1. 术前标记
<ol>
	<li>标记双侧腋窝的下皱襞、乳房边缘以及乳房下皱襞（图3）。</li>
</ol>
2. 定位与麻醉
<ol>
	<li>诱导全身麻醉并进行气管插管。</li>
	<li>将患者置于仰卧位，双上肢外展90°。</li>
	<li>使用聚维酮碘皮肤消毒剂对手术区域进行消毒。</li>
	<li>用无菌巾包裹双侧肢体，并固定于外展90°位置。</li>
</ol>
3. 前哨淋巴结活检（SLNB）
<ol>
	<li>使用1 mL注射器吸取碳纳米颗粒悬浮液与生理盐水按1:1比例混合的混合物，总体积为1 mL，用于制备前哨淋巴结示踪剂。将示踪剂（0.3 mL）注射至乳房外上象限实质内的任意三个点，按摩5分钟，随后等待5分钟。</li>
	<li>使用10 mL注射器吸取亚甲蓝与生理盐水按1:5比例混合的混合物，总体积为10 mL，并在超声引导或术前标记的指导下，将其注射至乳房周围的乳房悬韧带区域（即乳房投影区15），以明确解剖边界和范围。 
	注意：染色并非手术中的必要步骤。该步骤可辅助尚未完全掌握手术技巧的乳腺外科医生控制解剖范围。</li>
	<li>沿先前标记的线条做一条约4 cm长的弧形腋窝切口，以直视下完成前哨淋巴结活检（SLNB）。 
	注意：前哨淋巴结活检经术中冰冻切片病理检查确认为阴性。
	<ol>
		<li>若前哨淋巴结活检结果为阳性，则行完整的腋窝淋巴结清扫术（ALND），切除腋窝I级和II级淋巴结。</li>
		<li>确保切口前缘距离胸大肌前缘超过1 cm，且不超过腋前线。必要时，可适当向后延长切口。</li>
	</ol>
	</li>
</ol>
4. 气腔构建
<ol>
	<li>将患者手术侧上肢屈曲，并固定于头架上。</li>
	<li>从腋窝切口处寻找胸大肌外侧缘。在直视下分离乳腺后间隙约 2 cm，皮下瓣约 3 cm。</li>
	<li>将切口保护器置入切口，然后连接四通道单端口装置（图 4）。上方放置一个 12 mm 穿刺套管，下方放置一个 10 mm 穿刺套管，两侧各放置一个 5 mm 穿刺套管。</li>
	<li>将 12 mm 穿刺套管连接恒压气腹装置，以建立操作空间。通过以 8 mmHg 压力和 40 L/min 气流速率注入 CO2 维持气腹。</li>
	<li>然后通过 10 mm 穿刺套管插入直径为 5 mm、观察角度为 30° 的斜端刚性内窥镜。</li>
	<li>分别通过两个 5 mm 穿刺套管插入电钩和腹腔镜抓钳。</li>
</ol>
5. 胸大肌后间隙分离
<ol>
	<li>以乳房周围韧带作为乳房的边界，注意暴露并辨认韧带结构15。可通过染色、手指按压以及内镜器械的阻力来确保韧带的完整性。 
	注意：术中可由助手协助进行手指按压。此外，主刀医生也可暂时中止内镜操作，亲自进行手指按压以评估组织情况。上述操作均不影响内镜视野下的观察。</li>
	<li>按照图5所示顺序进行解剖。在内镜直视下使用电钩分离乳房后间隙，并向上延伸至锁骨下韧带，向内至胸骨旁韧带，向下至乳房下皱襞，向外至乳腺外侧缘。 
	注意：本例患者术前已确诊为导管癌（见图2），因此术中完整保留胸大肌筋膜16,17。该策略具有双重意义：一方面可减少手术创伤及术后不适，从而提高患者生活质量；另一方面，保留胸大肌筋膜有助于牢固固定补片，防止假体移位。</li>
</ol>
6. 皮下层解剖
<ol>
	<li>牵拉切口保护器，到达皮下层。 
