Pernicious Anemia

Pernicious anemia is a type of megaloblastic anemia caused by vitamin B12 deficiency, typically resulting from impaired absorption rather than inadequate dietary intake. In most cases, autoimmune destruction of gastric parietal cells or intrinsic factor prevents vitamin B12 from being absorbed in the terminal ileum, disrupting DNA synthesis and producing enlarged, dysfunctional red blood cells. Clinicians evaluate macrocytosis and related findings with blood counts, vitamin B12 testing, and assessments for intrinsic-factor or parietal-cell antibodies, while distinguishing the condition from other causes of B12 deficiency. Prompt diagnosis and replacement therapy can correct anemia, limit progressive neurological injury, and guide long-term clinical monitoring.

An accurate hemoglobin estimation method is lacking at the point of care and may hinder population-based programs for treating anemia. Therefore, we developed a point-of-care method based on pooled capillary blood and an auto-analyzer integrated into a custom software application to categorize the hemoglobin values into different grades of anemia.

Twin-to-twin transfusion syndrome and twin anemia polycythemia sequence are two potentially devastating problems in perinatal medicine. Both disorders occur only in monochorionic twins and result from unbalanced blood flow through placental vascular anastomoses. We provide a simple protocol to accurately evaluate the presence of vascular anastomoses using colored dye injection of placental vessels after birth.

Here, we introduce a novel approach-Laparoscopic Extracorporeal Knot-Tying for Uterine Vessel Occlusion during Hysterectomy with Cervical Cerclage in Large Uteri. A laparoscopic extracorporeal knot-pusher is used to perform cervical cerclage, achieving complete occlusion of uterine arteries. Thereafter, the corpus uteri are excised to enhance visibility and facilitate the operative field.
