Endobronchial Ultrasound

Endobronchial ultrasound (EBUS) is a minimally invasive imaging technique that uses ultrasound during bronchoscopy to visualize structures surrounding the airways, particularly mediastinal and hilar lymph nodes. A small ultrasound transducer at the tip of a flexible bronchoscope emits high-frequency sound waves, and returning echoes form real-time images; an integrated needle can then collect tissue through the airway wall. EBUS-guided transbronchial needle aspiration supports diagnosis and staging of lung cancer, evaluation of enlarged lymph nodes, and investigation of infections or inflammatory disease. By combining imaging and sampling in one procedure, EBUS can improve diagnostic precision while reducing the need for more invasive surgical approaches.

Endobronchial ultrasound-guided sampling using transbronchial needle aspiration plays a key role in staging and diagnosing lung cancer. We propose a systematic stepwise approach dividing the procedure into six landmarks that should be taught to new operators.

Using the Endoscope for Endobronchial Ultrasound in the Esophagus

Transesophageal ultrasound (EUS-B) is a safe and feasible procedure using the echoendobronchoscope (EBUS) in esophagus and stomach. After identifying six anatomical landmarks, additional structures can be identified and biopsied, sparing subsequent diagnostic sessions. Thus, EUS-B is an ideal continuation of bronchoscopy and EBUS in diagnosing lung cancer and other diseases.

Substernal thyroid lesions are common, and need to be differentiated from malignancy. Obtaining percutaneous fine needle biopsy is not possible due to its retrosternal location. This article proposes a protocol for biopsy of substernal thyroid lesions using Endobronchial Ultrasound-guided Transbronchial Needle Aspiration (EBUS-TBNA).

A Novel Application of Musculoskeletal Ultrasound Imaging

We describe a new ultrasound-based vector tissue Doppler imaging technique to measure muscle contraction velocity, strain and strain rate with sub-millisecond temporal resolution during dynamic activities. This approach provides complementary measurements of dynamic muscle function and could lead to a better understanding of mechanisms underlying musculoskeletal disorders.