	注意：以下措施可确保外科医生已到达皮下层。首先，在建立气腔阶段，可在直视下解剖部分皮下瓣。其次，借助内窥镜的放大效应，可在内窥镜视野下清晰观察到乳腺的浅筋膜。第三，乳腺通过 Cooper 韧带悬吊于真皮层，该结构可在内窥镜下观察到，从而引导外科医生进行皮下层的分离18。</li>
	<li>按照分区解剖顺序（图6），使用电钩依次解剖：外上象限、内上象限、外下象限、乳头后区及内下象限。</li>
	<li>使用内窥镜剪刀&#160;切断乳头根部，防止因热效应导致乳头乳晕复合体（NAC）&#160;发生坏死。</li>
	<li>从乳头后缘取组织活检标本，用于术中冰冻切片检查。</li>
	<li>使用电钩仔细分离乳腺的内下象限。</li>
	<li>经腋窝切口取出完整的乳腺标本。 
	注意：在内窥镜视野下单孔途径实现止血较为困难。使用电钩止血可能导致出血部位形成焦痂，可能妨碍对出血点的观察，并存在皮肤灼伤的风险。</li>
</ol>
7. 植入物放置
<ol>
	<li>称重乳腺标本，测量植入腔的直径，并结合术前测量数据选择合适的假体。</li>
	<li>用 2,000 mL 温热的无菌蒸馏水冲洗植入腔。</li>
	<li>将植入腔压力调整至 5 mmHg，在内窥镜视野下仔细检查植入腔并完成止血。</li>
	<li>用 0.45%–0.55% 聚维酮碘溶液浸泡植入腔 10 分钟，消毒手术区域，并更换手套。</li>
	<li>用 2 片补片完全包裹假体。使用 3-0 可吸收缝线和单纯间断缝合法缝合补片并固定。</li>
	<li>将假体置入植入腔内。</li>
</ol>
8. 引流
<ol>
	<li>在皮下层的双侧乳房下皱襞和腋窝处各放置一根引流管，并保持低负压吸引。</li>
	<li>使用4-0可吸收缝线缝合腋窝切口伤口（图7）。</li>
</ol>
9. 术后护理
<ol>
	<li>使用弹性绷带对乳房和腋窝进行加压包扎。</li>
	<li>对腋窝伤口进行适当的加压包扎。 
	注意：主要对假体的上缘和下缘施加压力，而非乳头乳晕复合体，以防止乳头乳晕复合体发生缺血性坏死。</li>
	<li>静脉注射适当的预防性抗生素24小时（将1.5 g头孢呋辛溶解于100 mL生理盐水中）。</li>
	<li>更换伤口敷料时注意观察乳晕复合体（NAC）的血液循环情况。必要时在NAC周围涂抹硝酸甘油以促进血液循环。观察NAC的颜色、有无水肿及瘀斑。</li>
	<li>保持引流管通畅；每日观察并记录引流液的颜色、性状及引流量。若连续3天每日引流量少于20 mL，可拔除引流管。</li>
	<li>术后2周内指导患者进行上肢功能锻炼19.</li>
	<li>确保患者从第2天开始穿戴压力衣，持续3至6个月nd 术后第几天</li>
	<li>确保患者对光面假体进行乳房假体移位锻炼20,21,22. 
	注意：对侧乳房以相同方式手术。</li>

传统的乳腺手术以乳房切除术为代表，常导致不理想的美容效果，并可能对患侧上肢功能产生不利影响。这种情况可引起显著的心理困扰，最终导致患者术后生活质量下降24,25,26。微创手术是现代外科实践中的关键进展。随着内镜技术的引入，传统手术的诸多不足已得到显著改善。与此同时，乳房重建因其能够恢复乳腺癌患者的乳房外形而日益普及24。研究表明，乳房重建可降低乳腺癌患者术后抑郁的发生率，并提升其术后生活质量27。这与新兴的&#34;生物-心理-社会&#34;医学模式相一致。随着内镜技术与乳房重建的结合，乳腺外科已进入一个以人性化和优化患者预后为核心特征的新阶段。这些目标正是我们始终追求并坚持的核心理念。
