Pressure Sore Assessment

Pressure sore assessment is a structured clinical process for identifying, describing, and monitoring pressure-related skin and tissue injury, helping clinicians determine severity and guide care. It combines inspection and palpation with documentation of wound location, size, depth, tissue type, drainage, surrounding skin, and patient risk factors. Because prolonged pressure and shear can reduce local blood flow, assessment focuses on changes that indicate tissue damage, healing, or possible complications. In clinical practice, consistent assessment supports staging, prevention strategies, treatment planning, communication among care teams, and evaluation of outcomes over time.
