End-neuroma Treatment

End-neuroma treatment comprises medical, rehabilitative, and surgical approaches for managing a painful neuroma, a mass of disorganized nerve fibers that forms after peripheral nerve injury or transection. When axons attempt to regrow but lack a normal pathway, they may become entangled with scar tissue and generate spontaneous or mechanically triggered electrical activity, producing pain, tenderness, or hypersensitivity. Care may include desensitization therapy, medications, injections, and surgery to remove or redirect the neuroma, including nerve relocation, targeted muscle reinnervation, or regenerative peripheral nerve interfaces. Effective treatment can reduce neuropathic pain, improve function, and support prosthetic use and recovery after amputation or trauma.

This protocol describes the tibial neuroma transposition model, which entails a lesion of the tibial nerve with subsequent transposition of the proximal nerve end toward a subcutaneous pretibial or lateral position. Behavioral testing of neuroma pain and plantar hyperalgesia is quantified using Von Frey monofilaments.