与传统的经乳晕后双平面法（IBBR）联合内镜保留乳头乳晕复合体乳房切除术（E-NSM）相比，经单一腋窝切口行内镜保留乳头乳晕乳房切除术联合即刻胸大肌前假体重建具有明显优势。首先，该术式可通过一个隐蔽的单一腋窝切口完成全部手术操作，显著降低切口的可见度，从而获得良好的术后美容效果11,28,29。此外，E-NSM 技术会破坏乳头-乳晕复合体（NAC）的纵向血供系统，转而完全依赖皮肤的血管网络供血。避免在乳房皮肤上做切口，可降低 NAC 及皮瓣缺血性坏死的风险8,30,31,32。与胸大肌下重建相比，胸大肌前重建操作更简便，乳房外形更自然，并可减少胸部功能障碍和术后疼痛33。然而，在胸大肌前重建中，由于缺乏胸大肌的保护，假体或补片与乳房表面切口直接接触，可能导致假体外露甚至丢失。此类并发症可能由皮瓣感染或缺血性坏死引发34,35。因此，本文所介绍的技术不仅确保了肿瘤治疗的安全性，还实现了更优的美容效果和更高的术后生活质量。
有几个关键的技术要点需要注意。首先，术前必须明确标记患者乳腺的范围。经验丰富的外科医生可在术前通过手动方式标记乳腺的边界，而经验较少的医生则可采用术前超声来勾画乳腺的边界。乳房周围韧带被视为乳腺的自然边界，可作为乳腺解剖的解剖学标志15,36。这些韧带既可在术前通过超声观察，也可在术中通过内镜视野下识别。其次，应遵循从胸肌后间隙向乳腺皮下层进行分离的解剖顺序，这被认为是一种便捷且高效的策略37。第三，在解剖胸肌后间隙时，沿乳腺边缘进行分离需格外细致。可通过染色剂、手指按压以及内镜器械的压力反馈感来评估分离程度。无论采用术前标记，还是使用染色等技术来界定乳腺边界，其核心目的均在于引导外科医生暴露并识别乳房周围韧带，并精确切除乳腺组织。这对于乳房重建手术至关重要，并有助于实现良好的术后美学效果36。第四，建议在乳腺皮下层解剖过程中保持皮瓣厚度的一致性。我们采用的一种技术是关闭手术室灯光，观察内镜光源在腔内穿透乳腺表面皮肤的程度，从而评估皮瓣厚度的均匀性。
该方法存在一些局限性。首先，尽管结合IBBR的E-NSM技术已得到广泛应用，并被认为是一种相对成熟的技术，但在循证医学层面，仍缺乏高级别的证据来明确确立其安全性。此外，在我们机构中，我们仅采用单孔法进行内镜乳腺手术。这一决策的原因在于主刀医生已成功克服学习曲线，并认为单孔入路可减少切口数量、隐蔽瘢痕，并降低乳头乳晕复合体（NAC）及皮瓣发生缺血性坏死的概率。然而，需要指出的是，这一观点具有主观性，目前尚缺乏对照研究来证实我们的立场。
总之，在考虑特定患者时，致力于优化临床实践以获得更佳的美容效果并提高患者满意度具有重要意义。这一努力体现了人文关怀与美学理念的融合。我们共同的目标包括对美的追求以及确保最大限度地使患者获益。

随着乳腺癌治疗的不断进步，越来越多的乳腺外科医生在治疗时不再仅仅关注恶性肿瘤的处理，而是更加重视患者治疗后的外观及生活质量。保留乳头乳房切除术（NSM）因其具有相当的肿瘤学效果，且患者满意度和美容效果良好，现已成为双侧早期乳腺癌患者以及接受预防性乳房切除术患者的常规手术选择1,2,3。
内镜乳腺手术（EBS）是一种新兴且具有前瞻性的外科技术，在保留乳头的乳房切除术（NSM）和乳腺保留手术中日益受到关注。与传统乳腺手术方法相比，EBS表现出更优的美容效果，由于其切口更为隐蔽，患者满意度更高，同时具有相当的肿瘤学安全性4,5,6。
