本研究由中央级公益性科研院所基本科研业务费专项资金（2019CZ-11）和中国康复研究中心项目（编号：2021zx-Q5）资助。

本研究是一项单盲随机对照试验，已获中国康复研究中心伦理委员会（CRRC-IEC-RF-SC-005-01）批准，并在中国临床试验注册中心注册（MR-11-23-023832）。
1. 参与者
<ol>
	<li>根据现有文献21采用实验组与对照组在治疗4周后的Fugl-Meyer运动功能评估上肢部分（FMA-UE）得分作为标准来计算样本量。设定效应量为0.28，检验水准（α）为0.05，Z值采用双侧分布，检验效能（power）为0.8，计算得所需样本量为28。考虑10%的脱落率，最终所需样本量为32。</li>
	<li>从中国康复研究中心作业治疗科招募患者。根据以下纳入标准选择患者：首次发病的右半球脑损伤（RHD）诊断；发病时间在3个月以内；年龄介于18至75岁之间；简易精神状态检查（MMSE）评分22&#62; 20；Brunnstrom分期I期或II期23 上肢和手部；以及惯用右手。</li>
	<li>排除明显抑郁、焦虑或合并严重躯体疾病且不配合训练的患者。</li>
	<li>仅纳入在研究开始前签署知情同意书的受试者。招募流程图如下所示： 图1.</li>
</ol>
2. 随机化与分组
<ol>
	<li>将符合实验标准的患者随机分配至实验组（EG）和对照组（CG）。指派未参与受试者评估或筛选的治疗师，使用计算机上的随机数据生成器（https://www.randomizer.org/）进行随机分组。</li>
</ol>
3. 干预措施
<ol>
	<li>对两组均给予常规药物治疗和常规康复治疗。所有患者每天接受20分钟功能性作业治疗（FOT）和10分钟不同类型的上肢运动（实验组进行主动辅助运动，对照组进行被动运动），每日总计30分钟，每周5天，持续4周。为确保干预的一致性，选择一名治疗师执行全部干预措施，并在研究前对该治疗师进行培训。</li>
	<li>功能性作业治疗（FOT）： 
	注：患者利用患侧手指的掌指关节和指间关节，在健侧手佩戴的手套驱动下完成手指抓握动作。
	<ol>
		<li>由治疗师对患侧上肢的肩、肘、腕、拇指及手指进行约1分钟的被动活动。</li>
		<li>被动活动结束后，指导患者使用健侧肢体和手带动患侧肢体和手完成推动泡沫滚筒、提起木钉、拾取小木棍、握持球体等动作。根据患者具体情况，每次训练选择两到三项活动。</li>
	</ol>
	</li>
	<li>辅助主动运动（AAM）
	<ol>
		<li>选择用于手部训练的康复训练设备。该设备旨在帮助患者完成被动或主动运动。选择智能镜像模式，将时间设置为10 min。询问患者的感受，并根据其体验和耐受程度选择1-10级强度。然后点击开始按钮。</li>
		<li>当健侧手进行自主抓握时，指导患者观察该动作，并尝试在手套辅助下用患侧手同步进行抓握（图2A）。</li>
		<li>当健侧手主动张开时，指导患者观察该动作，并尝试在手套辅助下张开患侧手指（图2B）。 
		注：当健侧手抓握时，健侧手套上的传感器无法检测到被遮挡的光信号，从而触发健侧手套抓握；当健侧手张开时，传感器检测到光信号，触发健侧手套张开。</li>
		<li>重复上述过程循环进行10分钟，设备将自动结束训练。</li>
	</ol>
	</li>
	<li>被动运动（PM）
	<ol>
		<li>让患者使用同一设备进行被动抓握和手部张开训练。将相应手套佩戴于患侧手上。选择被动模式，将时间设置为10 min，根据患者的感受将强度调节至1-10级，然后点击开始按钮。</li>
		<li>指导患者保持放松状态，在手套辅助下完成患侧手的闭合与张开动作（图2C）。持续训练10分钟，设备将自动结束训练。</li>
	</ol>
	</li>
</ol>
4. 评估
<ol>
	<li>由另一位未对分组情况设盲的治疗师进行临床评估。该治疗师应对每位患者进行两次评估：一次在干预前，另一次在为期4周的干预结束后立即进行。
	<ol>
		<li>收集患者的基本信息，包括年龄、性别和损伤类型。</li>
		<li>使用上肢Fugl-Meyer评估量表（FMA-UE）24在干预前后评估上肢运动功能。此外，使用FMA量表的手腕-手部组件（FMA-WH）评估患者的手部功能。</li>
