Medication Review

Medication review is a structured clinical assessment of a patient’s complete medication regimen to ensure that each medicine is appropriate, effective, safe, and used as intended. The process combines medication reconciliation with a review of indications, doses, adherence, adverse effects, drug interactions, duplicate therapy, and patient-specific factors such as renal or hepatic function. Clinicians use medication review to identify and resolve drug-related problems, support shared treatment decisions, and reduce preventable harm. It is especially valuable during care transitions, for people taking multiple medicines, and when treatment changes or new symptoms require reassessment.
