Population Health

Population health is the study and improvement of health outcomes across groups of people, including the distribution of those outcomes and the factors that influence them. It combines clinical data with information about health behaviors, environments, social conditions, and access to care to identify patterns, risk factors, and preventable disparities. In clinical practice, population health supports risk stratification, preventive care, chronic disease management, and coordinated interventions for defined patient groups. By connecting individual patient care with community and public health strategies, it helps healthcare systems allocate resources effectively, measure outcomes, and improve equity and quality of care.