然而，传统的经内镜保留乳头乳晕的乳房切除术（E-NSM）联合即刻假体重建术（IBBR）通常需要两个或更多的切口，切口位置通常位于腋窝、腋前线以及乳晕周围区域4,7。遗憾的是，该技术常导致美容效果不理想，尤其在采用乳晕周围切口的情况下更为明显，因为E-NSM会切断乳头乳晕复合体（NAC）的垂直血供通路，仅依赖真皮血管网提供营养8，从而可能导致NAC发生缺血性坏死。此外，在胸大肌前乳房重建中，乳房表面的切口可能成为感染或皮瓣缺血的入口，增加假体暴露、乳头坏死甚至假体丢失的风险7,9,10。随着内镜技术与器械的迅速发展，近年来E-NSM联合IBBR的新术式主要聚焦于单一腋窝切口结合充气技术11,12。这些创新带来了显著优势，包括降低乳头坏死风险、减轻患者不适从而加快恢复、显著改善美容效果以及优化患者整体获益等12,13。
本文全面描述了经单侧腋窝切口行内镜双侧乳头保留乳房切除术联合即刻胸大肌前假体重建的手术步骤。旨在展示该术式在可行性、卓越的美容效果以及生理学考量方面的优势。
为帮助读者理解本实验方案，我们提供一个典型病例进行说明。该患者为33岁女性，超声检查显示双侧乳腺肿块，分类为BI-RADS 4类，且肿块尺寸相对较大（图1）。通过Mammotome辅助的微创切除术进行组织病理学确认，发现双侧导管癌 原位14 (图2）。这位年轻女性存在双侧乳头不对称，并强烈希望纠正这一问题。由于她热爱舞蹈，因此对术后美观效果有较高要求。在向患者介绍手术方案后，她最终选择接受内窥镜下双侧保留乳头的乳房切除术 通过 单腋窝切口即刻胸大肌前假体重建乳房术

Endoscopic Bilateral Nipple-sparing Mastectomy via a Single Axillary Incision with Immediate Pre-pectoral Implant-based Breast Reconstruction

肿瘤整形乳房手术在确保肿瘤学安全的同时注重改善美容效果，从根本上改变了乳腺癌外科治疗的格局，催生了多种乳房重建技术。保留乳头乳晕复合体的乳房切除术（NSM）联合即刻假体植入乳房重建（IBBR）已成为早期乳腺癌治疗的重要手段。近年来，随着微创外科理念的发展，内镜技术在乳房手术中的应用日益广泛，包括内镜下保乳手术（E-BCS）和内镜下保留乳头乳晕的乳房切除术（E-NSM）等。得益于切口隐蔽且更短、术中视野更清晰以及可避免放疗等优势，E-NSM联合IBBR的应用正逐渐增多。然而，传统的E-NSM联合IBBR通常需要两个或更多切口，可能导致美容效果不理想，甚至出现假体丢失。本文系统阐述了经单一切口行内镜双侧保留乳头乳晕乳房切除术联合即刻胸肌前假体植入乳房重建的复杂手术步骤，相关经验源于本机构的临床实践。该术式的主要优势包括更佳的美容效果、术后生活质量提高，以及因采用单一切口行胸肌前假体重建而带来的生理功能改善。

手术持续了4小时，记录出血量为30 mL。术后恢复过程未出现明显并发症。术后第7天拔除引流管，患者于术后第10天出院。图8）。术后病理分析显示为导管癌 原位 在左侧残留乳腺组织中确认存在病变。同时，右侧残留乳腺组织中未观察到癌细胞。双侧乳头基底部及前哨淋巴结的最终病理学评估结果均为恶性肿瘤阴性。术后五个月时，患者对其乳房外观表示满意。图9).
