本研究中的动物实验程序已通过四川大学华西口腔医学院伦理委员会（WCHSIRB-D-2017-041）的审查和批准。本研究使用成年C57BL/6小鼠（见材料表）。该方案对右侧上颌第一磨牙（M1）施加机械力以实现近中移动，从而通过骨吸收与骨形成之间的耦合作用，诱导高度协调的骨重建过程（图1）。
1. 术前准备
<ol>
	<li>手术物品
	<ol>
		<li>准备以下手术物品用于手术：手术平台（图2A）、固定器（图2B）、手术器械（图2C 和 补充图S1）、正畸用品（图2C）以及牙体修复用品（图2D）。 
		注：定制的螺旋弹簧为特制，拉伸至10 mm时可提供10 cN的力。</li>
	</ol>
	</li>
	<li>灭菌
	<ol>
		<li>通过高压蒸汽灭菌法对手术器械进行灭菌，并对所有其他手术物品进行至少30分钟的紫外线照射灭菌。</li>
	</ol>
	</li>
	<li>麻醉
	<ol>
		<li>通过腹腔注射给予小鼠氯胺酮（100 mg/kg）和地西泮（5 mg/kg）进行麻醉。</li>
		<li>使用棉签将兽用眼膏涂抹于小鼠眼部，以防角膜干燥。</li>
		<li>仅当用镊子夹捏小鼠脚趾无反应时，方可开始手术。</li>

2. 手术过程
<ol>
	<li>用胶带将麻醉小鼠仰卧位固定在手术平台上，并展开四肢并用胶带固定。</li>
	<li>在头部两侧各固定一根27 G针头，腋窝下方两侧也各固定一根27 G针头。</li>
	<li>将一根橡皮筋缠绕在上述两根针头和上切牙周围，另一根橡皮筋缠绕在另外两根&#160;针头和下切牙周围。调整针头位置以控制口腔张开程度和方向（图3A）。 
	注意：进行正畸牙齿移动操作时，在颊肌完全绷紧前应使口腔尽可能充分张开。舌头应拉向非手术侧，以暴露手术视野并防止缺血。</li>
	<li>将一根3 cm长、304不锈钢丝的1.5 mm端弯曲，使用弯头眼科镊子从颊侧将弯曲端穿过第一磨牙（M1）与上颌第二磨牙（M2）之间的邻间隙。当从腭侧看到结扎丝的腭侧末端时，将其拉出约一半长度，并穿过定制螺旋弹簧的一端（图3B）。</li>
	<li>将结扎丝的两端在上颌M1的近中方向打方结，直至弹簧牢固固定在牙齿上（图3C）。剪除多余钢丝。</li>
	<li>同样，将第二根3 cm长、304不锈钢丝穿过螺旋弹簧的另一端。</li>
	<li>用棉球清洁并干燥切牙表面，使用棉签在所有处理表面上涂抹粘接剂，并进行光固化。</li>
	<li>将第二根不锈钢丝穿过上颌切牙之间的邻间隙，在唇侧打滑结（图3D）。剪除多余钢丝，使剩余钢丝紧贴牙面。</li>
	<li>注射光固化树脂覆盖结扎结和切牙，并进行光固化（图3E）。</li>
</ol>
3. 术后管理
<ol>
	<li>术后，对小鼠腹腔注射 0.05 mg/kg 的丁丙诺啡，以进行术后镇痛&#160;。</li>
	<li>将麻醉后的小鼠置于 37 °C 恒温电热毯上。当小鼠恢复意识并能自主行走时，将其放回独立的饲养笼中。</li>
	<li>由于术后门齿功能受限，需将常规硬质饲料替换为仅提供软质饮食。</li>
	<li>每天检查正畸装置。若在检查过程中发现任何影响正畸力传导的情况，例如弹簧变形、弹簧松动或装置脱落，应将该小鼠排除出实验。</li>
	<li>为保持实验的可比性，需在术后每天评估小鼠体重。任何体重下降超过术前体重 30% 的小鼠必须从实验中剔除。</li>

本文旨在逐步描述在小鼠上颌模型上进行最简单的正畸牙齿移动实验方案，以研究机械负荷诱导骨重建的潜在机制。除了骨重建研究外，该方法还有一些其他主流应用：1）加速正畸牙齿移动的方法学研究；2）正畸性牙根吸收的研究；3）正畸牙齿移动及疼痛的生物学机制研究；4）转基因模型的相关研究。
与其他机械负荷相关治疗方法（如下颌骨牵张成骨术37）相比，正畸牙齿移动是一种无需伤口和出血的最简单、最温和的方法。此外，小鼠模型具有操作简便、耗时少、成本低等优势38。上颌模型可在手术过程中提供广阔的视野和稳定的固定，并在术后最大程度减少舌头对矫治装置的干扰14。
根据本研究建立的模型，我们进一步描述了三个具有代表性的时间点。从术后第3天起，牙齿移动即可被宏观测量，且移动距离随时间逐渐增加。术后第3天，通过牙周膜纤维将机械负荷传递至骨组织，但骨组织尚未发生明显变化。术后第8天，骨重建过程已经启动，以骨吸收为主；而术...

