Prosthesis Patient Mismatch

Prosthesis-patient mismatch is a hemodynamic condition in which a replacement heart valve has an effective orifice area that is too small for the recipient’s body size, despite normal prosthetic function. After valve implantation, blood must pass through the undersized opening, creating higher transvalvular gradients and limiting the expected improvement in forward blood flow. Clinicians assess mismatch by calculating the indexed effective orifice area and interpreting it alongside symptoms, imaging, and pressure measurements. Recognizing this complication helps guide prosthesis selection, postoperative evaluation, and management, particularly in patients undergoing surgical or transcatheter aortic valve replacement.

Full-root aortic valve replacement by stentless aortic xenograft is a viable option in patients with small aortic roots. We describe, a technique for the full-root implantation of stentless aortic xenografts, with emphasis on the management of the proximal suture line and coronary anastomoses, and discuss its limitations and alternative options.

Body segmental inertial properties are required for inverse dynamics modeling. Using an oscillation and reaction board technique, inertial properties of below-knee prostheses were measured. Using direct measures of prosthesis inertia in the inverse dynamics model of the prosthetic leg resulted in lower magnitudes of resultant joint forces and moments.
