Surgical Instrument Rescue

Surgical instrument rescue is the controlled recovery, replacement, or safe removal of an instrument that becomes inaccessible, contaminated, damaged, or difficult to use during an operation. The process involves identifying the instrument’s location and condition, maintaining visualization of the operative field, and using an appropriate retrieval or grasping method while protecting surrounding tissue and preserving sterility. In medicine, effective instrument rescue can reduce procedural delays, limit contamination, support accurate instrument counts, and help prevent retained surgical items. Understanding these principles is especially relevant to operative planning, minimally invasive procedures, surgical education, and patient-safety protocols.

Capsulorhexis is an important step in phacoemulsification surgery. A surgeon creates a continous curvilinear tear on the anterior lens capsule by controlling the tearing vector forces. A peripherally extended tear is a serious complication. This video demonstrates a novel technique of rescuing capsular radial tear out using a cystotome.