		<li>使用改良Barthel指数（MBI）25评估患者执行日常活动的能力。</li>
		<li>使用功能性近红外光谱成像（fNIRS）监测被动运动任务期间初级运动区的激活情况。</li>
		<li>fNIRS数据采集
		<ol>
			<li>使用一种研究型近红外脑功能成像系统采集fNIRS数据。该系统采用三种波长的近红外光（780、805和830 nm）来监测氧合血红蛋白浓度变化（Δ[Oxy-Hb]）、脱氧血红蛋白浓度变化（Δ[Deoxy-Hb]）以及总血红蛋白浓度变化（Δ[Hb]）；其采样频率为13 Hz。</li>
			<li>根据国际10-20系统，在双侧初级运动皮层（M1）放置4个光源发射器和4个探测器，共形成20个通道。具体位置参见图3。</li>
		</ol>
		</li>
		<li>任务流程
		<ol>
			<li>按照模块化范式（静息[15 s]-任务[30 s]-静息[15 s]）连续进行5次fNIRS评估，具体步骤见4.1.6.2–4.1.6.9（参见图4）。</li>
			<li>打开fNIRS计算机界面，输入患者基本信息。然后选择探头排列方式2X4(R), 2X4(L)，选择15-30-15任务范式，并将评估次数设置为5次。</li>
			<li>根据探头布局将近红外系统设备佩戴于患者头部。调整发射器和探测器的位置，小心拨开头发以确保探头与头皮紧密接触。调整完成后，点击OK 按钮。</li>
			<li>进入系统的自动信号调节界面，点击Standby，并调整所有通道使其显示为绿色（信号良好）。</li>
			<li>将手套佩戴于患者的患侧手部，选择Passive Exercise模式。由于训练频率会随强度变化，测试期间为每位受试者选择平均强度，即5档。</li>
			<li>点击fNIRS计算机界面上的Start按钮，开始三阶段任务。首先测量15秒的初始静息期，从15秒倒计时至0秒。在此期间，指导患者安静地坐在椅子上，保持不动，尽量不要思考其他事情，使大脑处于放松状态。</li>
			<li>当倒计时至0秒时，点击手部设备上的Start 按钮，患者的患手将在手套辅助下开始被动抓握与张开运动。此时计算机开始30秒倒计时，即被动运动持续时间。当倒计时至0秒时，点击手部设备上的Stop 按钮以结束训练。抓握与张开的频率由设备设定；在30秒任务期间，完成3次抓握-张开循环，共重复3轮。</li>
			<li>如前所述，开始另一个15秒的静息期。该间隔结束后，第一次“静息-任务-静息”测试完成。</li>
			<li>重复上述“静息-任务-静息”测试共5次，随后结束fNIRS测试。</li>
		</ol>
		</li>
		<li>近红外数据分析：
		<ol>
			<li>本分析使用fNIRS系统中安装的数据分析软件，具体操作如下所述。</li>
			<li>剔除所有通道中因严重运动伪影导致的异常数据及缺失数据。 
			注：本方案实施过程中，实验组（EG）剔除1例异常值，对照组（CG）剔除1例异常值和2例缺失数据。</li>
			<li>剔除存在明显运动伪影的通道。</li>
			<li>分别对左右两侧通道（每侧10个通道）进行叠加平均处理。</li>
			<li>使用带通滤波器（0.01–0.08 Hz）去除信号中具有明显周期性波动的噪声成分，包括机械噪声和生理噪声。需消除的生理噪声类型包括心率（约1 Hz）、呼吸（约0.2–0.3 Hz）、Mayer波（约0.1 Hz）以及极低频生理波动（&#60;0.01 Hz）。</li>
			<li>将任务开始前后的15秒作为基线期，以一个完整周期（静息[15 s]-任务[30 s]-静息[15 s]）为测试单元，将五个周期叠加后取平均值。</li>
			<li>采用Savitzky-Golay方法进行平滑处理，设置平滑点数为5，平滑次数为126。</li>
			<li>预处理完成后，计算积分值和重心值。</li>
			<li>使用Shapiro-Wilk检验（Shapiro-Wilk, SW）检验两组在干预前后重心值、积分值及其差值的正态性；若所得P 值&#62;0.05，则认为数据服从正态分布。</li>
			<li>使用独立样本t检验比较两组在干预前后的数据差异；使用配对样本t检验比较两组内部干预前后的重心值和积分值差异。</li>

