本工作由重点研发计划资助&amp;四川省科学技术厅项目（2020YFS0539）

动物护理与使用的操作程序已获得绵阳师范学院伦理委员会的批准，且严格遵守了所有相关的机构及政府关于动物伦理使用的法规。本研究使用了SPF级大鼠（昆明品系，雌雄兼用，体重180–220 g），动物来源于商业渠道（见 材料表）。所有动物均饲养于无病原体环境中，并提供 随意摄取 食物获取
1. 试剂的准备
<ol>
	<li>称取2.5 g水杨酸钠粉末，配制5%水杨酸钠溶液（见 材料表使用电子天平称量后，将粉末溶于少量蒸馏水中，并定容至总体积50 mL。</li>
	<li>制备汤剂 D. caudatum （12.8 克/千克）。称取 128 克 D. caudatum （参见 材料表），放入含有1000 mL实验室蒸馏水的圆底烧瓶中，浓缩至最终体积为100 mL。</li>
</ol>
2. 分组与给药
<ol>
	<li>将大鼠分为不同组别，并通过灌胃给予所需溶液。 
	注意：本研究共使用18只大鼠（雄性9只，雌性9只），随机分为三组：对照组（C组）、模型组（M组）和治疗组（T组），每组6只。对照组给予生理盐水，模型组给予5%水杨酸钠溶液，剂量为10 mL/kg/天，连续给药5周。15. 治疗组先以5%水杨酸钠溶液按10 mL/kg/天的剂量预处理5周，随后给予 D. caudatum 以1 mL/100 g剂量灌胃给药10天15,16.</li>
	<li>使用尾部提拉法将大鼠粪便收集至干燥无菌的微量离心管中17 在实验的最后一天，将其放入液氮超低温冰箱中冷冻。</li>
	<li>采样结束后，采用颈椎脱位法处死大鼠（遵循机构批准的实验方案）。使用剪刀切开腹腔，暴露腹主动脉。
	<ol>
		<li>将针头（23–25 G）几乎平行插入腹主动脉，采集血清，并保存于超低温冰箱中。从腹腔中取出胃组织16 用10%福尔马林溶液浸泡以保存。 
		注意：在M组，若胃黏膜扩张、充血且腺体形态规则，则认为慢性胃炎模型构建成功15同时观察到固有层中有少量炎性细胞浸润。</li>
	</ol>

</ol>
3. 胃壁的病理切片
<ol>
	<li>取各组大鼠的胃壁组织样本制备病理切片，随后进行常规苏木精-伊红染色，以观察和分析病理变化18。</li>
</ol>
4. 血清胃肠激素的测定
<ol>
	<li>按照试剂盒说明书（见材料表），采用ELISA法检测血清样本中胃泌素（GAS）和丙二醛（MDA）的含量19。使用酶标仪进行分析。</li>
</ol>
5. 粪便总DNA提取及PCR扩增与纯化
<ol>
	<li>使用市售的DNA提取试剂盒（参见材料表）进行微生物DNA提取，并使用紫外-可见分光光度计测定其浓度和纯度16。随后通过1%琼脂糖凝胶电泳评估DNA的完整性18。</li>
	<li>使用热循环PCR系统，采用引物338F（5′-ACT CCT ACG GGA GGC AGC AG-3′）和806R（5′-GAC TAC HVG GGG GTW TCT AT-3′）扩增细菌16S rRNA基因17。引物来自商业来源（参见材料表）。 
	注意：按照以下PCR程序操作：变性（95 °C，3分钟），随后进行27个循环（95 °C，30秒；55 °C，30秒；72 °C，45秒），最后延伸（72 °C，10分钟）。PCR反应体系包含以下组分：4 µL 5× DNA聚合酶缓冲液、2 µL 2.5 mM dNTPs、每种引物各0.8 µL（5 µM）、0.4 µL DNA聚合酶以及10 ng模板DNA（参见材料表）。</li>
	<li>依次使用2%琼脂糖凝胶、市售DNA凝胶回收试剂盒和荧光计对PCR产物进行提取、纯化和定量17。</li>
</ol>
6. 测序
注意：步骤6和步骤7按照先前发表的方法进行20,21。
<ol>
	<li>使用Illumina平台进行测序。 
	注意：DNA片段的一端与芯片中嵌入的连接子的碱基互补，从而将其固定在芯片上。另一端随机与另一个相邻埋入接头的碱基互补，形成&#34;桥&#34;结构。</li>
	<li>进行PCR扩增以生成DNA簇。将DNA扩增产物线性化为单链。</li>
	<li>引入经过修饰的DNA聚合酶以及带有四种不同荧光标记的脱氧核糖核苷三磷酸（dNTPs）。每个循环仅合成一个碱基。</li>
	<li>利用激光扫描反应板表面，确定各模板序列在第一轮反应中聚合的核苷酸类型。</li>
	<li>通过化学方法切割&#34;荧光基团&#34;和&#34;终止基团&#34;，重新暴露3'端黏性末端，随后进入第二个核苷酸的聚合步骤。</li>
	<li>通过分析每一轮收集的荧光信号结果，获得模板DNA片段的序列。</li>
</ol>
7. 测序数据处理
<ol>
	<li>首先，对原始 fastq 数据的读段进行修剪，去除质量分数 &#60; Q20 的位点，并剔除含有不确定碱基的读段。此外，允许完全匹配的引物中存在两个错配。随后，将重叠区域 &#62;10 bp 的序列进行合并。</li>
	<li>使用 UPARSE 将操作分类单元（OTUs）按 97% 的相似性聚类，并利用 UCHIME 去除嵌合序列20,21。</li>
	<li>采用 RDP Classifier 算法，以 70% 的置信阈值，将每个 16S rRNA 基因序列与 Silva（SSU123）16S rRNA 数据库比对，进行分类学分析。</li>
	<li>分别使用 Mothur 和 Qiime 进行 Alpha 多样性分析和 Beta 多样性分析（见 材料表）。</li>
</ol>
8. 统计分析
<ol>
	<li>使用统计分析软件（参见材料表）对本研究中的数据进行分析。多个组之间的比较采用单因素方差分析（ANOVA），两组之间的比较采用最小显著性差异（LSD）检验。</li>
	<li>所有比较中，以P &#60; 0.05 为差异具有统计学意义，P &#60; 0.01 为差异具有极显著意义。</li>

