Clinical Assessment

Clinical assessment is the systematic collection and interpretation of information about a person’s health, used to identify problems, establish priorities, and guide care. It combines patient history, observations, physical examination, and, when appropriate, laboratory or imaging findings; clinicians integrate these data with symptoms, risk factors, and baseline status to form and refine diagnostic judgments. In clinical practice, assessment supports differential diagnosis, treatment planning, monitoring of response, and recognition of deterioration across primary care, hospitals, and specialty services. Standardized approaches and clear documentation improve communication, promote consistent decision-making, and provide a basis for evaluating outcomes while preserving attention to individual patient needs.