5. 统计学

	<li>使用 SPSS 进行统计分析。</li>
	<li>使用 SW 检验对数据的正态性进行检验。</li>
	<li>采用 Fisher 精确检验或独立样本 t 检验比较各组患者的一般资料。</li>
	<li>组间和组内行为学数据采用重复测量方差分析（ANOVA）进行比较，并以均值 ± 标准差表示。</li>

本研究通过近红外光谱技术，探讨了在不同运动模式下，功能性电刺激（FOT）结合上肢功能训练对风湿性心脏病（RHD）患者早期康复的影响。FOT可帮助患者被动活动僵硬的上肢，为后续训练奠定基础。其关键在于由健侧手引导患侧手完成有目的、有意义且具实用性的功能任务，尽可能使用日常生活中的实物，并模拟真实场景28。这种方法能够激发患者治疗的积极性，并最大限度地促进患者的主动运动。AAM的核心要点是患者的运动由健侧肢体和手驱动，同时患侧肢体和手进行自发性的主动尝试，这是其与被动运动最重要的区别。康复设备为患者提供实时的视觉和触觉反馈，在康复训练中实现了中枢神经系统与外周之间的闭环交互29。
康复任务的训练不涉及复杂技术，但在使用功能性近红外光谱（fNIRS）评估患者时需注意诸多事项。为确保获得良好的fNIRS信号并防止运动伪影干扰测试结果，我们通常在受试者前方的桌面上放置头部固定架。调节桌面高度，使受试者的下巴可舒适地 resting 在头部固定架上，以减少运动过程中的头部晃动。此外，头皮上的油脂会影响光学信号；因此，实验前我们使用吸油纸擦拭患者头部，以保证信号质量。根据以往经验，我们还发现降低自然光和噪声的影响有助于提升fNIRS信号的采集效果；因此，所有数据均在黑暗且安静的环境中采集30。
既往研究表明，镜像疗法（MT）可有效改善卒中后手指的灵活性31，尤其适用于亚急性期患者的上肢康复32，因此在恢复运动功能及提高大脑半球损伤后日常生活活动能力方面具有广阔前景33,34,35,36。当患者移动未受影响的上肢时，镜面形成的视觉错觉被患者感知为患侧手的运动，从而增强视觉和躯体感觉皮层区域的活动，提高患者的注意力，并降低单侧忽略的发生可能性37,38。通过这种方式，患者可有意识地更频繁地使用患侧肢体39。在传统镜像疗法的基础上，本研究通过AAM设备直接对患侧肢体提供本体感觉刺激和视觉反馈，减少了因患手本体感觉与视觉信息不同步所引起的不适感40，因而较传统镜像疗法展现出更广泛的治疗潜力。我们的训练设备操作简便，安全性高，可通过点击关闭按钮立即终止训练，以避免测试过程中可能出现的紧急情况。此外，一些研究已证实，镜像疗法可通过调节M1区的兴奋性，促进卒中后大脑半球间平衡的正常化。在后续研究中，我们将采用功能性近红外光谱技术（fNIRS）评估大脑皮层的静息态功能连接，进一步验证右侧半球损伤（RHD）患者治疗后的大脑半球变化41。
本研究存在若干局限性。首先，近红外光谱检测所选用的任务范式为被动式，而大脑激活在主动运动中可能更为显著。因此，主动尝试的任务范式可能比被动运动更为合适。其次，本研究仅监测了初级运动皮层（M1）区域，但镜像疗法（MT）还会增强与注意力分配和认知控制相关脑区的神经活动，通过加强运动控制中的认知作用来促进运动功能的恢复42；因此，监测前额叶的血流动力学变化也可能十分必要。此外，由于住院患者治疗方案较多，每日仅进行了10分钟的手功能康复训练。未来应延长训练时间，以更充分地探索康复效果。还需开展随访研究，以观察此类训练的长期效应。今后有望通过大样本、多中心的研究，为早期RHD患者提供最适宜的康复策略。