D. caudatum 是羌族常用的民间药物12，在治疗胃肠道疾病方面表现出显著疗效。随着现代药理学研究的进展，胃肠道微生态失衡导致的菌群失调被确认为急性和慢性胃肠道疾病的关键因素22,23。肠道中的某些微生物在维持机体动态平衡中发挥着重要作用，而血清中的激素则反映了机体的生理与病理状态。因此，本研究聚焦于肠道菌群与胃肠道激素。
16S rRNA 高通量基因测序是检测肠道微生物的主要方法。该技术能够检测所采集样本中微生物的种类和功能，实现对各类肠道微生物的准确且定量的分析。近年来，16S rRNA 高通量基因测序技术迅速发展，已广泛应用于各种生态环境中的微生物多样性分析24,25,26。
通过基于Illumina的16S rRNA基因测序对三组大鼠肠道菌群进行详细比较，研究了D. caudatum对胃炎大鼠肠道菌群的影响。现代科学研究表明，乳杆菌属（Lactobacillus）、Muribaculaceae科未分类属（norank_f__Muribaculaceae）、罗姆布茨氏菌属（Romboutsia）和双歧杆菌属（Bifidobacterium）均为益生菌，各自具有特定的健康益处，例如降低多种恶性肿瘤的风险、产生具有抗炎潜力的维生素K、促进多糖代谢以及改善腹泻和便秘27,28,29。梭菌属严格意义上的1型（Clostridium_sensu_stricto_1）是一种与肠道损伤相关的致病菌，可引起炎症和菌血症，并与血液系统及胃肠道肿瘤密切相关30。慢性胃炎和萎缩性胃炎是胃部肿瘤发生的重要促成因素31。
β多样性分析表明，经处理后乳杆菌属（Lactobacillus）、Muribaculaceae科未分类属（norank_f__Muribaculaceae）、罗姆布茨氏菌属（Romboutsia）、双歧杆菌属（Bifidobacterium）和严格梭菌属1（Clostridium_sensu_stricto_1）的含量发生了显著变化。其中，乳杆菌属、Muribaculaceae科未分类属和罗姆布茨氏菌属的含量显著增加，而经尾枝孢霉（D. caudatum）处理后，双歧杆菌属和严格梭菌属1的含量则显著降低。此外，群落柱状图显示，T组的群落组成更接近C组。这些结果表明，尾枝孢霉（D. caudatum）可能改善大鼠胃炎的发病情况。然而，尾枝孢霉（D. caudatum）导致益生菌双歧杆菌属含量下降的具体机制仍需进一步研究。
ELISA试剂盒方法具有高灵敏度、强特异性、操作简便以及适用于小样本量的优点32。本研究采用该方法检测大鼠血清中丙二醛（MDA）和胃泌素（GAS）的含量。MDA反映脂质过氧化程度，间接指示细胞损伤程度。在胃炎大鼠中，MDA水平显著高于正常大鼠，提示胃壁细胞受损。D. caudatum治疗显著降低了MDA水平，表明其在治疗胃炎方面具有潜在作用。相反，D. caudatum可提高降低的GAS水平，有助于胃炎的修复或治疗。
然而，16S rRNA 高通量基因测序和ELISA试剂盒方法均存在一定的局限性。在16S rRNA高通量基因测序中，由于分辨率较低，可能难以区分序列高度相似的菌株或属，从而导致在属、科或门等较高分类学水平上的分类困难33。对于ELISA试剂盒方法，通常需要进行稀释和洗涤等样本预处理步骤，这可能引入潜在的误差和变异性34。
D. caudatum 可能通过调节胃肠激素和肠道菌群来治疗胃炎，该研究为其广泛的临床应用提供了见解。然而，未来的研究应深入探讨其他胃肠激素、调节肠道菌群的机制以及所涉及的有效化学成分。

