Membranous Nephropathy

Membranous nephropathy is an immune-mediated kidney disease in which the glomerular filtration barrier becomes thickened and leaky, often causing proteinuria and nephrotic syndrome. In primary disease, autoantibodies commonly target podocyte antigens such as the phospholipase A2 receptor, forming subepithelial immune deposits that activate complement and disrupt podocyte function; secondary forms may accompany infections, autoimmune disorders, malignancy, or medication exposure. Clinical evaluation combines urine protein measurement, kidney function testing, serology, and sometimes renal biopsy to establish diagnosis and assess risk. These findings guide monitoring, supportive care, and immunosuppressive treatment while helping predict remission, progression, and complications such as thrombosis or infection.

Definition Diabetic nephropathy is a chronic kidney complication that results from prolonged hyperglycemia.Prevalence It is the most common cause of chronic kidney disease (CKD) and end-stage renal disease (ESRD) worldwide, affecting up to half of individuals with diabetes.Pathophysiology • Sustained hyperglycemia triggers multiple hemodynamic and metabolic changes in the kidney. • Early in the disease, increased renal blood flow and glomerular hyperfiltration occur due to afferent arteriolar...
