Hepatische Enzephalopathie

Hepatic encephalopathy develops from advanced liver dysfunction or portosystemic shunting, which allows gut-derived toxins to bypass hepatic detoxification. It may appear suddenly during acute liver failure or gradually in chronic conditions like cirrhosis. Precipitating factors include gastrointestinal bleeding, infections, electrolyte imbalances, high dietary protein, and procedures such as transjugular intrahepatic portosystemic shunt placement.

Ammonia, a byproduct of protein breakdown, accumulates when the impaired liver cannot effectively clear it. Ammonia crosses the blood-brain barrier and enters astrocytes, where glutamine synthetase converts it to glutamine. This causes osmotic swelling and cytotoxic edema, disrupting neural signaling and causing confusion and lethargy. Ammonia also alters cerebral blood flow and energy metabolism.

Neuroinhibitory substances with GABA-like activity accumulate due to impaired hepatic metabolism, increasing GABAergic signaling in the brain. These compounds enhance inhibitory neurotransmission, suppressing arousal, cognition, and motor control. Additionally, benzodiazepine-like substances that are normally processed by the liver accumulate systemically, further impairing neural function and causing disorientation and memory impairment.

Acute hepatic encephalopathy appears suddenly during fulminant hepatitis or acute liver failure, with rapid neurologic deterioration. Chronic hepatic encephalopathy develops progressively in chronic liver disease such as cirrhosis and portal hypertension. Both forms result from the brain's exposure to gut-derived toxins, but the timeline and underlying liver pathology differ significantly.

Gastrointestinal bleeding increases protein breakdown in the gut, generating more ammonia and other nitrogenous toxins. Infections trigger systemic inflammation and impair hepatic detoxification capacity. Both conditions increase the burden of gut-derived neurotoxins entering systemic circulation, overwhelming the already-compromised liver's ability to clear these substances and worsening neurologic symptoms.

Beyond ammonia, mercaptans, short-chain fatty acids, and manganese accumulate and contribute to neuronal dysfunction. These gut-derived toxins normally undergo hepatic metabolism but accumulate when liver function is impaired. Together with ammonia and GABA-like substances, they disrupt cerebral metabolism and neural signaling, producing the cognitive, behavioral, and motor impairments characteristic of hepatic encephalopathy.

Hepatic encephalopathy is reversible because it results from metabolic and toxic disturbances rather than permanent structural brain damage. When liver function improves or toxin accumulation is reduced through treatment, neurotransmitter balance is restored, cerebral edema resolves, and neural function recovers. This reversibility distinguishes it from neurodegenerative conditions and makes early recognition and management critical for patient outcomes.
