Abdominal Wall Transplant

Abdominal wall transplant is a form of vascularized composite allotransplantation that replaces complex, nonreconstructable abdominal wall tissue with a donor graft containing skin, muscle, fascia, blood vessels, and sometimes nerves. Surgeons connect the graft’s blood vessels to the recipient’s circulation, while immunosuppressive therapy limits immune rejection of the transplanted tissues. The procedure can restore abdominal wall integrity and protection of internal organs when conventional mesh or flap reconstruction is unsuitable, particularly after extensive trauma, tumor removal, infection, or repeated abdominal surgery. It may also be performed alongside intestinal or multivisceral transplantation, offering functional and structural reconstruction for patients with severe abdominal defects.

We describe a novel technique for heterotopic abdominal heart-lung transplantation (HAHLT) in rats. The transplant configuration results in a partially loaded graft circulation, allowing direct functional assessment. This model may be employed for acute or chronic studies of function and immunologic status of the transplanted graft.