大脑半球损伤可导致对侧肢体的感觉和运动功能障碍1,2,3，不同程度地影响患者的运动控制能力、活动能力和功能性学习4，从而给家庭和社会带来沉重负担5。对于右侧大脑半球损伤（RHD）患者而言，其恢复速度尚不理想。然而，在大多数RHD病例中，受影响的左侧肢体作为身体的非优势侧，往往未能得到患者及照护者的充分关注。鉴于上肢和手部功能障碍会严重影响日常生活活动能力和生活质量，亟需一种更有效的方法来改善RHD患者上肢功能的康复效果6,7,8,9,10。
运动疗法是帮助患者恢复肢体功能的重要方法。对于脑损伤患者的早期康复，通常采用被动运动（PM）和辅助主动运动（AAM）训练方法。AAM 是指患者通过自身肌力与外部辅助相结合，完成特定关节的活动11。其关键在于患者主动参与辅助性康复过程。人脑激活的准备状态有助于在运动控制循环中刺激和整合运动系统。许多研究表明，AAM 可诱导神经可塑性改变，从而促进患者功能恢复12,13。
功能性近红外光谱成像（fNIRS）是一种基于光学原理的成像技术。根据光在组织中衰减程度与不同浓度的光吸收物质之间的相关性，fNIRS能够定量分析脑组织中氧合血红蛋白和脱氧血红蛋白浓度的变化，从而监测大脑皮层的功能活动14。大量研究表明，fNIRS是监测大脑半球损伤后脑氧合状态及能量代谢的重要手段15,16,17。因此，fNIRS可能是研究大脑半球损伤后与上肢运动功能恢复相关的皮层变化的一种适宜监测方法。
不同感觉输入方式所产生的运动信号以及感觉皮层的调节状态存在差异18，19。被动运动与主动运动所产生的感觉刺激，与感知稳定性以及构建对自身所处环境精确表征的能力密切相关，进而指导个体的行为20。本研究旨在通过分析功能性近红外光谱（fNIRS）数据，探讨不同运动模式对中国台湾脑半球损伤患者早期上肢康复及脑激活的影响，为未来制定患者全面康复的科学策略提供依据。
本研究旨在探讨功能性电刺激联合不同类型的运动对右侧偏瘫患者上肢功能及大脑重塑的影响。我们假设，与功能性电刺激联合被动运动相比，功能性电刺激联合主动辅助运动在改善右侧偏瘫患者上肢功能和脑激活方面更为有效。

基于功能性近红外光谱技术探究不同类型运动对右半球损伤患者上肢功能恢复的影响

Investigating the Effect of Different Types of Exercise on Upper Limb Functional Recovery in Patients with Right Hemisphere Damage Based on fNIRS

通过分析功能性近红外光谱成像（fNIRS），研究功能性作业疗法（FOT）联合不同类型运动对右半球脑损伤（RHD）患者上肢运动功能恢复及脑功能重塑的影响。本研究纳入北京博爱医院的32例RHD患者，随机分为两组：FOT联合被动运动组（n=16）和FOT联合辅助主动运动组（n=16）。被动运动组（FOT-PM）每次治疗接受20分钟功能性作业疗法和10分钟被动运动训练，辅助主动运动组（FOT-AAM）每次接受20分钟功能性作业疗法和10分钟辅助主动运动训练。两组均同时接受常规药物治疗及其他康复治疗。治疗频率为每日1次，每周5次，持续4周。治疗前后采用Fugl-Meyer上肢运动功能评估量表（FMA-UE）和改良Barthel指数（MBI）评估患者运动功能及日常生活活动能力（ADL）的恢复情况，并通过fNIRS分析双侧运动区的脑激活变化。研究结果表明，与FOT联合被动运动相比，FOT联合辅助主动运动在改善RHD患者上肢及手指运动功能、提升日常生活活动能力以及促进运动区脑功能重塑方面更具优势。