骨是一种高度活跃且不断重建的组织，其大小、形状和特性会随着个体的生命周期而不断调整1,2。除了激素、衰老、营养以及其他生物学或生物化学因素3之外，机械负荷是决定骨结构的最主要因素这一观点已获得广泛认可4,5。在某些异常机械负荷的情况下，骨吸收与骨形成之间的失衡可能导致异常的骨重建及骨相关疾病。诸如失用性骨质疏松、长期卧床导致的骨量丢失，以及航天飞行中微重力环境引起的骨丢失等骨病，均与异常机械负荷密切相关6,7,8。
机械负荷也被用于治疗与骨骼相关的疾病，例如牵张成骨治疗和正畸治疗。牵张成骨治疗已用于颅缝早闭和下颌骨发育不全等发育性疾病9,10，而正畸治疗则广泛用于纠正牙齿位置异常及各类咬合不正11。正畸治疗的核心同样是机械负荷的调控。当骨组织受到机械负荷作用时，会诱发高度协调的骨重建过程，该过程通过先骨吸收、后骨形成的耦合机制，实现牙齿的移动，从而达到正畸目的12,13。
尽管正畸治疗已在临床实践中广泛应用，但由于我们对机械力生物学效应的认识有限，正畸治疗的效果仍难以控制。为克服这些局限性，已建立多种动物模型（如小鼠、大鼠、兔、猫、犬、猴和猪）用于研究机械力诱导骨重建的潜在机制（表1）14,15,16,17,18,19,20,21,22,23,24,25,26,27,28,29,30,31,32。大型动物（如犬、猴和猪）在正畸操作方面相较于小型动物具有一些优势——它们的牙齿和牙列更接近人类，因此手术操作更容易在人体上复制。此外，较宽的术野可降低操作难度，并允许应用多种正畸方案33,34。然而，大型动物获取困难，导致样本量受限，并面临伦理限制35。此外，常规的拔牙操作和复杂的器械使得实验实施困难，因此大型动物在研究中很少被使用。
在这些情况下，啮齿类动物主要用于建立正畸模型。在这些模型中，大鼠和兔子相比小鼠具有较低的操作难度和更多的牙齿移动方案。然而，小鼠模型具有独特的优势，即可利用的基因修饰小鼠品系数量众多，这对于研究潜在机制尤为重要36。然而，由于体型较小，小鼠模型的操作难度最大。回顾现有方法，将第一磨牙向近中方向移动是建立正畸模型唯一可行的方法。目前主要使用两种装置来移动牙齿：螺旋弹簧和弹性橡皮圈。使用弹性橡皮圈操作较为简便，但正畸力变化较大，难以获得稳定的结果。
Xu 等人15 建立了一种在下颌骨上使用螺旋弹簧的小鼠模型。然而，由于下颌骨具有活动性且舌体存在阻碍，无论从术中还是术后考虑，上颌始终是首选的实验部位。Taddei 等人16 在10年前描述了一种更为详细的小鼠上颌实验方案，但仍需补充更多直观且清晰的操作细节。综上所述，本实验方案系统地阐述了在小鼠上颌模型中进行正畸牙齿移动的详细操作流程，旨在帮助研究人员以标准化的方式掌握建模方法，并促进不同研究之间的比较与评估。

小鼠上颌正畸模型的建立

The Establishment of a Murine Maxillary Orthodontic Model

由于目前缺乏可重复的小鼠上颌正畸模型建立方案，本文提供了一种可靠且可重复的实验流程，为研究人员分析力学加载相关的骨重建过程提供了可行工具。本研究除提供详细的流程图外，还包含了多种示意图、手术操作照片及视频资料。我们已在11只成年野生型C57/B6J小鼠上实施该方案，并于术后第3、8、14天采集样本。显微CT与组织病理学数据已证实，采用本方案可成功实现牙齿移动并伴随骨重建。此外，根据术后第3、8、14天的显微CT结果，我们将骨改建过程划分为三个阶段：准备阶段、骨吸收阶段和骨形成阶段。该分期有助于关注不同阶段的研究人员合理设定样本采集时间点。本方案可为研究人员开展骨重建的再生机制分析提供有力工具。

我们对11只成年雄性小鼠（C57/BL6，3月龄）进行了OTM手术，并在术后第3、8和14天处死动物以获取结果。在这些实验中，右侧上颌为手术侧，左侧上颌为对照侧。显微CT显示，M1与M2之间的距离在术后呈现时间依赖性连续增加：术后第3、8和14天分别达到30 µm、70 µm和110 µm（图4）。由于机械负荷的作用，牙周膜在根部远中侧出现低密度区增宽，而在近中侧则变窄（图5）。此外，牙周膜保持连续，且任何牙根均未发生吸收。这些结果证明，采用本方案对M1进行物理移动是可行且安全的。
