老年性耳聋是全球最常见的疾病之一，影响着全球超过三分之一年龄在65岁及以上的人群1。其根本原因目前仍在争论中，并且正在被积极研究，可能包括连接内毛细胞（IHCs）与传入听觉神经纤维的特化突触的丧失2。这些带状突触由突触前结构组成，其上锚定着充满神经递质谷氨酸的囊泡，以及突触后α-氨基-3-羟基-5-甲基-4-异恶唑丙酸（AMPA）谷氨酸受体3,4,5。在沙土鼠中，约有20根传入听觉神经纤维与一个内毛细胞相接触6,7,8。位于内毛细胞朝向蜗轴一侧的神经纤维对应较大的突触带状体，而连接在内毛细胞支柱侧的纤维则面对较小的突触带状体（例如，在猫9、沙土鼠7、豚鼠10和小鼠3,11,12,13,14中均如此）。此外，在沙土鼠中，突触前带状体的大小与突触后谷氨酸受体斑块的大小呈正相关7,14。位于内毛细胞蜗轴侧、对应较大带状体的神经纤维直径较小，具有较低的自发放电率和较高的听觉阈值15。有证据表明，与位于内毛细胞支柱侧的高自发放电率、低阈值纤维相比，低自发放电率的纤维对噪声暴露10和耳毒性药物16更为敏感15。
带状突触的丢失是耳蜗神经性老年听力损失中最早的退行性事件，而螺旋神经节细胞及其传入性听觉神经纤维的丢失则滞后于前者17,18。相应的电生理指标包括听觉脑干反应17和复合动作电位的记录8；然而，这些指标并不能反映突触丢失的细微变化，因为低自发放电率的神经纤维对这些测量无明显贡献16。更有前景的电生理指标是基于群体电位的神经指数19和刺激锁时反应时间20。然而，这些指标仅在动物除听觉神经纤维丢失外不存在其他影响残余听觉神经纤维活动的耳蜗病理改变时才可靠8。此外，在沙鼠中通过行为学评估的听觉阈值与突触数量之间未发现相关性21。因此，唯有通过对耳蜗组织的直接检查，才能可靠地定量存活的带状突触数量，进而确定功能性听觉神经纤维的数量。
蒙古沙鼠（Meriones unguiculatus）是研究年龄相关性听力损失的合适动物模型。其寿命较短，具有与人类相似的低频听力，易于饲养，并且表现出与人类年龄相关性听力损失相关的病理特征2,22,23,24。当沙鼠达到36月龄时即被视为进入老年阶段，这接近其平均寿命的终点22。重要的是，已有研究表明，在安静环境中饲养并自然衰老的沙鼠会出现与年龄相关的带状突触丢失8,21。

Immunolabeling and Counting Ribbon Synapses in Young Adult and Aged Gerbil Cochleae

连接内毛细胞与传入听觉神经纤维的带状突触的丢失被认为是与年龄相关的听力损失的原因之一。检测带状突触丢失最常用的方法是免疫标记法，因为它允许从单个耳蜗的多个拓扑位置进行定量取样。然而，所关注的结构深埋于骨性耳蜗内部。沙鼠被用作研究年龄相关性听力损失的动物模型。本文描述了沙鼠耳蜗整体组织的常规固定、免疫标记、共聚焦成像以及带状突触数量和体积定量的实验方案。此外，还强调了从珍贵的老龄个体中获取优质样本所面临的特殊挑战。
将沙鼠实施安乐死后，进行心血管灌注，或小心地从颅骨中分离出鼓室泡。在耳蜗的顶端和底端打开后，直接转移至固定液中。无论采用何种初始方法，耳蜗均需进行后固定，随后进行脱钙处理。接着，使用针对突触前和突触后结构以及毛细胞的一抗对组织进行标记。然后，将耳蜗与荧光标记的二抗共同孵育，这些二抗特异性识别相应的一抗。对于老年沙鼠的耳蜗，还需使用自发荧光淬灭剂处理，以降低老年动物组织中通常较强的背景荧光。
