Tubular Atrophy

Tubular atrophy is the shrinkage, degeneration, or loss of epithelial cells lining renal tubules, a structural change that signals chronic kidney injury. Persistent ischemia, inflammation, obstruction, or toxic damage can injure tubular epithelial cells, impair regeneration, and promote tubular narrowing alongside interstitial fibrosis; these changes reduce reabsorption, secretion, and urine-concentrating capacity. Pathologists assess tubular atrophy in kidney biopsy specimens, often together with interstitial fibrosis, to characterize diseases such as chronic tubulointerstitial nephritis and transplant injury. Its extent can help estimate irreversible tissue damage, support diagnosis, and inform prognosis and clinical management in chronic kidney disease.
