Hepatic Duct Anastomosis

Hepatic duct anastomosis is a surgical connection between a hepatic duct and another bile duct or a segment of intestine, restoring bile flow when the biliary system is disrupted or obstructed. The surgeon aligns the duct openings and secures their walls with fine sutures, often creating a hepaticojejunostomy in which the hepatic duct drains into the jejunum. This reconstruction is used to treat bile duct injuries, congenital abnormalities, strictures, tumors, and selected complications of liver transplantation. Successful anastomosis supports effective biliary drainage, while careful technique helps reduce leakage, infection, and later narrowing at the repair site.

Auxiliary liver transplantation provides a temporary support in acute hepatic failure, until regeneration of the failing liver. The heterotopic auxiliary liver transplantation (HALT) with portal vein arterialization (PVA) renders sufficient liver function. We developed an analogous technique in the rat, to examine the influence of the portal vein arterialization on the morphology and function of the graft.

To circumvent the difficulty in completing the anastomosis of the pancreatic duct and jejunal mucosa under laparoscopy for patients with a small pancreatic duct and soft and fragile pancreatic tissue, we introduce a reverse needle continuous suture technique for laparoscopic pancreaticojejunostomy of the pancreatic duct and jejunal mucosa.