最后，将耳蜗解剖为6至11个节段。通过重建整个耳蜗长度，可在不同个体之间可靠地确定特定的耳蜗位置。依次获取的共聚焦图像堆栈有助于在选定位置可视化毛细胞和突触。对共聚焦图像堆栈进行去卷积处理后，可使用ImageJ手动计数突触，或通过在Matlab中自定义编写的图像分析程序对突触结构进行更全面的定量分析。

耳蜗组织可通过全身灌注固定液后取材，或在动物安乐死后迅速解剖并进行浸没固定。采用后一种方法时，内毛细胞在解剖过程中通常能保持原位；而当灌注失败导致组织固定不充分时，感觉上皮常被破坏。需要注意的是，作者曾遇到过经心脏灌注固定后，耳蜗固定效果不足而脑组织固定仍充分的情况。对于灌注效果较差的组织，可通过在耳蜗顶回和底回钻孔，并将耳蜗浸入4%多聚甲醛中再固定2天来挽救。
根据 Müller25 的方法测定整个耳蜗的长度，以评估特定耳蜗位置的内毛细胞（IHCs）及其突触，这些位置对应于不同的特征频率（表 1）。为此，所需耳蜗位置以从耳蜗基部起算的基底膜长度百分比表示。13 个年轻成年沙鼠耳蜗的中位长度为 11.3 mm（四分位距：11.02–11.52 mm）；24 个老年沙鼠耳蜗的中位长度为 11.5 mm（四分位距：11.24–11.73 mm）。年轻成年与老年耳蜗长度之间无显著差异（Mann-Whitney U 检验：U = -1.62，p = 0.105）；总体中位长...

本方案可实现对各年龄段沙鼠耳蜗中突触完整性的评估。这对于听力问题的基础研究尤为重要，特别是与衰老相关的听力衰退。该方案相对快速，并可从耳蜗的不同位置获得定量数据。此外，该方法通过使用自发荧光淬灭剂，解决了老化组织的一个特定问题。突触的退化是耳蜗功能丧失的最早迹象，对其定量分析是开发针对突触病变的非侵入性诊断工具的重要研究手段。耳蜗解剖是最关键且需要练习的部分，因此最好先使用非实验用的耳蜗进行练习。首先去除鼓泡，用剪刀剪开骨组织，或用镊子敲裂骨组织，再用镊子将其移除，以暴露耳蜗。去除锤骨、砧骨和半规管。小心去除覆盖耳蜗顶点的骨质，并用镊子刮擦耳蜗骨壁，于顶点和基底回处制造小孔，或小心地在这两个位置刺出小孔。立即将鼓泡组织转移至至少50毫升的冷固定液中。为将耳蜗切成两半，将一片长度超过耳蜗盘绕长度的刀片装入刀片架中。然后，将耳蜗置于立体显微镜下的培养皿中。使用刀片或精细剪刀切除多余的组织。用精细镊子固定耳蜗，并沿蜗轴将其切成两半。通过将自发荧光淬灭剂与70%乙醇混合，配制5%的淬灭剂溶液。将耳蜗放入该溶液中，在摇床上室温孵育一分钟，随后用一毫升PBS洗涤三次，每次五分钟。解剖耳蜗时，准备两把精细镊子、超细镊子、Vannas剪刀、弹簧剪、刀片架、剃须刀片，并用PBS填充聚苯乙烯培养皿及其盖子。将剃须刀片折成2至4毫米的切割面。按照组织制备部分所示，将耳蜗切成两半。用镊子小心固定耳蜗的一半，使切缘朝上。使用精细的弹簧剪刀剪除覆盖耳蜗顶的蜗孔上方的骨质。然后，用剪刀剪开蜗轴、听神经和前庭阶，以分离顶端螺旋。