2. 背部机械敏感性（BMS）测试
<ol>
	<li>在首次测试前连续5至7天，每天让动物在测试笼中适应30分钟。根据需要重复测试。</li>
	<li>使用 2% 异氟烷对动物进行麻醉31 （参见 材料表).</li>
	<li>在异氟烷麻醉下取俯卧位，使用动物毛发修剪器剃除目标区域（T6至L6椎体水平）背部的毛发（参见 材料表）。对于重复测量，每3天在不进行行为学评估的日子剃除背部毛发，以确保刺激始终直接作用于皮肤。使用永久性标记笔在皮肤上画一个黑点，以确保在不同天重复测试时，纤维丝始终作用于同一区域。</li>

下背痛（LBP）是全球致残的首要原因，具有显著的个人、经济和社会后果1,2,3,4。每年约有37%的人口受到下背痛的影响5。下背痛通常在数周内自行缓解，但在24%–33%的个体中会复发，并在5%–10%的病例中转为慢性2。为了理解下背痛的机制和影响，以及不同治疗干预的效果，已采用多种动物模型来模拟临床状况或下背痛的某些组成部分6。这些小鼠和大鼠模型可归入以下一个或多个类别：（1）椎间盘源性下背痛7,8,，（2）根性下背痛8,9,10,11，（3）小关节骨关节炎12，以及（4）肌肉源性下背痛13,14。由于无法直接测量非人类物种的疼痛，研究人员已开发出多种测试方法，用于量化这些模型中的类似疼痛行为8。这些测试评估由有害刺激（机械力15,16,17、热刺激18,19,20,21,22,23,24,25）诱发的行为或自发产生的行为26,27,28,29。
使用机械刺激的方法包括 Von Frey 试验15,16 和 Randall-Selitto 试验17。使用热刺激的方法包括尾 flick 试验18、热板试验19、Hargreaves 试验20 以及热探针试验21。使用冷刺激的方法包括冷板试验22、丙酮蒸发试验23 和冷足底试验24。用于自发行为的检测方法包括面部疼痛评分量表26、挖掘行为检测27、负重与步态分析28，以及自动化行为分析29。尽管已有大量可用的检测方法，但其中没有一种是专门为背痛模型设计的。
本研究的目的是建立一种简单且易于操作的检测方法，用于评估大鼠背部的机械敏感性。该技术主要基于应用于后肢足底表面的Von Frey试验15,16。Von Frey试验的基本原理是将一系列单丝纤维施加于目标区域，以传递恒定且预先设定的力。当大鼠出现伤害性防御行为时，视为阳性反应。随后可根据引发反应的纤维计算出机械阈值。在本研究中，提供了一种源自Von Frey试验的简单且易于实施的方法，用于测定大鼠背部的机械敏感性。

腰痛是全球致残的首要原因，给个人、经济和社会带来巨大影响。为了开发新的治疗方法，需要借助动物模型从转化医学的角度研究新疗法的作用机制和有效性。目前的研究中使用了多种啮齿类动物腰痛模型。然而令人意外的是，尚无经过验证的标准化行为学测试可用于评估腰痛模型的机械敏感性。这一点至关重要，因为需要确认假定患有腰痛的动物是否对伤害性刺激表现出局部痛觉过敏，并在旨在缓解腰痛的干预过程中监测其敏感性变化。本研究的目的是建立一种简单且易于实施的测试方法，用于评估大鼠背部的机械敏感性。为此方法专门设计了一个测试笼，其尺寸为长×宽×高：50 × 20 × 7 cm，顶部设有不锈钢网。该测试笼可允许对动物背部施加机械刺激。进行测试前，需剃除动物目标区域的背部毛发，并标记测试区域，以便在不同日期重复测试。使用Von Frey纤维丝通过先前描述的上下法（up-down method）测定机械痛阈值，刺激部位为椎旁肌。阳性反应包括：（1）肌肉抽动，（2）弓背（背部伸展），（3）颈部转动，（4）抓挠或舔舐背部，以及（5）试图逃离。该行为学测试（背部机械敏感性（BMS）测试）对于利用啮齿类动物腰痛模型开展机制研究，以及开发预防和管理腰痛的治疗干预手段具有重要应用价值。

