本方案遵循《赫尔辛基宣言》进行。已获得成都中医药大学附属医院呼吸内科批准委员会的批准，批准编号为 KY2022009。已获取知情同意。
1. 患者适应症筛查
<ol>
	<li>纳入标准
	<ol>
		<li>确保年龄在50至65岁之间。确保患者符合稳定期慢性阻塞性肺疾病（COPD）及肺肾气虚的诊断。确保患者自愿参加并能够遵守研究要求，且已签署知情同意书。</li>
		<li>稳定期COPD的诊断参照《慢性阻塞性肺疾病诊断、管理和预防全球策略2022年报告》15。确保患者改良版英国医学研究委员会呼吸困难量表（mMRC）评分 ≤ 2；气流受限程度为轻度或中度，即1秒用力呼气容积（FEV1）≥ 预测值的50%。</li>
		<li>肺肾气虚型喘息性COPD的诊断参照《慢性阻塞性肺疾病中医诊疗指南》16中的相应标准。主要症状包括：1）喘息、气短，活动后加重；2）乏力或自汗，活动后加重；3）易感风寒；4）腰膝酸软无力；5）耳鸣、头晕或眼前发黑；6）小便频数、夜尿增多，或咳嗽时遗尿；7）舌淡苔白，脉沉或弱。若1）、2）、3）中具备两项，且4）、5）、6）、7）中具备两项，即可诊断为COPD。</li>
	</ol>
	</li>
	<li>排除标准
	<ol>
		<li>排除患有严重影响呼吸功能或生活质量的疾病者，如急性心肌梗死、脑卒中、阿尔茨海默病及精神疾病患者。</li>
		<li>排除存在严重认知或运动功能障碍且不配合者。</li>
	</ol>
	</li>
</ol>
2. 材料准备
<ol>
	<li>检查将用于消毒的设备：75% 乙醇棉球、无菌镊子和医用刀片。</li>
	<li>提前准备药用种子。筛选呈球形的王不留行（Vaccaria segetalis）种子，获得直径约为 2 mm 的均匀种子，如图 1A所示。</li>
	<li>准备医用胶带和耳穴压豆贴敷模板。使用宽 5 cm 的低致敏性超薄纸质胶带，如图 1B所示。准备一张 5 cm × 8.5 cm 的白色耳穴压豆贴敷模板，如图 1B所示。 
	注意：对普通胶带过敏者，应使用脱敏胶带。注意避免胶带沾水，以防皮肤感染。</li>
</ol>
3. 治疗准备
<ol>
	<li>保持环境清洁卫生，避免污染。</li>
	<li>要求操作者用消毒剂洗手，然后用含75%乙醇的棉球擦拭手指。</li>
	<li>将王不留行（Vaccaria segetalis）种子置于耳穴贴压模板上，用医用胶带固定，如图1C所示。严格消毒，防止感染。</li>
</ol>
3. 治疗步骤
<ol>
	<li>体位：嘱患者取坐位。</li>
	<li>消毒与定位：操作者固定患者耳廓，保持患者坐位姿势。用一只手固定患者头部，用75%乙醇棉球进行一次性的皮肤消毒。</li>
	<li>取穴：参照2008年全国标准化技术委员会发布的国家标准《耳穴名称与定位》17。根据图1D和Table 1所示位置，确定耳穴肺（CO14）、脾（CO13）、肾（CO10）、神门（TF4）和平喘（AT1.2.4i）的位置。</li>
	<li>贴压：取已备好的耳穴压豆贴片，将其粘贴于相应的耳穴位置。每个耳穴使用一个贴片。</li>
	<li>按压：贴好贴片后，进行按压操作。用拇指和食指分别置于耳廓两侧，对耳穴上的药豆贴片施加相对压力，使患者感到轻微疼痛为宜，但不应影响睡眠。 
	注意：按压力度依据视觉模拟评分法（VAS）18的疼痛程度确定：以1至3分范围内的中等疼痛作为按压力度的标准。</li>
</ol>
