Airway Mucosa Anesthesia

Airway mucosa anesthesia is the topical or regional numbing of sensory surfaces lining the nose, mouth, pharynx, larynx, and trachea to reduce discomfort and protective reflexes during airway management and diagnostic procedures. Local anesthetics temporarily block voltage-gated sodium channels in sensory nerve membranes, preventing the initiation and transmission of pain signals as the agent contacts the mucosa. Clinicians use this approach during bronchoscopy, awake intubation, endoscopic examination, and other interventions to suppress gagging, coughing, and laryngeal responses while helping preserve patient cooperation and spontaneous breathing. Appropriate dosing and administration are essential to balance procedural comfort with airway safety.

Topical Airway Anesthesia for Awake-endoscopic Intubation Using the Spray-as-you-go Technique with High Oxygen Flow

The topical anesthetic lidocaine was atomized using a high oxygen flow through the working channel of a flexible intubating endoscope to achieve topical airway anesthesia for awake endotracheal intubation. We prefer this modified spray-as-you-go technique for endoscopic intubation to classical bolus application because of higher patient satisfaction and better compliance.
