Extremity Muscle Testing

Extremity muscle testing is a clinical assessment of strength and motor function in the muscles of the arms and legs, helping identify weakness and its possible neurological or musculoskeletal causes. During testing, a clinician observes a specific movement, positions the limb against gravity, and applies controlled resistance, commonly assigning a grade from 0, indicating no visible contraction, to 5, indicating normal strength. The findings help localize nerve, spinal cord, peripheral nerve, muscle, or joint involvement and establish a baseline for care. Repeated assessments can track recovery, guide rehabilitation, and support diagnosis in conditions such as nerve injury, stroke, and neuromuscular disease.

This protocol describes the process for performing a neurophysiological assessment of the lower extremity muscles, tibialis anterior and soleus, in a standing position using TMS in people post-stroke. This position provides a greater probability of eliciting a post-stroke TMS response and allows for the use of reduced stimulator power during neurophysiological assessments.

Individuals with chronic ankle instability (CAI) exhibit postural control deficiency and delayed muscle activation of lower extremities. Computerized dynamic posturography combined with surface electromyography provides insights into the coordination of the visual, somatosensory, and vestibular systems with muscle activation regulation to maintain postural stability in individuals with CAI.

Mobile Game-based Virtual Reality Program for Upper Extremity Stroke Rehabilitation

Here, we present a protocol to develop and apply a mobile game-based virtual reality program for the recovery of upper limb dysfunction in patients with stroke. The present study shows that the mobile program is feasible and effectively promotes upper limb recovery in stroke patients.

We describe an in vivo protocol to measure dorsiflexion of the foot following stimulation of the peroneal nerve and contraction of the anterior crural compartment of the rat hindlimb. Such measurements are an indispensable translational tool for evaluating skeletal muscle pathology and tissue engineering approaches to muscle repair and regeneration.