5. 治疗过程
<ol>
	<li>每日按压耳部穴位3次，分别于上午9:00、下午3:00和晚上9:00进行。按压力度控制在VAS疼痛评分1-3之间。每3天轮换一次按压部位，双耳交替进行，持续12天。</li>
	<li>每天检查贴片情况，若贴片潮湿，或耳腔出现皮肤损伤、冻伤或炎症，应立即更换。如无上述情况，则继续保留贴片。贴片施加的是埋针压力或中等压力，需嘱咐患者不要揉搓贴片，以防因感染导致皮肤污染。确保耳部贴片准确贴敷于耳穴位置，且不影响患者的睡眠。</li>
</ol>
6. 疗效评估
注意：疗效需结合客观与主观指标共同评估。对于量表填写等主观指标，患者必须在无任何提示性提醒的情况下独立完成评分。
<ol>
	<li>指导患者进行肺功能指标检测，包括第一秒用力呼气容积（FEV1）、用力肺活量（FVC）及FEV1/FVC。治疗后上述指标升高提示治疗有效。</li>
	<li>采用中医症状评分量表评估疗效19。治疗前后评分变化超过2分，表明治疗具有临床意义。</li>
	<li>采用改良版英国医学研究委员会呼吸困难量表（mMRC）评估患者生活质量。评分0-1分表示轻度呼吸困难，2-4分表示重度呼吸困难。治疗前后mMRC评分变化超过1分，表明治疗具有临床意义。</li>

现代医学主要使用长效β2受体激动剂（LABAs）、长效毒蕈碱拮抗剂（LAMAs）和ICS23治疗慢性阻塞性肺疾病（COPD）患者。中医根据咳嗽、咳痰、气促和呼吸困难等临床特征，将COPD归为“喘证”范畴24。同时，中医认为十二经脉均与耳部相连，耳穴与经络、组织和器官相互关联，与机体的生理功能和病理变化密切相关，因此耳穴疗法可有效防治多种疾病25。程等人26通过观察耳穴贴压豆疗法对老年慢性阻塞性肺疾病急性加重期（AECOPD）患者症状及肺功能的影响发现，采用常规治疗联合耳穴贴压豆疗法的观察组，其FEV1%和FEV1/FVC%显著高于仅接受常规治疗的对照组。刘等人9通过观察COPD患者接受耳穴贴压后FEV1、FEV1%、FVC及FEV1/FVC数值的变化，发现耳穴贴压豆疗法显著提高了临床疗效和肺功能水平。张等人27亦通过临床试验证实，耳穴贴压豆疗法对COPD具有更好的治疗效果，可改善患者的肺功能，增强活动耐力，并提高其生活质量。
本方案中所选取的五个耳穴，包括肺（CO14）、脾（CO13）、肾（CO10）、神门（TF4）和平喘（AT1.2.4i），是参考临床研究报告确定的28,29。研究表明，上述五个耳穴是治疗肺肾气虚型慢性阻塞性肺疾病（COPD）的核心耳穴28,29,30。根据中医理论，肺、脾、肾、神门及平喘等耳穴具有健脾补肾、降气平喘的作用8。因此，可改善COPD患者的喘息症状及肺功能。
该疗法作为稳定期慢性阻塞性肺疾病（COPD）且伴有肺肾气虚患者的辅助治疗，具有成本低、操作简便的优点31。此外，该操作属于非侵入性治疗，相对安全，感染风险较低32。因此，该治疗方式为改善COPD患者的症状提供了新的治疗方法和思路，可有效降低急性加重的发生率，改善患者的生活质量及预后33。
以下两个关键步骤需要特别注意。首先是耳穴定位。肺（CO14）、脾（CO13）、肾（CO10）、神门（TF4）和平喘（AT1.2.4i）等耳穴的定位必须严格依据耳穴诊断与治疗标准进行。第二个关键是按压的强度和频率。按压时应对准治疗点，用食指和拇指共同施力，按压耳廓的前后两面。压力强度以使耳廓产生温热感和轻微疼痛但尚可耐受为宜。按压频率为每日3次，分别在上午9:00、下午3:00和晚上9:00进行，每次持续3分钟。每3天轮换一次耳朵。12天为一个疗程。坚持上述耳压治疗方案可获得稳定的长期效果8。