慢性胃炎（CG）是最常见的临床疾病之一，其特征是胃黏膜上皮出现慢性且持续的炎症性改变，常受到多种致病因素的反复侵袭1。CG的发病率在所有胃部疾病中居首位，占消化内科门诊就诊人数的40%至60%2。此外，其发病率通常随年龄增长而升高，尤其在中老年人群中更为显著3。毫无疑问，慢性胃炎显著降低了人们的生活质量，凸显了发现新型治疗药物的迫切需求。
大量研究报道，慢性胃炎（CG）的发生与发展与胃肠激素的分泌有关，例如胃泌素（GAS）4,5。GAS 是消化道中一种常见的胃肠肽类激素，可通过促进嗜酸性粒细胞释放组胺，刺激细胞分泌胃酸。此外，它还能改善胃黏膜的营养和血液供应，促进胃黏膜及壁细胞的增殖6。因此，胃泌素可作为评估 CG 发展程度的指标。此外，活性氧诱发的脂质过氧化产物可激活炎症细胞，从而导致慢性胃炎的发生。丙二醛（MDA）是脂质过氧化的标志物，常用于衡量氧化应激程度，可在一定程度上反映胃黏膜中自由基的水平。MDA 水平可指示自由基对局部胃黏膜中不饱和脂肪酸的攻击程度7,8。
肠道微生态由数百万种定植于宿主肠道内的微生物组成，在维持宿主健康和调节宿主免疫方面发挥着至关重要的作用。健康的肠道菌群具有丰富的物种丰度、较高的多样性以及稳定的微生物功能，可通过参与代谢过程形成一道保护屏障，抵御致病微生物的侵袭。肠道菌群失衡会使个体更易罹患急性和慢性胃肠道疾病9,10。近年来，针对胃肠道疾病的菌群治疗迅速发展，并展现出显著疗效11。综上所述，在认识和防治胃肠道疾病时，必须充分考虑肠道菌群的作用。
作为中国传统医药宝库中不可或缺的组成部分，民间药材在临床实践及民族医药现代化进程中具有重要意义。在绵阳市北川羌族地区，Desmodium caudatum (Thunb.) DC. 的根及全草已长期广泛用于缓解胃部不适。一些文献报道了使用 D. caudatum 治疗胃肠道疾病的科学依据，指出其具有止血、抗氧化及胃肠道保护等作用12,13,14。然而，由于缺乏深入研究，目前尚未建立其临床药效动力学机制。本文旨在基于胃肠激素和肠道菌群，探讨 D. caudatum 治疗胃炎的作用，为其合理临床应用提供依据。

Effects of Desmodium caudatum on Gastrointestinal Hormones and Intestinal Flora in Rats with Gastritis

为了初步探讨山绿豆（Desmodium caudatum）对大鼠胃炎及肠道菌群的影响，采用经典的水杨酸钠法建立慢性胃炎大鼠模型。将18只SPF级大鼠随机分为三组：对照组（C组）、模型组（M组）和治疗组（T组）。取各组大鼠胃壁组织进行病理切片检查。同时，采用ELISA法测定各组大鼠血清中胃泌素和丙二醛的浓度。此外，通过Illumina平台的16S rRNA基因测序技术，对三组大鼠肠道菌群组成进行详细比较，以探讨山绿豆（D. caudatum）对胃炎大鼠肠道菌群的影响。结果表明，山绿豆（D. caudatum）煎剂可降低丙二醛含量，提高胃泌素含量。此外，山绿豆（D. caudatum）煎剂能够增加肠道菌群的多样性和丰度，通过调节和恢复肠道菌群，对胃炎的治疗产生积极影响。

