在无病环境中方便地维持，具有遗传同质性，且涉及的伦理问题相对较少2,3。啮齿类动物模型的另一大优势是，出生时新生幼鼠的肺部发育处于终末细支气管期/早期囊泡期，其形态学特征相当于即将发生支气管肺发育不良的24周早产人类新生儿的肺部4。此外，由于其肺部发育在出生后的前4周内迅速完成，因此可以在合理的时间范围内研究出生后的肺部成熟过程4。尽管具有这些优势，小鼠和大鼠幼崽体型较小，给多种干预操作带来困难，这使得大多数研究人员倾向于使用成年动物而非幼崽5。新生期肺组织处于发育阶段，新生儿对刺激因子的反应与成年个体不同。因此，使用新生动物模型来研究人类新生儿疾病状态是恰当的。

A Minimally Invasive Method for Intratracheal Instillation of Drugs in Neonatal Rodents to Treat Lung Disease

将药物直接滴注至新生啮齿类动物气管内，可作为研究局部给药影响的有力工具。该方法具有直接的转化应用价值，因为表面活性物质和药物通常需局部递送至肺部。尽管已有大量文献报道了在成年小鼠和大鼠治疗实验中采用微创经口插管技术，但在新生大鼠幼崽中的应用仍较为缺乏。由于幼崽口咽部及喉区结构微小，导致声门（声带）难以观察，从而影响了经气管给药的成功率。本文介绍一种有效的新生大鼠幼崽经口插管技术，该方法无创且创伤极小，适用于多次重复给药。我们使用配备照明系统和放大镜的手术耳镜来观察新生大鼠的气管开口，随后通过连接1 mL注射器与移液器吸头的方式进行药物滴注。通过伊文思蓝染料给药验证了该递送方法的准确性。该技术易于掌握，可有效实现药物向气管内的递送。此外，该方法还可用于接种病原体或致病因子以模拟动物疾病状态，以及用于多种肺部疾病的细胞治疗策略研究。

伊文思蓝灌注显示染料呈多灶性分布，累及所有肺叶（图4A、B）。如图4所示，本结果证明染料可有效分布至所有肺叶。图像为经气管内灌注染料后立即拍摄。经气管灌注染料的操作成功率达100%，随后染料扩散至双侧所有肺叶。预计染料可在肺小叶内进一步扩散。通过重复给药，我们已确保将染料成功递送至双侧肺叶及所有肺小叶，成功率均达到100%。我们确认无任何染料进入胃部或肺外区域，这证实了该技术将染料完全递送至肺部的高效性。异氟烷麻醉使幼鼠在操作后恢复更快。 
出生第5天的大鼠幼崽可耐受该操作，麻醉后操作时间少于5分钟。部分动物虽出现短暂性呼吸暂停，但在数分钟内即恢复正常的呼吸节律。

本方案表明，可通过直接可视化的方法将研究材料注入五日龄新生大鼠幼崽的气管内。该方法通过直接可视化提高了向气管内给药或递送研究材料的可靠性。由于避免了穿刺注射，该方法减轻了疼痛，更重要的是减少了气管内出血，从而提高了动物存活率。执行本方案需要掌握精细的操作技能，包括正确固定动物体位、熟练使用手术显微镜观察声门，以及准确将物质注入肺部。本实验操作将由本实验室的博士后研究员 Tara Sudhadevi 和研究生 Alison Ha 演示。实验开始前，需确认一只五日龄幼鼠在麻醉后已丧失尾部和足底反射，并使用实验室标签胶带将其固定于倾斜45度的平坦平台上，保持仰卧位。使用钝头镊子轻轻将舌头拉向一侧，并借助直径为2毫米的耳镜窥器，轻柔地固定舌头位置，以便观察喉部。使用合适尺寸窥器的手术耳镜，通过其放大镜头定位声带。进行气管内滴注时，取一支1毫升注射器，安装长角度凝胶上样吸头，先吸入0.9立方厘米空气，随后吸入30至50微升目标物质。将移液器吸头弯曲至30度角，并将装有样品的吸头通过窥镜插入至声带以下约2毫米处。推动注射器活塞，将目标物质及空气通过窥器注入声带区域。待全部物质注入后，将幼鼠置于38°C集成循环液体加热垫上并持续监测，直至呼吸恢复正常，再将其与母鼠放回同一笼中。为了评估物质在肺组织样本中的分布情况，先用70%乙醇擦拭安乐死后的新生大鼠，然后采用无菌技术按照标准操作流程取出肺组织进行肉眼观察。本研究通过肉眼检查经气管内滴注伊文思蓝染料的五日龄新生大鼠所获取的肺组织，评估气管内滴注技术的有效性。结果显示该滴注方法效率接近100%，染料在给药后立即扩散至整个肺部，并分布于肺组织的所有肺叶中。在肺组织以外的任何组织中均未观察到染料的存在，证实了该技术可用于将目标治疗物选择性递送至肺部。将动物最初放置在坚硬平面上的正确角度，对于下一步使用手术显微镜观察声带的成功至关重要。
