Collateral Artery Development

Collateral artery development is the formation and enlargement of alternative blood vessels that help maintain tissue perfusion when a major artery becomes narrowed or blocked. In response to reduced pressure gradients and increased fluid shear stress, endothelial cells in pre-existing arterial connections activate signaling pathways that promote cell proliferation, inflammation, and outward remodeling, a process known as arteriogenesis. This vascular adaptation can limit ischemic injury in coronary, cerebral, and peripheral artery disease, although its effectiveness varies among individuals. Understanding collateral growth supports risk assessment, treatment planning, and research into therapies that improve blood flow without replacing conventional revascularization.

Surgical correction of ALCAPA is highly recommended, regardless of age or the degree of intercoronary collateralization. This protocol presents a technique for the direct re-implantation of adult-type ALCAPA into the aorta to re-establish the dual-coronary perfusion. Whenever feasible, direct re-implantation is preferred to other surgical correction techniques.

Cerebral perfusion monitoring has been demonstrated to improve accuracy in ischemic stroke models. Technical difficulties often limit the use of this essential tool for cerebrovascular research.