胃壁病理切片结果如图1所示。与C组相比，M组显示轻度胃壁萎缩和轻度炎症；然而，与M组相比，T组未见明显炎症、肠上皮化生或萎缩。这表明D. caudatum煎剂可有效改善胃炎。
血清胃肠激素检测结果见图2。M组丙二醛（MDA）含量显著高于C组，经D. caudatum煎剂治疗后，MDA含量显著降低。就胃泌素（GAS）含量而言，M组显著低于C组，而T组则显著升高。上述结果表明，D. caudatum煎剂可能通过调节胃肠激素水平来治疗胃炎。
Alpha多样性分析结果（如表1和图3所示）显示，M组与T组的菌群之间存在显著差异。C组和T组的大肠杆菌数量高于M组，表明经治疗后，大鼠肠道内细菌的数量和种类逐渐趋于正常水平。
物种组成分析 
根据图4A所示的群落柱状图，Lactobacillus 和 norank_f__Muribaculaceae 在 C 组中所占比例最大。M 组中 Clostridium_sensu_stricto_1 和部分 Helicobacter 的丰度较高。T 组的群落组成与 C 组更为相似，主要成分为 Lactobacillus 和 Romboutsia。此外，Spearman 相关性热图（图4B）结合 Circos 图（图4C）显示，C 组与 M 组在菌群组成上存在显著差异，优势菌群有明显不同。经治疗后，T 组趋于恢复至正常菌群状态。此外，图5 表明 M 组与 C 组在群落组成上存在极显著差异，提示胃炎大鼠肠道菌群的结构与组成发生了显著变化。治疗后，C 组与 T 组的菌群组成具有相似性，T 组与 M 组之间也存在一定相似性。上述结果表明，D. caudatum 煎剂可使胃炎大鼠的肠道菌群恢复至正常或接近正常的状态。
物种差异分析 
来自多物种比较的柱状图 图6A可以观察到，Norank_f_Muribaculaceae 在 C 组中丰度较高，且与 M 组和 T 组存在显著差异。治疗后，T 组中 Norank_f_Muribaculaceae 的数量呈上升趋势。此外，在 M 组中丰度较高的 Clostridium_sensu_stricto_1 对恶劣环境具有较强耐受性，治疗后其数量显著减少，提示 D. caudatum 在一定程度上改善了肠道菌群的生存环境。另一方面，LEfSe多级物种层次树分析 图6B 表明有 44 个物种在各组间存在显著差异。Norank_f_muribaculaceae、Clostridium_sensu_stricto_1 和 Romboutsia 分别在 C 组、M 组和 T 组中丰度较高。

作为中医药宝库中不可或缺的组成部分，民间医药材料对民族医学的临床实践与现代化具有重要意义。本文旨在基于胃肠激素和肠道菌群，研究断肠草（D.caudatum）治疗胃炎的作用，为其临床合理应用提供依据。已有大量研究表明，慢性胃炎的发生与发展与胃泌素等胃肠激素的分泌密切相关。它可作为评估慢性胃炎发展程度的指标。此外，活性氧化物引发的脂质过氧化产物可激活炎症细胞，从而导致慢性胃炎。在16S rRNA高通量基因测序中，较低的分辨率可能难以区分序列高度相似的菌株和属，导致在更高分类学层级（如属、科、门）的分类上出现困难。对于ELISA试剂盒方法，通常需要进行稀释和洗涤等样本预处理步骤，这可能引入潜在的误差和变异性。配制5%水杨酸钠溶液时，使用电子天平称取2.5克粉末，将其溶于少量蒸馏水中。然后将其稀释至总体积为50毫升。制备山蚂蝗（Desmodium caudatum）煎剂时，称取128克D. caudatum，放入含有1升蒸馏水的圆底烧瓶中。将混合物浓缩至终体积100毫升。根据处理方式将大鼠分为不同组别。通过颈椎脱位处死大鼠后，使用酶标仪通过ELISA法测定血清样本中胃泌素和丙二醛的含量。与对照组大鼠相比，模型组大鼠胃壁的病理学检查显示轻度胃壁萎缩和轻度炎症。相比之下，经石斛煎剂治疗的大鼠未见炎症、化生或萎缩，表明该煎剂具有治疗作用。血清胃肠激素检测结果显示，模型组大鼠的丙二醛水平高于对照组大鼠，治疗后该水平显著降低。与对照组相比，模型组大鼠的胃泌素水平较低，但在汤剂治疗组中显著升高，提示该汤剂在调节胃肠激素方面具有疗效。
