Chronic Disease Management

Chronic disease management is the continuous, coordinated care of long-term conditions to control symptoms, prevent complications, and maintain quality of life. In clinical practice, it combines individualized treatment plans with regular monitoring of disease indicators, medication management, patient education, lifestyle support, and communication among patients and multidisciplinary care teams; clinicians adjust interventions as a person’s condition and goals change. This approach supports conditions such as diabetes, cardiovascular disease, asthma, and chronic kidney disease by promoting self-management and timely intervention. Effective management can improve adherence and functional outcomes while reducing acute exacerbations, avoidable hospitalizations, and healthcare burden.