在耳蜗骨上方的每一侧各做两个切口；或者，如果无需进一步评估血管纹，则直接穿过中回的血管纹进行切割。使用剃刀刀片分离耳蜗组织块。或者，在柯蒂氏器与血管纹之间进行切割。保持血管纹与螺旋韧带相连，用镊子去除脱钙的骨组织。接着，将耳蜗组织块转移至盛有PBS的培养皿盖中。去除支柱侧和蜗轴侧多余的组织，以便后续整体封片时能尽可能平整地贴附在载玻片上。较大的耳蜗组织块，特别是来自耳蜗基底回的组织块，应切割成两部分。使用超细镊子小心去除盖膜。将耳蜗组织块放置于载玻片上的封片介质液滴中，确保柯蒂氏器朝上。将耳蜗组织块移至矢状面，观察靠近内毛细胞处螺旋缘的内陷结构。该螺旋缘内陷在耳蜗的中段和基底段组织块中更为清晰可见。通过重复上述步骤，将整个耳蜗及整个血管纹完整转移至显微镜载玻片上。用盖玻片封片，并在边缘涂抹黑色指甲油以密封。避光干燥后，于4 °C保存。使用安装了BioVoxxel插件的ImageJ软件打开去卷积后的图像序列副本。通过点击图像，然后选择颜色，再选择拆分通道，将通道拆分。通过点击图像，然后选择颜色，再选择合并通道，以分配不同颜色。通过点击图像，然后进入颜色并点击将栈转换为RGB，将图像转换为RGB栈。通过点击图像、调整，然后亮度/对比度，来调节亮度和对比度。使用自动或滑块功能，确保突触前和突触后结构以及内毛细胞标记与背景清晰可辨。使用文本工具，在图像堆栈中点击目标位置，标记五个内毛细胞。接下来，转到“分析”，然后选择“工具”，并点击“ROI 管理器”。勾选复选框“标签”，以数字标记各点。放大感兴趣的目标内毛细胞。右键单击点工具以选择多点模式。滚动 Z 维度并计数功能型带状突触，这些突触通过位于突触前带状结构与突触后谷氨酸斑块相邻的位置来识别。选择所有感兴趣的结构后，单击添加 ROI 管理器，然后单击任意名称并选择重命名。使用手形工具调整图像，以完整显示下一个内毛细胞。从多点工具切换为单点工具，以避免向先前存储的数据中添加更多荧光点。在下一个内毛细胞内单击目标结构，然后切换回多点工具。在 ROI 管理器中持续添加功能性带状突触，直至所有内毛细胞均被评估完毕。在 ROI 管理器中点击一个数据集，然后点击“测量”（Measure）。等待显示所测数据点的新窗口出现，随后点击“文件”（File）并选择“另存为”（Save As），将此列表保存为电子表格。通过在 ROI 管理器中激活“显示全部”来保存图像。单击“拼合”以将点标签永久添加到图像栈中，然后单击“同意”以保存更改并关闭窗口。来自一只10月龄年轻沙土鼠耳蜗的Z栈投影显示，此处由白色虚线标出的肌球蛋白7A标记的内毛细胞与背景形成鲜明对比。使用抗CtBP2抗体标记突触前带状结构，显示为绿色，而使用抗GluA2抗体标记突触后谷氨酸受体斑块，显示为红色。来自一只38月龄老年沙鼠的耳蜗经自发荧光淬灭剂处理后，显示出清晰的内毛细胞以及明显可见的突触前和突触后结构。为检测免疫标记的稳定性，对同一样本来自年轻沙鼠的耳蜗分别在3个月和29个半月后进行了成像观察。尽管信噪比有所降低，功能性突触仍清晰可辨且可定量。即使某次切割失败，也应将组织的剩余部分安装固定，以便后续更准确地估算耳蜗的全长。若在本方案之前进行电生理或心理物理听力测试，并将所得数据逐一关联分析，则该方案所获数据尤为宝贵。
