White Rami Communicantes

White rami communicantes are myelinated nerve branches that connect spinal nerves with the sympathetic trunk, providing a route for communication within the autonomic nervous system. Located mainly between spinal levels T1 and L2, they carry preganglionic sympathetic axons from the anterior rami into sympathetic ganglia, where fibers may synapse, ascend or descend, or continue through as splanchnic nerves; they also convey visceral sensory fibers. Their organization explains how sympathetic signals reach structures throughout the body despite originating in a limited spinal region. Studying white rami communicantes supports understanding of autonomic anatomy, visceral function, and the effects of nerve injury or regional neurological disease.

Here we present methodology for the production of a focal stroke in murine white matter by local injection of an irreversible endothelial nitric oxide synthase (eNOS) inhibitor (L-Nio). Presented are two stereotactic variations, retrograde neuronal tracing, and fresh tissue labeling and dissection that expand the potential applications of this technique.

We present a protocol for selective chemical lumbar sympathectomy (CLS), which inactivates only gray rami communicantes and not the sympathetic trunk. Selective CLS can help achieve therapeutic efficacy in vasodilation, sweat reduction, and pain relief that are comparable to conventional CLS, and serious complications, particularly ureteropelvic damages, can be reduced.
