1. 鉴别食管插管与气管插管
<ol>
	<li>在实施全身麻醉前，准备一个高频线性超声探头（见材料表），在探头换能器表面涂抹一层超声耦合剂（见材料表）。在触摸屏的换能器菜单中选择线性探头，并在下拉菜单中指定MSK（肌肉骨骼）模式。按下触摸屏左下角的2D按钮，将超声设备置于扫描模式。按照负责麻醉医师的建议实施全身麻醉。</li>
	<li>实施全身麻醉后，将探头横向置于患者颈部前侧正中线上，位于胸骨上切迹稍上方位置（图1A）。确保超声仪器屏幕上探头标记位于左侧（见材料表）。</li>
	<li>识别气管中线，并注意紧邻气管外侧的食管缩窄部分（图1B）。如需进一步确认解剖结构，可向侧方扫查，必要时识别颈总动脉和颈内静脉。</li>
	<li>当气管插管进入气管时，观察是否存在与插管相关的明显气管及周围组织移动。若未观察到气管移动，可轻微旋转气管导管，尝试在超声图像上产生可见移动。
	<ol>
		<li>此外，确认由于气管导管的存在，气管后方高回声结构消失，并出现特征性的子弹形声影（此现象称为&#34;子弹征&#34;，见图2）。如果发生食管插管，则会在气管左侧出现明显的组织移动，并显示两个管腔结构，即出现两个空气/黏膜界面，称为&#34;双轨征&#34;（图3）。 
		注：在实时插管过程中使用该超声技术，可立即判断导管是否进入气管或食管。此外，在急诊气道管理中也可考虑使用该技术，因为在肺血流较差的情况下，呼气末二氧化碳检测可能不可靠17。</li>

2. 为进行环甲膜切开术而识别环甲膜
注意：对于紧急气道管理，如果操作者遇到困难情况，可能需要进行环甲膜切开术 &#34;无法插管，无法氧合&#34; 情况。当怀疑可能出现困难气道时，操作者可选择在麻醉诱导前预先定位环甲膜，以防需要进行环甲膜切开术。
<ol>
	<li>在患者仰卧位、颈部伸展的情况下进行环甲膜（CTM）定位。按步骤1.1所述准备超声探头。由于环甲膜位于颈部较浅处，应根据普通体型患者将探头设置为约1.5–2 cm的深度。 
	注意：有两种方法可利用超声定位环甲膜。</li>
	<li>按以下步骤执行第一种方法以定位环甲膜。
	<ol>
		<li>将线性高频探头置于患者颈部甲状软骨稍尾侧的矢状面（图4A）。在扫描图像中，甲状软骨表现为颅侧表浅的低回声结构，并产生声影（图4B）。</li>
		<li>接下来，寻找位于更尾侧位置的环状软骨，其表现为低回声。通过识别位于两者之间的环甲膜及其下方的气-黏膜界面（表现为贯穿气管全长的高回声线）进行定位。</li>
		<li>为进一步确认，继续向尾侧扫描以识别气管环，其表现为高回声的&#34;串珠样&#34;18结构。 
		注意：识别环甲膜的第二种技术（步骤2.5至步骤2.8）涉及在颈部前侧采用横向扫描方向。该技术有时被称为甲状软骨-气线-环状软骨-气线（TACA）法19。</li>
	</ol>
	</li>
	<li>按以下步骤执行第二种技术以定位环甲膜。
	<ol>
		<li>首先将线性高频探头置于甲状软骨水平的横向平面，此时甲状软骨表现为高回声并产生声影——呈黑色三角形，其尖端最靠近体表（图5）。</li>
		<li>向尾侧方向扫描，直至黑色三角形消失，表明甲状软骨结束、环甲膜开始。此时可见气-黏膜界面表现为一条伴有混响效应的明亮白色线条（图5）。</li>
		<li>继续向尾侧扫描，直至环甲膜结束、环状软骨出现。环状软骨表现为围绕气管的低回声带（图5）。一旦识别出环状软骨，即确定了环甲膜的下界。</li>
		<li>为确保正确识别了解剖结构，逆向重复上述步骤，向头侧方向扫描，再次确认环甲膜和甲状软骨的位置。确认这些解剖标志后，在患者皮肤上标记环甲膜位置。完成标记后，按计划实施麻醉诱导和气道管理，确保在极少数需要建立外科气道的情况下，环甲膜位置已准确识别。</li>
	</ol>