该方法已在先前的研究中使用，其中提供了完整的数据和统计结果，用于比较CFA组与对照组大鼠的背部机械敏感性30。来自先前研究中六只大鼠的代表性个体数据（左右侧阈值的平均值）如图3和表1所示。在基线时，各组之间的机械敏感性相似。向腰部肌肉注射CFA后，从注射后第7天至第28天，机械敏感性显著增加（阈值降低）。相比之下，对照（CTL）组大鼠未出现此类变化。如图3所示，由于此类行为学评估的特性，动物个体内部及个体之间均存在一定的变异性。然而，CFA诱导的高敏感性大鼠表现出较低的变异性。根据先前的研究30，每组8只大鼠（共16只，包括8只CFA组和8只CTL组）足以在5个时间点上检测到随时间变化的组间显著效应（η2p = 0.38）。

该方案可用于检测背部的机械敏感性。对于慢性背痛大鼠模型的纵向研究设计尤其有价值，其目的是评估干预措施降低背部对有害刺激敏感性的有效性。该方法操作简便、易于实施，在经过训练的实验人员操作下可获得可重复的结果。该方法主要基于Von Frey试验，后者常用于评估后肢足爪的机械敏感性。与其他行为学试验一样，实验人员应通过练习标准化地施加纤维丝，并确保能够清晰区分机械刺激引起的行为与其他行为。该试验本身操作简便，通常不会存在显著的应用困难。本实验操作将由我实验室的博士生 Nasim Eskandari 演示。为在大鼠中诱导慢性背痛，需向背部肌肉注射完全弗氏佐剂（Complete Freund's Adjuvant，简称 CFA）。使用 27 号针头，将 150 µL 预制的油包水型 CFA 乳剂单侧或双侧注射至椎旁肌内。将针头保持在原位至少三分钟。接下来，为两只动物制作测试笼，该笼由两个相邻的隔室组成，每个动物一个隔室，将两个隔室安装在四根33厘米长的有机玻璃腿上。隔室的侧壁使用透明有机玻璃，隔板使用黑色有机玻璃，以防止动物相互看见。然后使用由直径1毫米的金属丝制成、金属丝间距为8毫米的不锈钢网，制作测试笼的底面和顶面。在首次测试前，连续5至7天，每天让大鼠在测试笼中适应30分钟。在异氟烷麻醉下使大鼠呈俯卧位，使用动物毛发修剪器剃除目标区域约10个椎体水平范围的背部毛发。对于重复测量，每三天剃除一次背部毛发，且不进行行为学评估，以确保刺激始终直接作用于皮肤。使用永久性标记笔在皮肤上做标记，以保证在不同天重复测试时，纤维丝始终作用于同一区域。测试当天，将大鼠放入测试笼中静置15至30分钟，直至大鼠保持安静后再开始测试。然后将Von Frey纤维丝垂直施加于背部，始终从2克纤维丝开始，并采用上下法。当大鼠清醒、四肢站立且静止不动时，将纤维丝双侧施加于距脊柱10毫米的目标区域，每次持续2秒，每15至30秒处理一次。若施加某根纤维丝后未观察到反应，则使用系列中下一根力度更大的纤维丝继续施加。若观察到阳性反应，例如弓背、舔舐、肌肉抽搐、躲避细丝或身体转动，则使用系列中下一个力度较小的细丝进行测试。从首次出现行为变化后开始，继续该程序直至获得四次读数。本研究探讨了CFA处理组与对照组大鼠背部的机械敏感性差异。在基线时，各组之间的机械敏感性相似。在腰肌内注射CFA后，机械敏感性从第7天到第28天显著增加。相比之下，对照组大鼠未表现出任何显著变化。此外，组织学检查证实，注射完全弗氏佐剂（CFA）的肌肉存在慢性炎症改变，并伴有明显的白细胞浸润。作为对照，图中显示了健康大鼠背部肌肉在注射生理盐水后的组织学表现。除了背部肌肉外，还通过标准的Von Frey试验观察到后肢足底的机械性痛觉过敏。确保将细丝以垂直于背部的相同方式施加，并且在施加过程中动物保持不动，这一点非常重要。实验人员还应能够准确判断与现代运动相关的行为反应。