2021年6月至2022年10月期间，一组共10例被诊断为双侧乳腺癌（BBC）或需接受对侧预防性乳房切除术（CPM）的患者，接受了经单侧腋窝切口的内镜下双侧乳头保留乳房切除术，并同期行胸大肌前假体重建乳房。患者平均年龄为42岁，平均体重指数（BMI）为22.12 Kg/m2。手术平均持续时间为4.25小时，术中平均出血量为33 mL。每位患者的术后恢复过程均顺利，无显著并发症发生，术后平均住院时间约为11天。术后，所有患者均接受定期随访，术后前两年内每三个月安排一次随访就诊（表1）。值得注意的是，3例患者的Harris评分被评为优，7例患者评分为良23。所有患者均对术后美容效果表示满意。

鉴于传统内镜保留乳头乳晕复合体的乳房切除术存在不足，本文介绍一种经单侧腋窝切口行内镜双侧保留乳头乳晕复合体的乳房切除术联合即刻胸大肌前假体重建的新技术、新理念及详细手术步骤。一名33岁女性超声检查发现双侧乳腺肿块，分类为BI-RADS 4类，且肿块相对较大。通过Mammotome辅助下的微创切除获取组织，经病理学检查确诊为双侧导管原位癌。通过单侧腋窝切口行内镜下双侧乳头保留乳房切除术，并同期行胸大肌前假体重建。超声显示左乳六点钟位置存在结节，右乳十点钟位置存在多发钙化灶。术前病理结果显示双侧乳腺导管原位癌。标记双侧腋窝下皱襞、乳房边缘及乳房下皱襞。实施全身麻醉并进行气管插管。将患者置于仰卧位，双上肢外展90度。使用聚维酮碘皮肤消毒剂对手术区域进行消毒。用无菌手术单包裹双侧肢体，并将其固定于外展90度的位置。使用1毫升注射器抽取碳纳米颗粒混悬注射液与生理盐水按1:1比例混合后的溶液，配制总量为1毫升的前哨淋巴结示踪剂。在乳房外上象限的实质内任意三点注射示踪剂。按摩五分钟，等待五分钟。沿之前标记的线做一条长约四厘米的弧形腋窝切口，直视下完成前哨淋巴结活检（SLNB）。前哨淋巴结活检术中冰冻切片检查结果为阴性。将患者手术侧手臂弯曲，并固定于头架上。从腋部切口处寻找胸大肌的外侧缘。在直视下解剖胸肌后间隙2厘米，皮下脂肪3厘米。将切口保护器插入切口，随后连接四通道单端口装置。将12毫米套管针连接恒压气腹装置，以建立操作空间。通过注入二氧化碳维持气腹，压力为8毫米汞柱，气流速率为40升/分钟。将直径为5毫米、观察角度为30度的斜端硬性内窥镜经由10毫米套管针插入。分别通过两个5毫米的套管插入电钩和腹腔镜抓钳。以乳房周围韧带作为乳房的边界，注意暴露并辨认韧带结构。可通过染色、手指按压以及内镜器械的阻力感来确保韧带的完整性。在内窥镜视野下使用电钩解剖胸膜后间隙。将解剖范围向上延伸至锁骨下韧带，向内至胸骨旁韧带，向下至乳房下皱襞，向外至乳腺外侧边界。本例为术前确诊的导管原位癌。因此，术中应完全保留胸大肌筋膜，以减少创伤和疼痛，并允许将补片固定于胸大肌筋膜，防止假体移位。牵拉伤口保护器至皮下层。遵循分区解剖顺序进行操作。解剖操作使用电钩按以下顺序进行：外上象限、内上象限、外下象限、乳头后区域。使用内镜剪刀切断乳头根部，以防止乳头乳晕复合体坏死。从乳头后缘取组织活检标本，用于术中冰冻切片检查。在下象限。利用电钩仔细分离乳腺的内下象限。通过腋下切口取出完整的乳腺标本。对侧乳房以相同方式手术。用2,000毫升温热的无菌蒸馏水冲洗假体腔。用0.45%至0.55%聚维酮-碘溶液浸泡假体腔10分钟。对手术区域进行消毒，并更换手套。用两层网片完全覆盖假体。使用3-0可吸收缝线，采用单纯间断缝合法缝合网片以固定。将假体植入种植腔内。在皮下层的双侧乳房下皱襞和腋窝处各放置一根引流管。保持低负压吸引。使用4-0可吸收缝线关闭腋窝切口。2021年6月至2022年10月期间，一组10例被诊断为双侧乳腺癌或需行对侧预防性乳房切除术的患者，通过单侧腋窝切口接受了内镜下双侧乳头保留乳房切除术，并同期行胸大肌前假体重建乳房。患者的平均年龄为42岁，平均体重指数为22.12千克/平方米。手术过程的平均持续时间为4.25小时，术中平均失血量为33毫升。每位患者的术后恢复过程均顺利，无显著并发症发生，术后住院时间通常平均为11天。随后，所有患者均接受定期随访，术后前两年内每三个月安排一次复诊。 值得注意的是，三名患者的Harris评分被评为优秀，而七名患者被评为良好。他们普遍对术后美容效果表示满意。对于光滑表面的乳房假体，有必要进行假体移位锻炼。我们得出结论，该手术方法安全可行，是年轻患者、双侧乳腺癌患者、需行对侧预防性乳房切除术且对美观和身体功能有较高要求的患者的优选方案。