3. 预测困难气道管理所需参数的获取
注意：为预测困难气道管理，需测量皮肤至会厌距离及舌后壁厚度（LPWT）。这些步骤应在麻醉诱导前完成。
<ol>
	<li>测量皮肤至会厌距离时，将患者置于仰卧位，颈部保持中立位，并按照步骤1.1所述准备探头和超声设备。
	<ol>
		<li>将高频线性探头横向置于颈部前方甲状舌骨膜水平处（图6A）。</li>
		<li>识别会厌，其表现为位于舌骨与甲状软骨之间的等回声结构中部的低回声结构（图6B）。会厌的喉面形成一条高回声线，代表气-黏膜界面。若会厌前缘显示不清，可向任意方向倾斜探头以优化图像。</li>
		<li>注意观察一个回声增强（含脂肪）的会厌前间隙20。</li>
		<li>测量皮肤至会厌距离时，通过触摸屏幕底部较大的冻结按钮冻结图像。随后点击屏幕右侧的蓝色距离按钮。用手指将一个游标拖动至会厌浅表表面，另一个游标移至颈部前表面（皮肤）。皮肤至会厌距离将显示在屏幕左上方的灰色框内。 
		注：根据此测量结果可预测插管困难。若皮肤至会厌距离大于2.7 cm，则直接喉镜检查时可能遇到Cormacke-Lehane分级为3级或4级的情况21。</li>
	</ol>
	</li>
	<li>测量喉旁壁厚度（LPWT）时，将患者置于仰卧位，颈部保持中立位。
	<ol>
		<li>将低频凸阵探头置于乳突下方冠状面位置，并与颈动脉走行一致（图7A）。</li>
		<li>使用多普勒血流模式识别颈动脉。操作时按下屏幕左下角的C按钮。用手指在触摸屏上将黄色取样框移至颈动脉血管区域。通过观察搏动性血流信号确认颈动脉。</li>
		<li>测量LPWT时，按下屏幕底部的冻结按钮冻结图像（图7B），然后点击屏幕右侧的蓝色距离按钮。将一个游标置于颈动脉下缘，第二个游标置于气道前壁。LPWT值将显示在屏幕左上方的灰色框内。 
		注：在需要快速顺序诱导的紧急气道情况下，可省略步骤3.2，因面罩通气可能无需进行，且为节省时间起见。</li>

未被发现的食管插管会导致显著的并发症和死亡率；因此，必须立即识别食管插管，并将导管重新放置到气管内，以避免发生灾难性的呼吸功能损害。传统的气管插管确认方法依赖于临床检查，例如听诊双侧呼吸音和观察胸廓起伏3,4。即使美国麻醉医师协会（ASA）已将呼气末二氧化碳（end-tidal CO2）监测列为确认气管插管的必要手段，仍存在未能及时发现的食管插管案例，导致严重的并发症和死亡5。将气管超声检查纳入插管操作的主要优势之一，是能够立即识别食管插管，并实时、直接地可视化确认导管位于气管内。在最近的一项荟萃分析中，该方法用于确认气管插管的汇总敏感性和特异性分别为98%和94%，显示出其卓越的诊断准确性6。在本方法学论文中，将通过视频示例展示导管误插入食管的情况、对该并发症的即时识别，以及导管在气管内的正确放置过程。这突显了在插管操作过程中，POCUS（床旁超声）所提供的实时可视化优势。

患者气道的术前评估包括传统的床旁临床检查（例如，Mallampati 分级、张口度、颈椎活动范围等）。这些评估方法存在若干问题。首要且可能最突出的问题是，它们在预测困难气道情况方面的准确性较低10。此外，这些检查需要患者的配合，而在某些临床情况下无法实现（如创伤或意识状态改变的患者）。

此外，研究人员已开始识别提示面罩通气困难的解剖结构。其中一种解剖结构是咽侧壁，其厚度（LPWT）已被证明与阻塞性睡眠呼吸暂停（OSA）的严重程度及呼吸暂停-低通气指数相关15。初步数据还表明，术前测量LPWT可为面罩通气困难程度提供依据16。本文及配套视频将演示如何使用便携式超声成像获取LPWT，以评估患者OSA的严重程度及面罩通气困难的可能性。

在使用超声进行气管插管之前，可通过步骤2所述方法，直接观察甲状软骨和环状软骨，并在纵向和横截面视图中定位环甲膜（CTM），从而标记CTM位置（图4 和 图5），以便在需要建立外科气道时节省寻找CTM的时间。