治疗后，FEV1 和 FVC 等肺功能指标有所升高。与治疗前相比，患者的咳嗽、咳痰和喘息等症状评分得到改善。然而，MRC 分级未见变化，均为 2 级。耳压豆在一定程度上可缓解稳定期慢性阻塞性肺疾病（COPD）患者的喘息症状20，但对 mMRC 分级无改善作用。这可能与 COPD 不完全可逆的病理改变以及耳穴压豆治疗持续时间不足有关。
该方案的实施存在一些局限性。耳穴贴压时间较长，贴敷部位皮肤容易发生过敏反应，耳部皮肤可能出现持续性红肿、皮疹和瘙痒。因此，操作者应尽量选用抗过敏医用胶带，贴敷时间不宜过长，应每3天更换一次。若中途出现污染，应立即更换新的贴有压豆的胶带。注意，耳部有皮肤损伤或耳软骨相关疾病者不宜进行此项操作。此外，本疗法仅适用于病情稳定且处于缓解期的患者；若处于急性加重期或病情较重，应与其他治疗药物联合使用。

慢性阻塞性肺疾病（COPD）是一种常见、可预防且可治疗的慢性呼吸系统疾病。该疾病以持续性呼吸系统症状和气流受限为特征，通常与因长期暴露于有害颗粒物或气体所引起的气道和/或肺泡异常有关1。2016年，全球有超过300万人死于COPD，占全球死亡总数的6%2。COPD带来的医疗费用正日益成为全球的沉重负担3。目前，COPD已成为一项重大的公共卫生问题，具有高死亡率、高发病率、高致残率和高疾病负担的特点4。
慢性阻塞性肺疾病（COPD）的主要症状包括慢性咳嗽、咳痰、呼吸困难、喘息和胸闷。根据症状和肺功能情况，该病可分为急性加重期和稳定期5。在稳定期进行积极治疗，可有效减少急性加重次数，并延缓肺功能恶化。目前，稳定期的治疗主要依赖支气管扩张剂、糖皮质激素和祛痰药等药物治疗。非药物治疗手段仍存在局限性6。本实验提供一种特殊的中医药疗法——耳穴压豆，以拓展治疗方式。
耳穴按压疗法中，将小型圆形颗粒物贴敷于耳部穴位表面，通过适度揉捏、按压、掐压和挤压等手法，产生酸、麻、胀、痛等刺激感。作为耳压疗法的主要材料，Vaccaria segetalis 的种子可刺激耳部穴位，用于疾病的治疗7。这是一种无药物的外治法，用于疾病干预8。该疗法操作简便、成本低廉、安全性高、无不良反应，且不易引发感染，在改善临床症状、提高生活质量及减少急性加重方面具有一定优势，适用于稳定期慢性阻塞性肺疾病（COPD）患者的辅助治疗9。
关于耳穴按压治疗喘证的作用机制，中医认为，按压耳廓上的特定穴位可以沟通气血、经络以及内外脏腑，调和内外脏器功能，从而发挥调理气机、止咳平喘的作用10。现代医学研究表明，耳穴按压可刺激耳部的血管、神经和淋巴管11，通过激活胆碱能抗炎信号通路，调节免疫系统，抑制炎症反应，进而减轻支气管炎症，缓解喘息症状。全息生物学理论认为，耳廓与人体信息呈异速对应关系，根据这种对应关系，耳穴按压与喘证症状之间可能存在关联12。
根据相关研究，肺肾气虚证在稳定期慢性阻塞性肺疾病（COPD）中所占比例为23.20%13。此类患者常表现为慢性咳嗽伴喘息，提示肺脏功能不足；同时伴有畏寒、腰膝酸软及自汗等症状，提示肾脏功能亏虚14。本实验对肺肾气虚型COPD患者实施耳穴压豆治疗。肺肾气虚证喘息的辨证依据参考了中国针灸学会耳穴诊治专业委员会发布的2020年版标准8。本实验所选用的耳穴处方包括：肺（CO14）、脾（CO13）、肾（CO10）、神门（TF4）和平喘（AT1.2.4i）。