基线 
2021年10月至2023年6月期间，我们共招募了35名患者，其中32名最终完成了研究；试验期间未发生任何不良事件。
关于两组患者的临床症状（表1），实验组和对照组的平均年龄分别为53.19 ± 10.72岁和55.88 ± 12.32岁（P = 0.515）。两组在性别、疾病类型、上肢Fugl-Meyer评分（FMA-UL）或改良Barthel指数（MBI）评分方面均无显著差异（P > 0.05）。干预前，两组所有患者的腕手Fugl-Meyer评分（FMA-WH）均为0分。
FMA-UL 具有重要的临床意义，可有效且可靠地评估脑损伤患者上肢功能的受累情况。FMA-UL 共包含 33 项上肢评估项目，每项单向评分标准为：完全完成得 2 分，部分完成得 1 分，未完成得 0 分。上肢运动功能总分为 66 分。作为 FMA-UL 的一个子类别，腕-手量表（FMA-WH）包含 12 个项目，总分最高为 24 分。
重复测量方差分析结果显示，组别对FMA-UL评分的主效应显著，F = 5.564，p = 0.030，ɳ2p = 0.214；时间的主效应显著，F = 34.716，p &lt; 0.001，ɳ2p = 0.831；组别与时间的交互效应显著，F = 5.554，p = 0.030，ɳ2p = 0.256。（表2）
组间对 FMA-WH 评分的主效应显著，F = 8.817，p = 0.006，ɳ2p = 0.227；时间主效应显著，F = 13.357，p = 0.001，ɳ2p = 0.308；时间和组间的交互效应显著，F = 8.817，p = 0.006，ɳ2p = 0.227。（表 2）。
改良巴氏指数被广泛用于评估个体执行日常活动的能力，可衡量一个人完成十项基本日常活动的能力。巴氏指数的总分为100分，分数越高，表明患者进行日常生活活动的能力越强。
组别对 MBI 评分的主效应显著，F = 8.512，p = 0.007，ɳ2p = 0.221；时间的主效应显著，F = 588.559，p &lt; 0.001，ɳ2p = 0.952；组别与时间的交互效应显著，F = 10.425，p = 0.003，ɳ2p = 0.258。（表 2）。
积分值是指任务执行期间血氧信号的积分，反映了任务过程中血流动力学反应的幅度。重心值是指在整个任务期间血氧信号变化区域中心的垂直线所对应的时间（s），是反映任务全程时间进程变化的指标，代表了血流动力学反应的速度27。
干预前，两组的积分值或质心值之间均无显著差异（图5A，P &gt; 0.05）。干预后，对照组（CG）受试者右侧半球的积分值为0.20 ± 0.32，实验组（EG）受试者右侧半球的积分值为-0.06 ± 0.24，两组总体均值之间存在显著差异（t=-2.489，d=0.92，P = 0.020，P &lt; 0.025视为具有统计学意义）（表3）。干预后，对照组受试者左侧半球的积分值为0.18 ± 0.32，实验组受试者左侧半球的积分值为-0.04 ± 0.26，两组总体均值之间无显著差异（t=-1.975，P=0.059，d=0.75）。干预后，两组之间的质心值无显著差异（P &gt; 0.025）（图5B）。

我们正在致力于解决脊髓损伤患者的上肢康复问题，并研究了辅助主动训练在右半球损伤患者的功能性作业治疗中的有效性。目前，我们的研究采用了头部康复机器人和近红外功能脑成像技术。迄今为止遇到的挑战包括患者在进行fNIRS检测时需保持头部稳定，以及需要仔细调节光学探头以确保通道信号质量良好。我们的研究结果表明，在未来的康复训练中，应充分调动患者的积极性，并辅助其进行主动康复训练，而非单纯的被动训练，因为这在脑功能重塑的康复训练中更为有效。首先，确定受试者和对照组。对患者患侧肢体的肩部、肘部、腕部、拇指及手指各关节进行被动活动，持续一分钟。然后指导患者使用健侧手带动患侧手推动泡沫轴、提起木钉、举起小木棒并握住球。进行辅助主动运动时，选择智能镜像模式，并在康复训练设备上将时间设置为10分钟。询问患者的感受，并根据患者的体验和耐受程度选择1至10级之间的相应级别。然后点击开始按钮。当患者用未受影响的手主动执行抓握动作时，指导其观察该动作，并尝试使用手套带动受影响的手进行抓握。同样，指导患者观察未受影响手的手指运动，并尝试张开受影响手的手指。对于被动运动，将相应的手套戴在患侧手上。使用同一设备，选择被动模式，设置时间为10分钟，并根据患者的感受调节强度水平。启动设备后，让患者患侧手进行10分钟的握紧和张开动作。首先，为患者组和对照组提供功能性作业治疗。进行功能性近红外光谱成像（fNIRS）时，打开计算机界面，输入受试者的基本信息。然后选择光源探测器排列方式为“2×4R 2×4L”，接着选择“15-30-15”任务范式，并将评估时间设置为5分钟。根据光极布局将近红外系统设备放置在患者头部，并调整发射器和探测器的位置。在移除头发以确保与头皮紧密接触后，点击“确定”按钮。接着，依次点击待机和自动调节开始，以校准所有通道并显示为绿色。然后将手套戴在患者患侧手上。在 fNIRS 计算机界面上点击开始按钮后，让患者安静就座并避免外界干扰，测量其初始静息状态 15 秒。随后点击手部设备的开始按钮，30 秒后点击停止按钮以结束该次训练。为完成测试，再次开始15秒的休息期。最后，重复静息任务测试五次以完成fNIRS检测。干预后，fNIRS数据显示，受试者右侧半球积分值的总体平均值在对照组与实验组之间存在显著差异，而两组之间的重心值未观察到显著差异。