气道POCUS在急诊或困难气道管理中有多种应用指征。在本视频中，我们将回顾其中一些指征，以及如何获取必要的超声图像以实现有效评估。使用气道POCUS的主要优势在于它是一种快速、易于获取且无创的影像学方法，可用于高危临床情况。尽管这些操作规程可以相对较快地掌握，但我建议您在非紧急情况下进行练习，以达到熟练和高效。这是本部门的住院医师Erica Chemtob博士，正在演示该操作流程。首先，在高频线性超声探头上涂抹单层超声耦合剂，以准备探头。在触摸屏的换能器菜单中选择线性探头，并从下拉菜单中指定MSK。接着，按下触摸屏左下角的2D按钮，将超声设备置于扫描模式。全身麻醉诱导完成后，将探头以横向位置放置于患者颈部前正中线上，位于胸骨上切迹稍上方。确定气管中线，并注意位于气管外侧稍窄的食管。为进一步确认解剖结构，可向侧方扫查以识别颈总动脉和颈内静脉。此外，应观察到由于气管插管的存在，气管后方的强回声结构消失，并出现特征性的子弹形声影。为进行环甲膜（CTM）识别，将患者置于仰卧位，颈部伸展。根据普通体型患者的情况，调整超声探头深度至约1.5至2厘米，因为环甲膜位于颈部较浅处。随后，采用第一种环甲膜识别方法：将线阵高频探头置于患者颈部甲状软骨稍偏尾侧的矢状面上。甲状软骨在扫描图像的颅侧表现为表浅的低回声结构，并产生声影。现在定位位于尾侧的环状软骨，其表现为低回声。利用其下方的空气-黏膜界面，识别位于这两个结构之间的环甲膜，该膜表现为沿气管长轴走行的高回声线。为进一步确认，向尾侧扫描以定位气管环，其表现为高回声的串珠样结构。或者，也可通过将线阵高频探头置于甲状软骨水平的横切面上来定位环甲膜，此时甲状软骨呈高回声，并投射出一个黑色三角形的声影，其中尖端为最表浅部分。继续向尾侧扫描，直至黑色三角形声影随甲状软骨终止而消失，此时即为环甲膜起始处。识别此处为气-黏膜界面，表现为一条伴有混响效应的明亮白色线条。继续向尾侧方向扫描，直至声门下区（CTM）结束并出现环状软骨。环状软骨将表现为围绕气管的一条低回声带。确定环状软骨后，再找到环甲膜（CTM）的下界。为确认已正确识别解剖结构，可逆向操作，再次向头侧方向扫描，重新识别环甲膜及甲状软骨。一旦确认这些解剖标志，应在患者皮肤上标记环甲膜的位置。为测量皮肤至会厌的距离，需按先前演示的方法安置患者，并准备好探头和超声设备。将高频线性探头置于颈部前侧甲状膜水平，呈横向位置。在舌骨与甲状软骨之间的中点处识别会厌，即位于此区域的低回声结构。会厌的喉表面形成一条高回声线，代表空气-黏膜界面。如果会厌前缘显示不清，可将探头向任意方向倾斜。注意观察高回声的会厌前间隙。为测量皮肤至会厌的距离，可触摸触摸屏底部较大的冻结按钮以冻结图像。随后，选择屏幕右侧的蓝色距离按钮。为测量声门旁工作厚度（LPWT），将曲线型低频探头置于乳突下方、与颈动脉对齐的冠状位。然后，通过按屏幕左下角的 C 按钮使用多普勒血流来识别颈动脉。用手指在触摸屏上将黄色框移动至颈动脉血管区域，并通过观察脉动性血流来确认颈动脉。为测量左心室后壁厚度（LPWT），请参照之前的方法冻结图像。将一个游标置于颈动脉下缘，另一个游标置于气道前壁。左室后壁厚度（LPWT）将显示在屏幕左上方的灰色框内。超声测量左室后壁（LPW）显示，LPWT 与基于呼吸暂停-低通气指数的阻塞性睡眠呼吸暂停（OSA）严重程度相关。必须牢记，在紧急气道管理的情况下，时间至关重要，可能影响患者的预后。实施气道超声的医护人员必须熟练掌握图像采集与解读技能，才能在这些情况下发挥积极作用。