本研究旨在为伴有喘息及肺肾气虚的稳定期慢性阻塞性肺疾病（COPD）患者提供一种非药物治疗方式——耳穴压豆。在筛选出处于COPD稳定期的患者后，实施耳穴压豆治疗。该治疗方案在临床上操作简便、安全且具有成本效益。
病例介绍： 
患者为一名来自中国四川的64岁男性。近5年来反复出现咳嗽、咳痰，近一个月来胸闷症状加重。患者患有糖尿病5年，一直服用二甲双胍控制血糖，但未定期监测血糖水平。此外，患者有7年高血压病史，口服苯磺酸氨氯地平控制血压，亦未定期监测。患者否认有手术史、外伤史及输血史。吸烟史30年，平均每日2包，10年前已戒烟。患者否认有家族遗传病史。
诊断、评估与治疗方案： 
医生对患者进行专科检查后，发现其呈桶状胸，肋间隙增宽。胸部CT显示慢性支气管炎、肺气肿及冠状动脉钙化。肺功能检查提示患者存在中度阻塞性通气功能障碍，大气道气流轻度受限，总气道阻力增高。结合患者的症状、体征、辅助检查及既往病史，诊断为慢性阻塞性肺疾病、高血压及糖尿病。治疗上给予布地奈德、格隆溴铵和富马酸福莫特罗吸入剂，每日早晚各1次15；另予静脉滴注羟丙茶碱（2 mL: 0.25 g，将1支用250 mL 5%葡萄糖水稀释），每日1次16；并给予耳穴压豆以缓解喘息症状。

耳穴按压作为稳定期慢性阻塞性肺疾病喘息症状的辅助治疗

Auricular Acupressure as an Adjuvant Treatment for Wheezing in Stable Chronic Obstructive Pulmonary Disease

慢性阻塞性肺疾病（COPD）是一种重要的公共卫生问题。由于呼气气流受限，其特征为肺组织的肺气肿性破坏。呼吸困难是主要的临床症状之一。耳穴压豆是中医特有的临床疗法，通过按压耳部穴位来治疗疾病。通常使用王不留行籽等种子 Vaccaria segetalis 用于刺激耳穴，具有调节气机、缓解喘息的作用。本文提出将这一中医特色疗法应用于稳定期慢性阻塞性肺疾病（COPD）患者肺肾气虚型喘息的临床症状治疗。选取的耳穴治疗方案包括肺（CO14）、脾（CO13）、肾（CO10）、神门（TF4）和平喘（AT1.2.4i）穴。本方案描述了一例应用耳穴贴压治疗慢性阻塞性肺疾病患者以缓解喘息症状的病例研究。

上述方案描述了一项对照试验，旨在研究耳穴按压对改善稳定期慢性阻塞性肺疾病（COPD）患者喘息症状的疗效。本研究纳入并完成试验的患者共1例，为男性，64岁，患有慢性阻塞性肺疾病5年以上。长期规律使用吸入制剂治疗慢性咳嗽和喘息症状。
患者因咳嗽、咳痰、呼吸困难及胸闷入院。入院后完成了肺功能、胸部CT及其他相关检查。患者肺功能FEV1/FVC <0.7，符合慢性阻塞性肺疾病（COPD）的诊断。治疗方案包括常规西医吸入制剂与中医耳穴压豆疗法。在图2中，结果显示患者FEV1由1.51 L提升至1.63 L，FVC由2.48 L提升至2.76 L，PEF由3.95 L/s提升至5.94 L/s，VC由2.48 L提升至2.8 L。同时，表2中的症状评分由18分降至13分，表3显示治疗前后mMRC评分为2级。总体而言，该患者经治疗后肺功能指标有轻度改善，中医证候评分降低超过2分，mMRC评分未发生变化。这表明上述治疗方法可改善稳定期COPD患者的喘息症状。同时，既往临床研究亦表明，耳穴压豆对COPD患者的喘息症状及肺功能具有显著改善作用。廖桂香等人开展了一项纳入60例COPD患者的随机对照试验，其研究结果证实耳穴压豆可显著改善喘息症状20。此外，陈等人也证实该疗法可改善患者的呼吸系统症状及肺功能21,22。
